Understanding external ventricular drainage.

An understanding of the ventricular system and cerebrospinal fluid production is essential to providing appropriate care to the patient undergoing external ventricular drainage. A brief operative procedure introducing an external ventricular drain is a valuable treatment for increased intracranial pressure; however, its value must not obscure the associated risk of infection. Critical nursing care associated with successful patient outcomes includes both comprehensive neurological assessments and appropriate external ventricular drainage system maintenance.

Enhanced clearance of vancomycin by hemodialysis in a child.

Clearance of vancomycin by hemodialysis has previously been reported to be negligible. We describe a child undergoing chronic hemodialysis using a hollow fiber dialyzer with cellulose acetate membrane. When the patient was treated with intravenous vancomycin for staphylococcal bacteremia, the serum vancomycin half-life was found to decrease by more than 90% during each course of hemodialysis. We conclude that, contrary to prevailing opinion, vancomycin can be rapidly cleared by hemodialysis in the small pediatric patient, and that the dosage should be adjusted accordingly.

Wernicke-Korsakov syndrome lesions in coronial necropsies.

The clinical and pathological features of the Wernicke-Korsakov syndrome are described in 12 cases in which the syndrome had not been recognised before death. In a 1 year period there were 9 cases (5.3%) among 169 necropsies ordered by the coroner in unexpected or unexplained non-violent death. The characteristic brain lesions were grossly apparent in 5 cases and identified only histologically in 7 cases. The findings suggest that in cases of coma or patients found dead who might have been alcoholics, adequate postmortem examination of the brain is likely to demonstrate the lesions of the Wernicke-Korsakov syndrome in a significant number.
