Cardiovascular pharmacology of ASL-7022. III. Peripheral vascular adrenergic mechanisms.

The peripheral vascular actions of i.v. administered ASL-7022 were investigated in anesthetized, open-chest dogs and in isolated hindlimbs of normal, acute baroreceptor-denervated and spinal dogs. ASL-7022 decreased diastolic arterial blood pressure in open-chest dogs, an effect which was inhibited by ganglion blockade (hexamethonium bromide, 10 mg/kg i.v.). In hindlimbs from control animals, ASL-7022 produced vasodilation. Propranolol (1.0 mg/kg i.v.) reduced but did not eliminate vasodilation in these preparations but combined beta adrenergic and ganglion blockade converted responses to vasoconstriction. ASL-7022 induced greater vasodilation in hindlimbs from acute baroreceptor-denervated animals than in control animals. In acute baroreceptor-denervated preparations ganglion blockade eliminated vasodilation, propranolol partially blocked vasodilation and combined beta adrenergic and ganglion blockade converted responses to vasoconstriction. In spinal dogs i.v. infusion of low doses of ASL-7022 induced small increases in perfusion pressure; higher doses produced small decreases in perfusion pressure. The compound caused only vasoconstriction in propranolol-pretreated hindlimbs and caused only vasodilation in phentolamine-pretreated hindlimbs. ASL-7022 also dose dependently inhibited vasoconstrictor responses to electrical stimulation of the lumbar sympathetic chain and to exogenously administered norepinephrine. The data suggest that ASL-7022 blocks sympathetic vasoconstriction by either inhibiting the sympathetic nervous system or by inducing postsynaptic alpha adrenoceptor blockade. The compound also produces beta adrenoceptor-mediated vasodilation and, under appropriate pharmacological conditions, can be demonstrated to produce alpha adrenoceptor-mediated vasoconstriction.

Sustained effects of transdermal nitroglycerin in patients with angina pectoris.

The antianginal efficacy of a transdermal therapeutic delivery system for nitroglycerin (TNG) was compared with that of placebo in a double-blind crossover study. Twenty-five patients with stable angina pectoris were evaluated. The transdermal system delivered 5 mg of nitroglycerin over a 24-hour period and was applied once every 48 hours. Treadmill exercise testing (Bruce protocol) was done 48 hours after the patch was applied in the first phase of the crossover and at the conclusion of the second phase of the crossover, 48 hours after the final dose of the second treatment. Exercise performance was significantly improved (P less than 0.05, analysis of covariance) with TNG as compared with placebo, as were frequency of episodes of angina and nitroglycerin consumption (P less than 0.05, analysis of variance). The incidence of mild-to-moderate headache in patients was greater during treatment with TNG (20%) than during placebo treatment (6.7%). Four cases of mild transient dermatitis and occasional reports of dizziness, lightheadedness, and nausea were noted.

Changing patterns in the management of locally advanced breast cancer: a preliminary report.

Over the 8 years from 1976 to 1983, 28 patients with stage IIIB (T4a,b,c, NX-2, M0) and inflammatory breast cancers without evidence of disseminated disease at the time of diagnosis were treated at Ellis Fischel State Cancer Center in Columbia, Mo. This group comprised 4% of all cases of primary breast cancer seen during that period. Radiotherapy was the primary treatment in one half of the group during the first 4 years of the study (group A). Since 1981, locally advanced breast cancer has been treated by multidrug chemotherapy followed primarily by mastectomy (group B). The rate of local control was the same for both groups (78%). However, the median survival for group A was 11 months, only one patient being alive 5 years after diagnosis, whereas in group B, 12 of 14 patients were alive and clinically free of disease 9 to 31 months after diagnosis with a median follow-up of 16.5 months. This preliminary report confirms recent findings, supporting the use of polychemotherapy followed by mastectomy in the management of patients with locally advanced breast cancer.

Nutritionally relevant supplementation of vitamin B6 in lactating women: effect on plasma prolactin.

Pharmacologic doses of vitamin B6 administered to lactating women have been reported to suppress plasma prolactin. As a result, some physicians have recommended restriction of vitamin B6 intake for lactating women. In the present investigation, 20 lactating women were given supplemental doses of vitamin B6, 0.5 to 4.0 mg/d, beginning 24 hours after delivery. Plasma prolactin, plasma pyridoxal phosphate, and breast milk total vitamin B6 concentrations were determined during the first 9 months postpartum. Women receiving the supplement of 4.0 mg compared with 0.5 mg of vitamin B6 per day had significantly higher plasma pyridoxal phosphate (P less than .01) and breast milk total vitamin B6 concentrations (P less than .05) beginning at 1 month postpartum and continuing through the duration of the study. Plasma prolactin concentrations were not significantly different between the two groups. The percentage of all women, regardless of treatment, in whom lactation persisted at 1 and 2 weeks and 1, 3, 6, and 9 months were 100%, 100%, 100%, 90%, 80%, and 65%, respectively. All women who ceased to lactate during the study reported doing so by choice. Nutritionally relevant doses of vitamin B6 elevated plasma pyridoxal phosphate and breast milk total vitamin B6 concentrations of lactating women without reducing plasma prolactin concentration or halting lactation.

Synthesis and antiarrhythmic and parasympatholytic properties of substituted phenols. 2. Amides.

Thirty amides patterned after the antiarrhythmic drug changrolin were synthesized and their antiarrhythmic and parasympatholytic activities were assessed. There was no correlation between antiarrhythmic and parasympatholytic activities. Several of the amides were found to be potent antiarrhythmic agents that possessed low parasympatholytic activity. All of the compounds appear to act by a class I mechanism.

Apparent vitamin B6 deficiency in sickle cell anemia.

In 16 patients with sickle cell anemia, plasma pyridoxal phosphate (PLP) concentrations were significantly lower than in 16 controls (p less than 0.0001) whereas sickle cell anemia erythrocyte PLP concentrations were significantly elevated (p less than 0.0005), possibly reflecting a greater affinity of PLP to the sickle Hb beta chain compared to the normal beta chain. Oral supplementation of five patients with 50 mg pyridoxine twice daily for 2 months resulted in increased plasma and erythrocyte PLP levels and a slight, but not significant, increase in erythrocyte cell number, Hb, concentration, and hematocrit. In one subject there was also a reduction in the frequency and duration of painful crises and a virtual elimination of hospitalizations for the treatment of the painful crises. Since pyridoxal and PLP have been shown to have antisickling properties in vitro, these studies suggest that pyridoxine supplementation may also be of therapeutic benefit in vivo in sickle cell anemia.

Analyzed vitamin B-6 intakes of pregnant and postpartum lactating and nonlactating women.

Daily dietary food composites were collected for three consecutive days from 36 women (23 who planned to breast feed their infants and 13 who planned to formula feed their infants) at approximately the 37th week of pregnancy and at one, three, and six months postpartum. When the food composites were collected, each woman also kept detailed food intake records. Daily protein and energy intakes were calculated from the records. Representative aliquots of the homogenized food composites were analyzed microbiologically for total vitamin B-6. The data from the composite analyses indicate that the daily dietary intakes of vitamin B-6 for both groups of women in late pregnancy through six months postpartum were substantially less than the Recommended Dietary Allowances (RDAs). During pregnancy, the women who planned to breast feed their infants consumed diets which provided 54% of the RDA. The women who planned to formula feed their infants consumed diets which provided only 44% of the RDA for vitamin B-6 for pregnant women. At one, three, and six months postpartum, the diets of the lactating women provided 61% of their RDA for vitamin B-6, and the diets of the nonlactating women provided a mean of 46%. Of the 402 daily dietary composites analyzed, only 6% met or exceeded the RDA for vitamin B-6 for the women while they were pregnant or postpartum, either lactating or nonlactating.

Synthesis and antiarrhythmic and parasympatholytic properties of substituted phenols. 1. Heteroarylamine derivatives.

Twenty-four structural derivatives of the antiarrhythmic drug changrolin were synthesized and tested for antiarrhythmic and parasympatholytic activities. It was found that while the bis(pyrrolidinylmethyl)phenol pattern of changrolin appeared to be optimal in this series, a wide latitude existed for the heteroaryl substituent for maintaining good antiarrhythmic activity. Further, the antiarrhythmic and parasympatholytic activities tended to exhibit parallel changes.

Dietary magnesium intake and the concentration of magnesium in plasma and erythrocytes of postpartum women.

Dietary magnesium intakes and concentrations of magnesium in plasma and erythrocytes were determined in 23 lactating and 13 nonlactating healthy pregnant women from 37 wk of gestation through 6 mo postpartum. Food composites, collected for 3 days, were analyzed for magnesium, and energy intake was calculated from 3-day dietary records. The lactating women consumed significantly greater (P less than .05) amounts of magnesium and energy than the nonlactating women. Mean postpartum intake of magnesium was 55% of the recommended dietary allowance (RDA) for lactating women and 48% of the RDA for nonlactating women. Neither lactation nor length of lactation significantly affected plasma magnesium concentration or erythrocyte magnesium concentration. Dietary magnesium intake was not significantly correlated with the level of magnesium in either the plasma or erythrocytes.

Forms of vitamin B6 in human milk.

A previously developed high performance chromatographic method has been modified slightly and used to determine the forms of vitamin B6 in human milk. The chromatographic traces are free of compounds that would interfere with the two principal forms found, pyridoxal and pyridoxal phosphate. The method is fast and reliable; it yields recoveries of from 90 to 106% for the vitamers and agrees with results obtained on the same samples with the standard microbiological assay.

Dietary zinc intake and zinc concentrations of plasma, erythrocytes, and breast milk in antepartum and postpartum lactating and nonlactating women: a longitudinal study.

The zinc status of a group of 23 lactating and 13 nonlactating women was assessed longitudinally from 37-wk gestation through 6 months postpartum. Dietary zinc intake was determined by chemical analysis of 3-day duplicate plate food composites. In addition, zinc concentrations were measured in plasma, erythrocytes, and breast milk. The mean dietary zinc intake of both groups of postpartum women as determined by direct analyses was 42% of the Recommended Dietary Allowances or less. The plasma and erythrocyte zinc concentrations were not significantly different between the lactating and nonlactating women. Plasma zinc concentration increased from the time of delivery to one month postpartum for both groups and did not significantly increase further through 6 months postpartum. Erythrocyte zinc decreased from delivery through 6 months postpartum for both groups. Breast milk zinc concentration decreased with the duration of lactation. The calculated zinc intake of infants from breast milk was less than 50% of the Recommended Dietary Allowances. There was no correlation of maternal dietary zinc intake or maternal plasma and erythrocyte zinc with the concentration of zinc in breast milk.

Comparison of infarct size produced by coronary artery occlusion in purebred beagle and mixed-breed dogs.

This investigation demonstrated a significant difference in infarct size following coronary artery occlusion between purebred beagles and mixed-breed dogs. Myocardial infarctions were produced by a two-stage ligation of the left anterior descending artery (LAD). Myocardial infarct size, expressed as percent of left ventricular mass, was determined at 24 h by nitro blue tetrazolium staining. Infarct size was significantly smaller (12.5 +/- 3.4%) and more variable (coefficient of variation 72%) in beagles than in mixed-breed animals (28.0 +/- 2.4% infarct size, 23% coefficient of variation) (p less than 0.001). Ectopic activity at 24 h was similar for both beagles and mixed breed, 92 +/- 7 and 92 +/- 2% of total heart rate, respectively. Thus this study indicates that the breed of dog (i.e., purebred beagles versus mixed-breed animals) can be an important variable in the study of infarct size resulting from coronary artery occlusion.
