The Caribbean Animal and Plant Information Network.

The Inter-American Institute for Cooperation on Agriculture (IICA) is presently implementing a four-year project in the Caribbean region to establish a regional information network on animal and plant health--the Caribbean Animal and Plant Health Information Network (CARAPHIN). CARAPHIN's immediate goal is to give the participating countries the technical capability to process and analyze the phyto- and zoo-sanitary information that they generate, share this information within the region, and use it as an effective instrument for decision making for agricultural development and trade. Sharing of information is further facilitated through the periodic regional and hemispheric meetings organized by IICA, by the Inter-American Animal Health Commission, and by the Technical Advisory Committee on Plant Protection. The Veterinary and Plant Protection Services of the region have expressed interest in the continuation of CARAPHIN as a permanent mechanism for agricultural health information management in the Caribbean. This motivated IICA to seek financing for a second phase of CARAPHIN, which will be executed from 1992 through 1996. Phase II of the project will seek to ensure the continuing application of the techniques and methodologies acquired by the agricultural health services of the different countries, and the functioning of the disease/pest information systems at the regional level. This will be achieved through technical assistance, continuing education, publications, provision of useful databases, and transfer of the project to a regional institution.

Spinal cord tumor presenting as a herniated lumbar disc.

The lesion encountered in this case was that of an intradural-extramedullary tumor. Histologic section showed an organized hematoma, which is a very rare tumor. In a recent review of the literature, only 85 cases have been reported, most in relation to a bleeding intraspinal neoplasm. Our case is especially unusual because there was no clotting abnormality or repeated lumbar punctures or arteriovenous malformation. This case also points out the necessity of a complete physical examination. A CT scan was initially negative because the scan started just below the tumor, hence it was never visualized. Metrizamide myelogram followed by the enhanced CT scan provides a more complete diagnostic study. In this patient's case, the CT scan would have led to inappropriate therapy, whether lumbar laminectomy or chemonucleolysis.

Civilian gunshot wounds.

A wide variety of injuries are encountered in civilian gunshot wounds with the severity directly related to the amount of kinetic energy imparted on the tissues. A surgeon should be aware of the different ballistic properties of the various firearms and be prepared to treat each patient as an individual. Furthermore, the physician should be attentive to severe complications that may occur even after appropriate treatment has been given. During an 18 month period on the LSU Surgical Service, 431 patients were admitted for gunshot wounds of which 218 had extremity injuries. One hundred seventy of these patients were evaluated and surveyed for type of injury, surgical procedure, and complications relating to the velocity of the involved.

The Grosse-Kempf interlocking nail: technique of femoral and tibial fractures.

An anatomical reduction is required on both the lateral and AP view. In our hands this is easiest obtained and maintained with the patient in the supine position. The supine position facilitates better anesthesia care in the multiply injured patient and aids in movement of the image intensifer during the procedure. Each step should be carefully followed and the horizontal screw placement is possible only when the image of the holes is perfectly round. When the previously mentioned conditions and steps are followed, a successful locking nail insertion can be achieved.
