Storing biological sequence databases in relational form.

SUMMARY: We have created a set of applications using Perl and Java in combination with XML technology to install biological sequence databases into an Oracle RDBMS. An easy-to-use interface using Java has been created for database query and other tools developed to integrate with our in-house bioinformatics applications. Interprofessional collaborative alliances: health care educators sharing and learning from each other.

The clinical setting in which health care professionals work is interdisciplinary, complex, and outcome-oriented. Interprofessional health care teams in academia should work collaboratively to design educational experiences that provide students with the skills necessary for contending with the complexities of practice. The interprofessional team may also collaborate in research efforts that evaluate the impacts of new educational strategies on student outcomes. This work establishes a collaborative framework, the interprofessional alliance model, that describes the stages and relationships that are established during the process of interprofessional collaboration. The new model is a synthesis of two established models: the five-stage model of collaboration and the alliance model. The authors illustrate the application of their model with a case example that describes the collaborative relationship that emerged among educators in physical therapy and nursing at a single university. The interprofessional alliance model may be used to examine the process of collaborative efforts among health care providers of varied disciplines.

[EC Respiratory Health Surgery in the north of Italy: methodological problems of a multicenter study of prevalence of asthma].

The EC Respiratory Health Survey (ECRHS) design is presented: the survey aim is to estimate asthma prevalence and risk factors distribution in the various European countries population. In Italy three centres are involved: Pavia, Turin and Verona. EC Health Respiratory Survey is an international two stages cross-sectional survey on probability samples of general population aimed to asses asthma prevalence in different countries. In the first stage a random sample was sent a Screening Questionnaire by post; in the second stage a subsample was invited to clinical centres in order to undergo lung function tests, allergy tests and a standardised clinical interview. The analysis of the response pattern in Italy shows that both in stage 1 and in stage 2 a selection bias overestimating real prevalence is present. Some methodological issues concerning the design of cross sectional surveys are addressed.

Mentorship: a feminist critique of current research.

Reciprocal professional nurse relationships called mentorships have been researched over the last decade from the perspective of coming to know that they are, why they work and what they bring to each of the participants. 'Knowing' always has its limitations, since we, the inquirers, are only able to come to understand through the ways we have been shown to ask questions or where to ask them. If the personal is political, then nurses who explore mentorship/protégé relationships or any other area in nursing practice or education, must become aware of the unconscious biases which affect our choices in research. From a feminist perspective, each of us must decide, at least, to explore alternate ways of 'knowing' through the use of methodologies which engage women's lived experiences in a context which allows for stories to be shared and not reported. This paper is an opportunity to explore historical concepts, current needs for mutual support and the environmental and contextual experiences which may be fertile soil for new ways to understand how we can authentically bring meaning to each other wherever we work or study.

Feminist critique: searching for meaning in research.

Feminist critique is presented as an important method for examination of prior knowledge for androcentric and ethnocentric bias in all aspects of knowledge development, from theoretical underpinnings through the steps of the research process. The history and implications for using feminist critique as a mode of inquiry for nursing science are developed. In addition, pragmatic information for the conduct of feminist critique is provided.

Tumor-stimulated release of tumor necrosis factor-alpha by human monocyte-derived macrophages.

Tumor necrosis factor-alpha (TNF) release by monocytes and macrophages may be an important determinant of the physiologic response of the host to neoplastic disease; however, the mechanisms which regulate TNF release by macrophages in hosts with neoplastic diseases are poorly understood. The purpose of this study was to determine if cell membranes and growth medium from human leukemia cell lines and solid tumor cell lines induced TNF release by cultured human blood monocyte-derived macrophages. The capacity for TNF release and direct tumor killing was highest in monocytes cultured for 7 to 11 days. Cell membranes and culture media from K562 erythroleukemia and several small cell lung carcinoma cell lines, including H82, induced the release of up to 1500 TNF units per 10(6) macrophages over 24 hr. By contrast, allogeneic peripheral blood lymphocytes, cell membranes from normal mixed donor peripheral blood leukocytes, or growth medium from normal embryonic lung fibroblasts induced the release of little or no TNF during culture up to 24 hr, suggesting that this macrophage response was specific for tumor cells. Release of TNF by tumor-stimulated macrophages was gradual, peaking 24 hr following the addition of stimuli. Induction of macrophage TNF release was concentration dependent, with half-maximal TNF levels induced by 12.5 and 25 micrograms/ml cell membranes prepared from K562 and H82, respectively. Pretreatment of tumor cell membranes with polymixin B, which inhibits many of the actions of endotoxin, failed to neutralize tumor induction of TNF, suggesting that endotoxin was not responsible for this activity. Depletion of macrophages by treatment with 3C10 monoclonal antibody and complement abrogated tumor-induced TNF release, indicating that macrophages were the source of the secreted TNF. HPLC analysis of H82 growth medium demonstrated a single peak of macrophage activating activity with approximate 40-kDa molecular weight. We have demonstrated that cell membranes and growth medium from some human leukemia and solid tumor cell lines, but not from normal human cells, induce human peripheral blood monocytes and monocyte-derived macrophages to release functionally active TNF. This process may contribute to the host response to some neoplastic diseases.

Persistent and recurrent hyperparathyroidism: assessment with gadopentetate dimeglumine-enhanced MR imaging.

The effect of gadopentetate dimeglumine on signal intensity of abnormal parathyroid glands was assessed in 14 patients with persistent and recurrent hyperparathyroidism. Non-contrast material-enhanced T1- and T2-weighted spin-echo images were compared with T1-weighted images obtained 1 minute and 10 minutes after administration of gadopentetate dimeglumine. Percentage of contrast between the abnormal gland and surrounding tissue was determined with the use of skeletal muscle, subcutaneous fat, and thyroid gland as reference tissues. All 11 abnormal parathyroid glands showed low to intermediate signal intensity on T1-weighted images and high signal intensity on T2-weighted images. Gadopentetate dimeglumine significantly increased the signal intensity enhancement ratio of all abnormal parathyroid glands, causing increased percentage of contrast relative to the thyroid gland and skeletal muscle on T1-weighted images. However, the percentage of contrast relative to these tissues was even greater on nonenhanced T2-weighted images. Thus, gadopentetate dimeglumine can substantially enhance the signal intensity of abnormal parathyroid glands and improve differential contrast with some neighboring tissues on T1-weighted images, but it does not improve contrast with surrounding tissue beyond that achieved on T2-weighted images.

Content validity of a Scale to measure silencing and affectivity among women living with HIV/AIDS.

This study measured quantitatively and explored qualitatively silencing behaviors and affectivity (mood) in women living with HIV/AIDS and confirmed the validity of the Silencing the Self Scale and the Positive and Negative Affect Scale. Silencing behaviors are interpersonal communication styles that suppress personal needs and feelings to preserve relationships with others. Silencing behaviors serve as protective strategies that allow one to divorce oneself from an overbearing culture. Affectivity is a way of measuring one's personal mood state by a positive to negative continuum. The results indicate that the women silenced themselves profoundly, especially when it came to putting the needs of their children or dependents before their own. The women also had high levels of negative affectivity. The research findings from this study extend nursing knowledge by addressing the unique social processes of women living with HIV/AIDS within health care service structures and significant social groups. Further exploration of "silencing" as a phenomenon of this group through measurement and experience will help define specific interventions that are meaningful to and for women living with HIV/AIDS.
