Amyloid deposition in serosal membranes. Its occurrence with cardiac tamponade, bilateral ureteral obstruction, and gastrointestinal bleeding.

Widely dispersed amyloid deposition of the serosa occurring in a patient produced impairment of cardiac, renal, and gastrointestinal integrity by encasement. This patient survived the acute complications of both cardiac tamponade and bilateral ureteral obstruction due to amyloid. This case demonstrates the utility of resorting to aggressive life-support mechanisms despite an apparently systemic spread of amyloid disease in selected cases.

The bacteriostatic field in urology.

Infections related to wound drain sites, cystoscopy, transurethral resection, open prostatectomy, and urinary catheter drainage systems are preventable by the appropriate application of bacteriostatic fields which seal off these systems from environmental bacteria incorporating bacteriostatic and bacteriocidal compounds. A high level of sterility is achieved without the routine use of antibiotics.

Transcatheter embolotherapy for congenital renal arteriovenous malformations. Long-term follow-up.

Transcatheter embolization was performed five times in 3 patients with congenital renal arteriovenous malformations (AVM). The patients were followed for one to seven and a half years. Two patients whose AVM were completely occluded with Gelfoam have been asymptomatic with no evidence of hematuria. A third patient, embolized with Gelfoam and Ivalon without complete occlusion of the AVM, bled ten months after the first procedure, was re-embolized, and later had the AVM removed surgically. Superselective transcatheter embolization should be the initial treatment modality for congenital AVM with the aim to control bleeding and occlude those malformations supplied by the terminal portion of interlobar arteries. Large malformations supplied by multiple feeders may require operative ligation and removal of the AVM when vessels cannot be occluded satisfactorily because of massive shunting.

Experience with penile prostheses in the treatment of impotence in diabetic men.

Increasing use of penile prostheses to treat impotence in diabetic men warrants a detailed assessment of functional and subjective outcomes within this group. This study uses record review and patient-partner questionnaire techniques to examine this population and evaluate postoperative satisfaction in terms of changes in patterns of sexual activity, health status, and complication rates. The satisfaction rates among patients and partners were 81% and 83%, respectively. Frequency of sexual activity was increased, and physical enjoyment was improved among satisfied recipients. Most postoperative complications were treatable and led to dissatisfaction only if they were irreparable. Most prostheses are therefore being used successfully. We recommend that impotent diabetic men be informed of the availability of prosthetic implants.

Maintenance of sterility in urinary drainage bags.

When the outlet tube of urinary drainage sets was kept full of 3 per cent hydrogen peroxide at all times, no evidence of bacterial growth in the urine of the collection bag was noted in at least 92.0 per cent of the patients and 93.3 per cent of the drainage periods in a group of 60 patients and 75 drainage periods. One patient had a positive urinary drainage bag culture and a positive bladder culture as a result of improper disconnection of the catheter, and four patients had bacterial growth from urinary drainage bag samples, two of which were possibly due to a faulty culture technique and one of which may have resulted from contamination of amphoteracin B irrigating solution. Hydrogen peroxide, 3 per cent, appears to approach 100 per cent effectiveness as an antibacterial barrier. Only one patient of 60 or 1.7 per cent had a urinary tract infection develop which was related to the improper handling of the drainage set.
