[Nodular fasciitis of the upper arm in a sports marksman].

Nodular fasciitis is a benign, fibroproliferative, tumorlike lesion. Histological differentiation from sarcomas can be difficult. Surgical excision is most effective, recurrences are rare. This case report deals with the onset and natural course of nodular fasciitis in the upper arm of a marksman. When instructing young people in shooting, he had to pace different weapons against the upper third of his arm instead against the shoulder. Here a tender swelling developed. After extirpation he reported pain relief. A recurrence has not been observed so far.

[Intraosseous lipomas (author's transl)].

This is a report on four intraosseous lipomas. One in the tibia diapysis of an infant, one in the fibula head, one in the proximal epimetaphysis of the humerus, and one in the calcaneus of three adults. Together with fourty-two intraosseous lipomas which have been reported by other authors, they are evaluated via clinical, radiological, and pathological aspects. The metaphyses of long bones and the calcaneus are common localizations of intraosseous lipomas. There may be no clinical symptoms or non-specific ones. The history may last for decades. Trabecular structures are noted usually on x-ray and almost always on histological examination. A capsule, capillaries, and the lack of sinusoids and haemopoietic bone marrow demonstrate the neoplastic nature of intraosseous lipomas. Because of their good prognosis intraosseous lipomas may be treated conservatively or by curettage.

[Dependence of hippuric acid excretion in urine on the toluol concentration and the respiratory volume per minute].

In inhalative toluol loads lasting one hour a dependence of the secretion of hippuric acid on the concentration of toluol in the inspiration air influenced by the respiratory minute volume was shown. It is concluded that the estimation of the hippuric acid from a one- to two-hours' collective urine of the working shift may give actual references to the toluol load. The increase of hippuric acid in the urine of 1.5 g/l after toluol exposition refers to the transgression of the MAK-values according to TGL 22310.

[Congenital diaphragmatic eventration].

The nomenclature of congenital defects of the diaphragm is not unequivocal. Congenital eventration means the incomplete ingrowth of muscle fibers between the pleural and peritoneal sheet of the diaphragm during the embryonal period. The differential diagnosis of eventration and hernia may be very difficult. The adequate surgical treatment differs. As concomitant congenital defects of the ipsilateral lung often occur, congenital eventration should be operated by transthoracic approach. The prognosis of symptomatic eventration is poor in newborns and good in adults. This is illustrated by two own cases. The one deals with a newborn who had a large congenital eventration of the left diaphragm; the other has been reported detailed elsewhere.

Chondrosarcoma of the first metatarsal--primary or secondary to enchondroma?

Enchondromas are said to be extremely rare precursors of secondary chondrosarcomas. Peripheral enchondromas of the hand and foot may be considered as benign, even when the histomorphological study reveals pleomorphic features with atypical nuclei. Our case deals with a 56 year old woman who had an enchondroma of the first metatarsal. Its distinct histopathological signs of focal malignant transformation were only seen in later review. Nine months after curettage and autologous bone-grafting a chondrosarcoma was diagnosed. The enbloc-resection of the medial foot provided adequate surgical treatment of the chondrosarcoma.

[Hyperostosis of the sterno-clavicular joint - an enthesopathy?].

Erosion of the insertions of tendons and ligaments in bone (i.e. of the enthesis) due to chronic aseptic inflammation causes reparative ossification. With regard to our clinical and radiological findings in ten patients hyperostosis of the sterno-clavicular joint seems to be the late stage of such an enthesopathy. Pain and swelling of the soft tissues and an elevated erythrocyte sedimentation rate are characteristics of this seronegative rheumatic disease. For visualizing the extraarticular changes in the sterno-costo-clavicular region tomography is necessary. Early diagnosis and consequent antiphlogistic therapy should remit pain and help avoid hyperostosis.
