Bilateral anesthesia into Spix's spine. Ten years' experience.

AIM: This study examines 10.112 cases in 10 years of bilateral block anaesthesia to Spix's spine. The aim of this study is therefore to eliminate any doubt about executing a bilateral anaesthesia to Spix's spine for the extraction, in inclusion and impacted of the inferior wisdom teeth. METHODS: A retrospective study was carried out on 10,112 operations (59% females, 41% males,) regarding the removal of the lower 3 molar teeth undertaken in the past 10 years. Locoregional anaesthesia and standard instruments and protocol were applied to all operations. The anaesthetic used was mepicavina at 2% with epinefrine 1:100,000 with a 4 x 25 mm, 27 Gauge needle. In order to completely anaesthetize the areas subject to surgery a quantity of approximately 3-ml on each side was injected. Local anaesthetics used were the same throughout the study; all the patients were in good health and not undergoing any pharmaceutical treatment. RESULTS: The onset of complications was 3 times less when using local anaesthesia (LA) compared to general anaesthesia (GA). It was also proven that local anaesthesia was advantageous, as the operation time was considerably less (24 min for LA and 46 min for GA). CONCLUSIONS: The paper shows that LA offers a significant reduction in complications, psychological advantages for the patients and facilitation of the operational position too, so that GA is to be preferred only for non-cooperative patients.

Comparison of the body composition of age-matched Italian, Ukrainian, and Dutch children.

In the present study, the body composition was measured in 35 Ukrainian children, 14 boys and 21 girls, aged 8-12 years, by dual-energy X-ray absorptiometry (DEXA), anthropometry, and bioelectrical impedance. The results were compared with those obtained from a sample of the same number of Italian and Dutch children of the same sex and range of age. The Ukrainian children were slightly smaller, had a lower body weight, and had a lower body fat percentage, as measured by skinfolds, as compared with the Italian and Dutch children, as well as a lower calculated fat-free mass (FFM) by impedance. Comparison between FFM as determined by different methods in the Ukrainian children showed that all assessed values differed significantly. In particular, all methods overestimated the FFM as compared with DEXA, but the differences between DEXA and the other methods were not dependent on the level of FFM as determined by DEXA. Furthermore, the difference between predicted FFM and FFM values obtained by DEXA was correlated for skinfolds and body mass index (r = 0.77, p < 0.01), but not for body mass index and impedance, or skinfolds and impedance. The mean differences were 1.8 +/- 1.5 for body mass index, 1.0 +/- 1.4 for impedance, and 3.6 +/- 1.6 for skinfolds. Some differences in the body composition have also been found between boys and girls. The boys had higher values of lean tissue (23.3 +/- 3.1 vs. 20.7 +/- 3.3; p < 0.01) and FFM (24.5 +/- 3.3 vs. 21.8 +/- 3.5; p < 0.01) as compared with the girls and slightly higher values of bone mineral content. It should be noted, however, that the differences between different groups and different methods are always small, even when they are statistically significant.

[Cefoxitin in emergency surgery].

In basic pharmacological studies, sodium cefoxitin has proved to possess the following features: a broad action spectrum, resistance to bacterial beta-lactamase, activity against anaerobic species, rapid distribution, rapid serum and bile concentration, rapid, elevated excretion in the urine in active form, and low local and general toxicity. Patients in an Emergency Surgery Department often present the following conditions: highly compromised general situation, serious, often polymicrobic sepsis with aerobic and anaerobic flora, prior treatment without result with various cycles of chemo-antibiotic treatment. The theoretical conditions therefore exist to assess the effectiveness of sodium cefoxitin in patients admitted to an emergency surgery department and presenting serious post-surgical or post-traumatic sepsis, or sepsis due to pathology that can be corrected surgically. A study of 14 patients (3 g i.v. or i.m. for 6 days) showed 71% complete cure, 21% definite improvements and one death (treatment commenced 36 hours prior to death). It is therefore maintained that sodium cefoxitin is particularly indicated as first-choice antibiotic in emergency surgery situations.

Multifrequency impedance in the assessment of body water losses during dialysis.

Multifrequency bioelectrical impedance was used to predict changes in body water compartments during renal dialysis. Weight loss during dialysis was assumed to be water loss. Predicted total body water (TBW) from impedance after dialysis did not differ significantly from TBW determined by deuterium oxide dilution. However, the predicted change in TBW from bioelectrical impedance largely exceeded the observed weight (water) loss. The predicted change in extracellular water was slightly but significantly lower compared to the observed weight loss. The ratio of impedance at 1-100 kHz increased in all subjects during dialysis, and may be a simple tool to assess body water distribution.
