Point/counterpoint. Centers of excellence. Ranae Dahlberg vs. David Plocher.

This issue's Point/Counterpoint considers the pros and cons of centers of excellence. On the pro side of the debate is Ranae Dahlberg, RN, BSN, Director, Quality Management, United Resource Networks (URN), a Minneapolis-based transplant management company owned by United HealthCare. URN, which began operating in 1990, has a network client base 337 customers. Ms. Dahlberg argues that though expensive, well-managed organ transplant programs have demonstrably improved outcomes. Dissenting is David Plocher, MD, a physician and consultant with Ernst & Young LLP, Minneapolis. He argues that outcomes data often are misleading depending on the criteria used to evaluate patient results and promised cost savings also may be illusory.

Mandibular trauma: secondary problems in reconstruction.

Cosmetic and functional restoration of the fractured mandible in the great majority of cases is the sine qua non of therapeutic success. Not only must an acceptable low complication rate be obtained, but when present, their complex nature must be understood from the onset. Successful treatment of complications is a multifaceted problem requiring planned, interdisciplinary, often times staged reconstruction. The radiologist and dental prosthodontist make valuable contributions towards the surgeon's ultimate rehabilitation of the patient. A three year experience of 111 mandibular fractures treated on the UCSD Otolaryngology Service is characterized. Sixteen complications nine of which were seen after initial treatment elsewhere provide the focus of this report. Special emphasis is given to the pre-treatment planning and surgical techniques necessary to correct malunion and nonunion. Particular advantages achieved by the use of osteogenic autogenous marrow, cellulose acetate filters and Vitallium (chrome cobalt) trays are detailed.

Pairing and segregation of the sex chromosomes in X1X2-males of Dysdercus intermedius with a note on the kinetic organization of heteropteran chromosomes.

The existence of an X1X2-mode of sex determination is confirmed by a study of all meiotic stages in the male cotton stainer (X1X2) and pertinent stages in the female (X1X1X2X2). In the male, the X-chromosomes are heterochromatic and pair end-to-end in early meiotic prophase. At diakinesis, they disjoin and align side-by-side in the center of the spindle, forming a pseudotetrad. Anaphase I is equational for the sex chromosomes. At late anaphase or telephase, X1 and X2 join end-to-end but form spindle fiber connections to only one of the poles of the metaphase II spindle, leading to one daughter cell without X chromosomes and one with both X1 and X2. An attempt is made to explain sex chromosome pairing and orientation on the basis of a telocentric organization of meiotic chromosomes. The apparent differences in the kinetic organization of mitotic and meiotic chromosomes in Heteroptera are discussed.
