Traditional and new types of spondarthritis with special consideration of spondylodiscitis.

In rheumatology the so-called "seronegative spondarthritis" is a group of diseases characterized by the presence of HLA-B 27. This group includes the typical ankylosing spondylitis as well as atypical spondylopathies such as those occurring in psoriasis, Reiter's disease and chronic inflammatory enteropathies, which attack mainly the spine and secondarily the peripheral joints. In some severe cases, non-infectious, sterile spondylodiscitis was observed. These can lead to instability and fracture, followed by pseudarthrosis of the involved segment of the spine. In contrast to these traditional spondarthritides three new types are marked by the lack of HLA-B 27. 1) "Spondarthritis hyperostotica pustulo-psoriatica" (F. Schilling), a very rare variation of psoriatic spondylopathy, sometimes accompanied by spondylodiscitis. 2) Arthritis and spondarthritis in acne fulminans. 3) Destructive arthropathy and spondylopathy in long-term hemodialysis, occasionally occurring with spondylodiscitis, a very new type of spondarthritis. The amyloid B (beta-2-micro-globulin), discovered only four years ago, plays a dominant role in the pathogenetic chain of this disease. Details of the etiology of these very impressive diseases are presented. Destructive spondylodiscitis will no doubt be a challenge to neurosurgeons.

Acne Vulgaris

[Cell kinetics in experimental rheumatoid lesions and application in antirheumatic drug research (author's transl)].

The kinetics of chronic inflammatory cellular infiltrates in antiglobulin-induced arthritis, peritonitis and subcutaneous fibrin granuloma were investigated. Only small numbers of the mononuclear cells were labeled after 3H-thymidine impulse labeling, whereas a high labeling index results after 3H-thymidine labeling of the bone marrow. It is concluded that in experimental arthritis the inflammatory so-called cellular hyperplasia consists in bone marrow derived mononuclear cells. In experimental peritonitis glucocorticoid treatment decreases the 3H-thymidine labeling indices. By means of such labeling studies the antiproliferative and antiexsudative properties of steroids were demonstrated quantitatively. Therefore we suggest to make use of experimental cell kinetics for the recognition of anti-rheumatic drugs.

The influence of branched-chain and omega-alicyclic fatty acids on the transition temperature of bacillus subtilis lipids.

The influence of branched-chain and omega-alicyclic fatty acids on the transition temperature of Bacillus subtilis lipids was studied by measuring the fluorescence depolarisation of the probe 1,6-diphenyl-1,3,5-hexatriene incorporated into lipid bilayers. Only anteiso-C15 and C17 fatty acid-enriched lipids showed no transition in the observed temperature range. Compared to the transition of normal lipids iso-fatty acid-enriched lipids have a slightly higher transition temperature. The incorporation of omega-alicyclic fatty acids with increasing size of the alicycle leads to a decrease in the transition temperature. A possible role of omega-cyclohexane fatty acids in Bacillus acidocaldarius is proposed.

Bacillus subtilis

Early appearance and mitotic activity of multinucleated giant cells in mice after combined injection of talc and prednisolone acetate. A model for studying rapid histiocytic polykarion formation in vivo.

Combined intraperitoneal injection of talc and particulate prednisolone acetate resulted in the formation of significant numbers of multinucleated giant cells within 48 hours. Neither one of these two agents had a comparable effect when injected alone. Evidence is presented showing that the multinucleated giant cells arose by cell fusion. Synchronous nuclear division could be observed in a considerable proportion of newly formed polykarions. Although the late prophases were usually normal, high proportions of abnormal mitotic figures were observed at later stages of mitosis, particularly in anaphase and telophase. The chromosome abnormalities were so severe that it is highly improbable that mitosis led to successful nuclear multiplication within a given syncytium. Similar to prednisolone acetate, prednisone elicited the formation of multinucleated giant cells when injected with talc. No polykarion formation was observed, however, when prednisolone acetate was substituted for other steroid preparations of similar particle size such as cortexon acetate, cortisone, or testosterone isobutyrate.

[Heterogeneity of antinuclear antibodies (ANA), immunoglobulin- and complement levels in sera of patients with rheumatoid arthritis (RA) (author's transl)].

ANA typing of RA patients according to the heavy- and light chains, complement fixing ability and immunofluorescent staining pattern was performed by the immunofluorescence technique and partially by qualitative immunoelectrophoresis. In the corresponding sera the immunoglobulins and complement components were quantiatively determined. Dependent on the ANA-immunoglobulin classes we found the following distribution: IgG-ANA; 40/93 (43%), IgM-ANA: 19/93 (20%), IgA-ANA: 2/93 (2%), and Ig-comb.-ANA:17/93 (18%). ANA only positive for polyspecific antimmunoglobulin serum: 15/93 (16%). After L-chain typing we found within a given ANA-Ig-class positivity for both L-chain subclasses (kappa and lambda): IgG-ANA: 19/40 (48%), IgM-ANA: 3/19 (16%), IgA-ANA:1/1 and Ig-comb.-ANA: 14/17 (82%). ANA cases within a given Ig-class only positive for the L-chain/type kappa:IgM-ANA:6/19 (32%), IgG-ANA: 9/40 (23%), Ig-comb.-ANA:2/12 (12%). ANA cases within a given Ig-class only positive for the L-chain type lambda: IgM-ANA: 7/19 (37%), IgG-ANA: 3/40 (8%), Ig-comb.-ANA:1/17 (6%) and IgA-ANA:1/1. In all cases of restricted ANA-positivity (only positive by use of polyspecific anti-immunoglobulin serum) no light chains were detected. In total we found approximatively 25% of complement fixing ANA, predominantly IgG-ANA (70%). Concerning the immunofluorescent nuclear staining we stated the following patterns: homogenous: 34/93 (36.6%), speckled: 29/93 (31.2%), mixed pattern : 22/93 (23.6%) peripheral: 5/93 (5.4%) and nucleolar: 3/93 (3.2%). The quantitative Ig-determination reveals a significant increase of IgG in ANA (single type) positive sera. IgG + IGM + IGA-ANA and IgG + IgM-ANA are found together with a significantly increased IgG- and IgM-serum levels, in the case of IgG + IgM-ANA we could demonstrate an additional relative decrease of the complement component C4. Sera with positive complement fixing ANA compared with non complement fixing ANA are characterized by an absolute increase of IgG and relative decrease of the complement component C4. No differences in serum-immunoglobulin- and -complement concentrations were registered dependent on ANA-immunofluorescent staining patterns.

Synthesis of omega-alicyclic fatty acids from cyclic precursors in Bacillus subtilis.

A mutant of Bacillus subtilis synthesizes a variety of omega-alicyclic fatty acids when fed with the respective alicyclic carboxylic acids. These fatty acids are: omega-cyclopropane, omega-cyclobutane, omega-cyclopentane, omega-cyclohexane, and omega-cyclohexene fatty acids. These unusual fatty acids did not lead to an inhibition of growth at 37 degrees C and pH 7. The selective advantage of these fatty acids under extrene conditions was studied in comparison with the acidophilic, thermophilic bacterium B. acidocaldarius, which normally contains a high proportion of omega-cyclohexane fatty acids.

The effect of vascular perfusion fixation on the ultrastructure of the juxtaglomerular apparatus of the rat.

After fixation by low pressure retrograde perfusion the electron microscopic picture of the rat JGA was studied and compared with the picture obtained after fixation by dripping and immersion. The most conspicuous difference was the presence of the basal compartments of the macula densa cells, which might land electron microscopic support to the tubulo-glomerular feedback theory. Among the JG granular cells, desmosome-like connected structures were seen. Crystalline and granule in granules specific secretion forms were frequent in the epitheloid cells.

[L-tryptophan-associated chronic eosinophilia-myalgia syndrome treated with cyclosporin].

After 2 weeks of ingestion of 130 g L-Tryptophan a 52 year old female develops an Eosinophilia Myalgia Syndrome with acute onset of deep venous thrombosis of forearm and possible initial cardiac manifestation featuring intermittent sinustachykardia. This is followed by a severe chronic disease (follow-up 15 months) with diffuse scleroderma and sensomotoric polyneuropathia. The deep muscle biopsy-specimen shows mononuclear infiltration of fascia and interstitial myositis with rare eosinophils. A blood eosinophilia (900/ul) occurs only in the initial acute onset of the illness. Plasma level of Kynurenine is significantly high (4000 pmol/ml), collagenneosynthesis is activated (Procollagen type III peptid 0.927 U/ml). No significant clinical improvement was seen with Acathioprine (100 mg/d) and Prednisolon (40-60 mg/d), after treatment with Ciclosporin scleroderma regresses completely, polyneuropathy is persisting.

Biopsy

[Serum immunoglobulin patterns in rheumatoid arthritis].

Immunoglobulins were estimated quantitatively in sera from patients with rheumatoid arthritis. According to the significantly increased immunoglobulin class(es), in RA patients as compared to normals the following immunoglobulin patterns were registered: IgG pattern, IgM pattern, IgA pattern, IgG-IgM-IgA pattern, IgG-IgM pattern and IgG-IgA pattern. The IgG-, IgA- and IgG-IgM-IgA patterns were predominantly found in sera from patients with seropositive RA. A positive correlation was found between other inflammatory parameters and high titered rheumatoid factor of the IgG-IgM-IgA-, IgG-IgA- and IgM patterns. C-reactive proteins were increased more frequently in seropositive cases and correlated quantitatively with the IgG-IgA-, IgG-IgM-, IgG-IgM-IgA and IgA patterns. Maximal erythrocyte sedimentation rates were accompanied with the IgG-IgA-, IgG-IgM-IgA- and IgG-IgM pattern. The serum complement components C 3 and C 4 were not decreased in neither seropositive nor seronegative RA. The formation of antiglobulins in hypergammaglobulinemic patients with RA induced by an increased immunoglobulin catabolism is suggested as a possible pathogenetic mechanism.
