[Leiomyosarcoma of the stomach].

A 35-year old woman showed gastrointestinal bleeding right after normal pregnancy. With endoscopy of the GI-tract with biopsy, ultrasound, conventional radiology of the stomach and computed tomography a gastric leiomyoma was diagnosed. After en-bloc resection of tumor, part of the stomach and spleen, histologic examination and size of tumor changed diagnosis in gastric leiomyosarcoma. Patient died 15 month postoperatively on systemic metastasis. Case report and review of the literature show difficulties in diagnosis and prognosis of gastric leiomyosarcoma.

[The dynamic condylar screw (DCS) in proximal femoral fractures. Initial experiences].

Beginning January 1st 1987 19 fractures of the proximal femur were stabilized utilizing the "dynamic condylar screw" (DCS). During or after reduction of the fracture the DCS is accomplished by a supporting plate. In the cases we applied the DCS hitherto employment of a bone distractor of complementary cancellous bone graft has been mandatory. Although we feel encouraged by our preliminary results the DCS will have to undergo arduous clinical trial.

[Malignant teratoma of the thyroid gland].

Two years after a bilateral subtotal thyroidectomy because of nodular goitre a 45-year-old woman noticed a firm nodule on the right side of her neck. Because malignant cells were suspected on cytological examination of a fine-needle biopsy, thyroidectomy was performed which revealed an immature malignant teratoma of the thyroid. A 4 cm local recurrence was noted by computed tomography only three weeks later. Despite chemotherapy (four cycles of vinblastine, cisplatin and bleomycin), radiotherapy to the neck (total dose of 60 Gy) and surgical excision of the infiltrated sternocleidomastoid muscle, bone, liver and lung metastases occurred within 10 months. Four weeks later the patient died of global cardiac failure and tumour cachexia.

[Luxations in the sternoclavicular joint].

Primary open reduction and fixation with Kirschner wires and/or fascia lata or PDS is recommended in the treatment of acute traumatic luxations of the sternoclavicular joint. If not bent or secured against migration, K-wires may be the reason for lethal postoperative complications. We report on 7 own patients with luxation of the sternoclavicular joint and discuss the problems in diagnosis, operative and postoperative treatment of this rare lesion.

Lysolecithin concentration in pancreatic tissue during therapy with phospholipase A2-inhibitors in acute necrotizing pancreatitis.

Four hours after hemorrhagic necrotizing pancreatitis was induced in experimental animals the concentration of lysolecithin in pancreatic tissue rose (p = 0.0025). Parenteral administration of unspecific inhibitors of phospholipase A2, such as chlorpromazine, gabexat mesilat and CaNa2 EDTA, can neither inhibit elevation of the enzyme nor influence the course of the disease.

[Fibrolamellar liver cell cancer. A case report].

Fibrolamellar hepatocellular carcinoma of the liver is a rare variant of hepatocellular carcinoma with characteristic morphological patterns and a good prognosis. Preoperatively the tumor is rarely diagnosed. Surgical treatment is resection, hemihepatectomy or transplantation of the liver. We report a case of a 51 years old patient with fibrolamellar hepatocellular carcinoma of the liver.

[Intestinal perforations in the newborn infant].

Between 1976 and March 1985 16 neonates with gastrointestinal perforations were treated. The most common site of perforation was the ileo-coecal region, the most frequent causes of perforation were due to intestinal obstruction, necrotising enterocolitis and so-called spontaneous perforations (ischemic perforations). The surgical procedure depended on the site and cause of the perforation, taking the general condition into account. 7 out of the 16 newborns (44%) died due to sepsis and septic complications.

[Surgical therapy of ascites--results of peritoneovenous drainage with the Hakim-Cordis ascites valve].

Peritoneo-venous shunting of ascites was performed in 16 patients using the Hakim-Cordis ascites valve system. After an average of 4.3 months four patients presented with closure of the valve or deconnection of the system that is to say, complications caused by the valve system. Early postoperative complications occurred in 7 patients; they usually can be avoided. Correct placement of the system has to be controlled intraoperatively by x-ray examination. If the ascites fluid contains much protein, repeated depressing and releasing of the antechamber has to be performed in order to avoid obstruction. The probability of diffuse intravascular coagulation (DIC) after valve placement can be predicted by measuring certain coagulation parameters preoperatively after intravenous infusion of ascites. We conclude, that because of high flow rate, easy placement of the small-sized catheter and up to now good clinical results the Hakim valve system offers an enrichment of the therapeutic methods in resistant ascites.

Paramagnetic changes in pulmonary tumors.

Electron spin resonance studies on healthy and tumorous human lung samples have been conducted in order to determine possible differences in free radical concentration and shaped of the spectra between the different sections of the lung. It could be shown that in healthy lung tissue the signal caused by the semidehydroascorbate (SDA) radical is not prominent because of the prevailing high partial oxygen pressure. On formation of a tumor, the spin concentration increases, possibly due to the higher metabolic rate; here, the SDA peak is also more pronounced which indicates alterations in the interaction between cell constituents and ascorbic acid. Within the tumor, the spin concentration is considerably reduced which is probably caused by a still higher concentration of ascorbic acid. Addition of ascorbic acid to the different lung specimens enhanced the just described effect while oxidizing substances, such as H2O2, reversed it.
