Epithelial-myoepithelial carcinoma of the salivary gland.

Epithelial-myoepithelial carcinoma of salivary gland is a rare tumor only recently described in the English language literature. This paper adds four new cases to the 30 which have previously been reported. The clinical presentation of this unusual neoplasm is documented in the accompanying case reports and the appropriate surgical management outlined. Follow-up ranges from six months to 12 years. The histopathological characteristics of this interesting tumor, in particular its classical biphasic cellular appearance, are described in detail both by light and electron microscopy. On the basis of histologic appearance and biological behavior, the authors postulate that epithelial-myoepithelial carcinoma is an uncommon variant of adenoid cystic carcinoma.

An unusual first branchial cleft cyst.

First branchial cleft anomalies have been documented with increasing frequency within the past decade, with more than 200 cases reported to date. A case of a first branchial cleft cyst with some unusual features is presented. The literature of such anomalies is reviewed, with some distinct clinical differences being noted between cysts and other branchial cleft anomalies.

Amyloidosis of the maxillary antrum.

A rare case of primary amyloidosis which presented as a destructive lesion of the maxillary antrum is discussed. Current methods of diagnosis and treatment of amyloidosis are outlined.

Late complications of prolonged tracheal intubation.

Questionnaires were sent to patients who had tracheal intubation for periods longer than three days in a large multidisciplinary Intensive Care Unit. The information sought was of complaints related to talking, breathing, coughing, swallowing and chest infection. Of patients who had been intubated for seven days or less, 63 per cent of the 52 patients responding had no complications while only one of the remainder had a major complication requiring surgical removal of a granuloma. Forty-eight per cent of patients intubated for more than seven days had no complaints and the rest of the patients had minor complaints which did not persist. Most complained of hoarseness. Of patients who had a tracheostomy following prolonged intubation, only 23 per cent were free of complications. From this it is concluded that tracheal tubes can be left in place for seven days and at this time direct laryngoscopy should be done. If no significant laryngeal pathology is seen at this examination, tracheal intubation may be continued.

Chemodectoma of the larynx.

The ninettenth reported case of chemodectoma of the larynx is presented and its management discussed. Some reflections on the laryngeal paraganglia are made and the past literature is reviewed.
