Therapeutic effects of antidepressants in chronic pain.

Depression and chronic pain syndromes are often associated. Over the last twenty years there has been a number of controlled and uncontrolled studies evaluating the efficacy of antidepressants in various pain problems. The administration of antidepressant medications in the management of chronic pain has become a commonly prescribed therapeutic modality in the treatment of this complex syndrome. This paper reviews the clinical studies in which antidepressants have been used to control chronic pain, summarizes the results of the clinical studies, and comments on the mechanism of action of antidepressants in chronic pain.

Long-term use of narcotic analgesics in chronic pain.

The use of narcotic analgesics have been avoided by clinicians in patients with chronic pain syndromes. Uncertainty as to the etiological cause of chronic pain, development of addiction and habituation and associated psychological and behavioral symptoms found in chronic pain states which are not amenable to narcotic medications are the major reasons narcotics are not prescribed. This communication describes the long-term use of low dose narcotic analgesics as a treatment component of a comprehensive pain management program and addresses the questions of whether or not narcotic efficacy is maintained in long-term use, improvement of patients' function is continued and side effects develop as a result of this treatment.

Differentiation of depression from chronic pain with the dexamethasone suppression test and DSM-III.

The concept of chronic pain has become enmeshed with depression. In an attempt to unravel this complex relationship, the authors studied a uniform group of 42 patients with chronic pain, i.e., patients who had chronic low back pain with defined organic pathology, in relation to the dexamethasone suppression test (DST). The results were analyzed in relation to the presence or absence of major depression and cortisol suppression. Forty-one percent of the patients with major depression had abnormal cortisol responses to dexamethasone administration; all patients without major depression had normal responses. These results suggest that chronic pain patients differ from patients with major depression and a positive DST.

Management of grief in the hypochondriac.

This paper illustrates the presentation and management of a grief reaction in a chronic hypochondriacal patient seen in a family medicine office. The manifestations of grief in the hypochondriacal patient are influenced by the unique characteristics of the chronic, somatic complainer in whom the grief reaction appeared as an intensification of the chronic complaints. The treatment of the grief reaction in these patients should take into account the techniques for managing the hypochondriacal patient.

Referral of patients from primary care physicians to a community mental health center.

The referral process between primary care physicians and a community mental health center was examined in a community which lacks a geographically based psychiatrist. The method included inspection of referral completion rate and evaluation or patients' characteristics and attitudes concerning mental health care. The findings indicated that only 10 per cent of referred patients who have had no prior contact with the community mental health center completed the referral. Reasons for this low referral completion rate are discussed.
