Evaluation of psychic parameters in seamen and fishermen with a long employment period. Part I.

The aim of the present study was to select these personality features of seafarers with a long employment period, which affect the process of occupational adaptation. A group of 588 seamen of deck, catering and engine crew--including fishermen from the fish processing plant, mean age 44,89 years and the mean period of employment at sea 19,42 years, was subjected to MMPI test. The highest number of results deviating from the norm were found in the group of engine crew. The highest rate of psychological disturbances were found in fishermen from the processing plant. The smallest amount of pathology revealing results were noted in the deck and catering crew--it comprised the highest number of seamen counted as psychologically normal and the least as neurotic. In all the seamen and fishermen tested the examinations revealed a high prevalence of neuroses--36% of the population examined particularly psychosomatic neurosis. Neurosis presents a serious health problem in this occupational group, and it is probably caused by the specific conditions of life and work at sea.

EEG changes measured by spectrum analysis under hyperbaric conditions.

The EEG activity was followed during simulated diving to 20 m depth. Altogether 49 EEG records of 5 min length were taken in 5 divers breathing compressed air in a close system. No significant changes of the EEG could be found during either compression or decompression if only conventional indicators, such as the amplitude of theta activity, were tested. However, a complex "normality ratio," based on multivariate analysis of a normal material, was capable of showing significant impairment of the brain activity during the decompression. The indicator has been designed to reflect the abnormality level rather than the short-lasting EEG changes, the latter being mostly dependent on the alertness fluctuations. The EEG changes associated with alertness fluctuations were assessed separately by means of a specific "alertness indicator" and a prominent alertness increase was found during both compression and decompression phases of the diving experiment.

EEG changes under hyperbaric conditions: spectral analysis during simulated diving.

Continuing investigations of the influences of the hyperbaric pressure upon EEG record in divers during simulated diving. The hyperbaric ecological system exerts influences on brain bioelectric activity manifesting itself by changes of alpha frequency and appearance of slow waves. The changes in the brain activity are mostly observable at the changes in chamber pressure values. The changes in EEG recordings are smallest at constant pressures. EEG examination can be used as a measure of the safety of newly developed decompression schedules.

Evaluation of psychical state of the engine room crew members of a long period of service at sea.

The work was made to find the traits of personality in the engine room crew members of a long occupational experience, which alleviate or aggravate their process of adaptation to the conditions of work at sea. The goal of investigation was to answer the question whether and if so then to what extent neurotic and psychopathic disturbances are negatively select in the scope of their occupational adaptation. 270 seafarers were subjected to examinations performed by means of the following tests: MMPI, Behaviour Examination Patterns, Social Readjustment Rating Scale, Scale of Psychical Load with work. The research resulted in conclusions: neurotic disturbances do not select negatively members of the engine room crew as to their occupational adaptation. Neurotic disturbances are getting more intense with the age and the employment period. In spite of this the seafarers affected are objectively regarded good workers. Disturbances of behaviour and personality reveal decreasing tendency with the age and the length of service, most probably it is the mechanism of natural selection that should be accounted for. In this group 6.5% of persons show tendency to fall in alcohol dependency. Seamen aged 40-50 years constitute a group that such deviated types are most frequent, therefore they should be given a special medical attendance.

Preliminary reports on hyperbaric oxygen therapy in multiple sclerosis.

Preliminary clinical examinations comprised 16 patients suffering from multiple sclerosis, the mean duration of the disease 9.33 years, of the manifestations at different stages of the development--evaluated on the basis of Kurtzke scale. The patients were subjected exclusively to the treatment with oxygen hyperbary--all in all 25-30 exposures, with the intervals of 24 hours, under the O2 pressure of 2 ata. The qualification for the purposes of treatment and the classification of the symptoms present were carried out according to Fisher, the results obtained were evaluated in conformity with standard version of the Disability Status Scale of Kurtzke. Apart from the routine clinical and laboratory tests the programme of the examinations included also a quantitative determination of immunoglobulins level, complement activity as well as the determination of T and B lymphocytes. An eminent change for the better was found in 14 afflicted persons, in one patient after 15 exposures a worsening was observed and the treatment was arrested. As to the immunological parameters investigated, a general decrease of IgG, IgM, IgA in serum, increase of the complement fraction and its 50% hemolysis determined activity were observed, whereas quantitatively investigated T and B lymphocytes did not show any typical changes. The authors discuss the results.

Some remarks concerning methodological problems of medicine.

The present paper deals with methodological problems in medicine as science--against other practical sciences especially pedagogics. The authors try to define the specificity of medicine, which relies on dissimilarity of the methodological directives directing the research practice of medicine. In the authors' belief this type of reflection is an essential, complementary element in the individual's knowledge of medicine making the medical research practice a more rational process.

EEG changes measured by spectrum analysis under hyperbaric conditions.

The subject of experimental, saturated dividing to 20 m depth was alertness level measured by means of spectral analysis of EEG records. Air was served as breathing mixture compressed to a given depth in closed circulating system. Altogether 49 EEG records made during compression, plateau and decompression, were analysed. Two methods of analysis were applied. "Normality ratio" combined the information from 20 different EEG values and "EEG alertness indicator" showed in three cases pathological slowing and fluctuations of the alertness. According to index of alertness, the most prominent changes were seen during the compression and decompression phase. During the plateau phase sleepiness was recorded. The applied method could provide information when slowing is due to metabolic impairment of brain activity from that of sleepiness.
