Management of glioblastoma multiforme: with special reference to recurrence.

Between 1985 and 1995, 46 patients underwent craniotomy for glioblastoma multiforme. The mean age was 47, varying from 9 to 71 years. The influence of such prognostic factors as age, preoperative Karnofsky score, extent of resection, tumour site, tumour size, radiotherapy, reoperation as well as initial symptoms upon survival were studied. Of these, gross complete removal, radiotherapy, preoperative Karnofsky score, and reoperation were shown to be statistically significant to the survival time according to logrank and univariate tests. However, age, preoperative Karnofsky score, tumour size and temporal localisation remained as significant factors in multivariate analysis. The overall median survival was 53 weeks, with no patients surviving more than 3 years. Of the patients, 41% survived over a year and 8.6% lived over two years. Twenty-six patients developed a recurrent mass after an interval of 32 weeks. The median interval time from operation to recurrence was longer in those patients who underwent gross removal than in those who had a subtotal resection, 28.2 against 20 weeks (P < 0.05). Of patients who had a recurrent mass, 16 were reoperated on, with a subsequent median survival time of 26.5 weeks. Our experience suggests that the survival of patients with glioblastoma depends on many factors, including radical surgery as an initial step. In addition, the gross total removal of the tumour also delays the development of recurrence.

Aqueous humor penetration of topically applied ciprofloxacin, ofloxacin and tobramycin.

UNLABELLED: The purpose of this study was to determine the aqueous humor concentrations of topically applied ciprofloxacin (CAS 86393-32-0), ofloxacin (CAS 82419-36-1) and tobramycin (CAS 79645-27-5). Thirty patients undergoing cataract extraction or trabeculectomy were randomly divided into three groups and each of the group received either 0.3% ciprofloxacin, ofloxacin or tobramycin topical drops preoperatively. Eyedrops were instilled for six times at a frequency of one drop every 15 minutes, beginning 90 minutes before initiation of the surgery. At the time of surgery, 0.1 ml aqueous fluid was aspirated from the anterior chamber. Concentrations of the antimicrobial agents were determined using the microbroth dilution procedure outlined by the National Committee for Clinical Laboratory Standards. Escherichia coli (ATCC 25922) was used as a standard strain for determination of minimal inhibitory concentrations (MICs). The mean aqueous levels of ciprofloxacin and ofloxacin were found to be 0.092 +/- 0.077 microgram/ml, 0.964 +/- 0.693 microgram/ml, respectively. Tobramycin did not reach the concentration that could be detected by applied method. CONCLUSION: The mean aqueous humor levels of ofloxacin and ciprofloxacin were more than the MICs levels for most of the ocular pathogens which may cause postoperative endophthalmitis.

Synthesis and antimicrobial activity of electron rich olefin derived cyclic ureas.

Seventeen cyclic ureas containing imidazolidine and benzimidazoline nuclei were synthesised by the reaction of electron-rich olefins with appropriate group 16 elements. (O, S, Se, Te). The compounds synthesised were identified by 1H, 13C-NMR, FT-IR and mass spectroscopic techniques and micro analysis. All compounds studied in this work were screened for their in vitro antimicrobial activity against standard strains: Enterococcus faecalis (ATCC 29212), Staphylococcus aureus (ATCC 29213), Escherichia coli (ATCC 25922) and Pseudomonas aeruginosa (ATCC 27853). Eight of the compounds were found effective to inhibit the growth of Gram-positive bacteria (Enterococcus faecalis and Staphylococcus aureus) at MIC values between 25-400 micrograms/ml. None of the compounds exhibit antimicrobial activity against gram-negative bacteria (Escherichia coli and Pseudomonas aeruginosa) at the concentrations studied (6.25-800 micrograms/ml).

Synthesis and antimicrobial activity of substituted benzimidazole, benzothiazole and imidazole derivatives.

New benzimidazole, benzothiazole and imidazole derivatives were synthesized by reacting electron-rich olefins (5, 23 and 29) with appropriate reagents. The compounds synthesized were identified by 1H, 13C-NMR, FT-IR and mass spectroscopic techniques and micro analysis. All new and related compounds were evaluated for their in vitro antimicrobial activity against different bacteria. The compounds 17, 18, 19, 20, 21, 22 and 24 were found very effective to inhibit the growth of Enterococcus faecalis (ATCC 29212) and Staphylococcus aureus (ATCC 29213) at minimum inhibitory concentrations (MICs) of 25, 25, 12.5, 50, 25, 50 and 50 micrograms/ml, respectively. The compounds 4, 10a, 10c, 16, 25, 26 and 31 were significantly effective against Enterococcus faecalis (ATCC 29212) and Staphylococcus aureus (ATCC 29213) with MIC values of 100-200 micrograms/ml. None of the compounds proved to be effective against Escherichia coli (ATCC 25922) and Pseudomonas aeruginosa (ATCC 27853) in the concentrations studied.

Double meningocele. Case report.

The coexistence of two distinct meningoceles of the spine is a very unusual event. We report a three-day-old boy with double meningoceles at the thoracic and lumbar levels. The connection between the stalk of the thoracic meningocele and the spinal cord, as seen on magnetic resonance imaging, showed a neurological involvement in this lesion. Our case is only the third without association of congenital anomalies or neurofibromatosis to be reported to date.
