Changes of tibia bone properties after spinal cord injury: effects of early intervention.

OBJECTIVE: To evaluate the effectiveness of an early intervention program for attenuating bone mineral density loss after acute spinal cord injury (SCI) and to estimate the usefulness of a multimodality approach in diagnosing osteoporosis in SCI. DESIGN: A single-case, experimental, multiple-baseline design. SETTING: An SCI center in a university hospital. METHODS: Early loading intervention with weight-bearing by standing and treadmill walking. PATIENTS: Nineteen patients with acute SCI. OUTCOME MEASURES: (1) Bone density by peripheral computed tomography and (2) flexural wave propagation velocity with a biomechanical testing method. RESULTS: Analysis of the bone density data revealed a marked decrease of trabecular bone in the nonintervention subjects, whereas early mobilized subjects showed no or insignificant loss of trabecular bone. A significant change was observed in 3 of 10 subjects for maximal and minimal area moment of inertia. Measurements in 19 subjects 5 weeks postinjury revealed a significant correlation between the calculated bending stiffness of the tibia and the maximal and minimal area moment of inertia, respectively. CONCLUSION: A controlled, single-case, experimental design can contribute to an efficient tracing of the natural history of bone mineral density and can provide relevant information concerning the efficacy of early loading intervention in SCI. The combination of bone density and structural analysis could, in the long term, provide improved fracture risk prediction in patients with SCI and a refined understanding of the bone remodeling processes during initial immobilization after injury.

[Tuberculous mastitis--a rare differential diagnosis of non-puerperal mastitis].

Tuberculosis in a case of non-puerperal mastitis is a rare clinical finding. A special case of a 28-year old Thai patient was diagnosed and successfully treated with an antituberculous chemotherapy. The most important diagnostic procedure was the microbiological and histological investigation to enable the diagnosis to be made. Surgical therapy was not necessary.

[Breast cancer grading by automated image processing].

Individual oncological therapy of carcinoma of the breast depends on information on the individual prognosis and malignancy. Image processing and analysis of the digital structure of the cellular nucleus adds new prognostic data to standard morphological and biochemical parameters and should be used for further individualisation of oncological therapy.

Treatment of vaginal candidosis with fluconazole.

In patients with vulvovaginal mycosis systemic treatment is being discussed for eradication of possible fungal reservoirs, therefore, the efficacy and toleration of fluconazole, a triazole-derivative, has been investigated in the course of an open, comparative study. 104 patients with clinical, microscopic and cultural evidence of vaginal mycosis were included, 53 of whom were treated with a single dose of 150 mg of fluconazole and 51 patients daily received local treatment with 50 mg of econazole over a period of 6 days. The results regarding clinical efficacy were comparable for both preparations. At follow-up visits 4 to 10 d and 30 to 45 d post therapy the mycological as well as the subjective cure rates were similar. However, there was a higher mycological cure rate in the rectum of fluconazole patients which was statistically verifiable. This enables the elimination of rectal fungal infection as a source of homologous reinfection.

[Value of conization in the therapy of carcinoma in situ].

The volumes of cone biopsies of 156 patients with primary diagnosed carcinoma in situ of the cervix uteri were compared with the histological findings of cone biopsies and subsequent cervicectomy. The therapeutic result did not depend essentially on the size of cone biopsy, but rather on including the transitional area and endocervical epithelium together with electrocoagulation. The coagulation produces a security zone which significantly increases the effective volume of the cone biopsy.

Measurement of nuclear DNA in the management of cervical intraepithelial neoplasia.

Cytophotometry was used to study the DNA content in 166 smears. The results were compared with the histological findings at cone biopsy which showed 117 carcinomas in situ, 30 microinvasive cancers, and 19 invasive carcinomas. There were statistically significant differences in DNA content and distribution between the intraepithelial neoplasia and microinvasive or invasive carcinoma. We also found cytophotometric differences between exocervical and endocervical in situ carcinoma and invasive cancers. The DNA content of cells did not vary significantly with age. Exocervical carcinoma in situ to have aneuploid histograms while endocervical in situ carcinoma had a euploid distribution of DNA, cytophotometry can help in planning the limits or extent of a cone biopsy.

Histographic DNA pattern as an aid for diagnosis and therapy of minimal invasive endometrial cancer.

The atypical adenomatous hyperplasia of the endometrium may be considered as carcinoma in situ. The histologic diagnosis from curettings of passages to minimal invasive or occult carcinomas of the corpus uteri may be difficult. Also the determination of DNA in the cell nuclei and its histographic evaluation do not permit to differentiate between atypical hyperplasias and invasive adenocarcinomas. Only the early or minimal invasive carcinoma shows a higher rate of proliferation, assuming a stepwise development of carcinoma in which phases of activity and retirement are changing. In these cases the DNA photometry may support the histologic diagnosis of invasive growth and facilitate the choice of therapy.

Cytophotometric determination of DNA in minimal invasive cancer of the human cervix uteri.

The early invasive cancer of the human cervix uteri is marked by a higher proliferation rate in its DNA histograms, which is a sign of perforation of the basal membrane. When this process finishes and an invasive carcinoma is present, the range of the DNA values decreases. The carcinoma has possibly reached a silent phase that will end with the penetration into the lymphatic system.

Cytophotometric determinations of DNA, histone, arginine, lysine, and their concentrations in eu- and heterochromatin of the cell nucleus of dysplasias, carcinoma in situ, and carcinoma of the human cervix uteri.

In the cell nuclei of dysplasias, carcinoma in situ, and carcinoma the amount of DNA, histone, arginine, lysine, and their condensed areas were determined by cytophotometry. Moreover their concentrations in the euchromatic and heterochromatic areas of the nuclei were measured. Statistical analysis showed that the quantitative relations between parameters did not change during carcinogenesis. The significantly increased amount of lysine underlines the role of the lysine-rich histones in producing denser coiling of the chromatin fibril in heterochromatin.

Cytophotometric measurement of nuclear DNA and lysine as a method for automated screening of cervical smears.

An automated initial screening of cervical smears would be of great benefit as much time would be saved by removing the need to rely on cytologists for the diagnosis of negative smears. The computerized evaluation of DNA histograms obtained by impulse cytophotometry shows promise but the additional determination of the amount of lysine in the nucleus would seem to be of value. Three mathematical methods for the evaluation of histograms are presented and compared. It is shown that measurements of nuclear DNA as well as lysine improve the diagnostic accuracy of automated screening processes.

[Investigations about the influence of halothane and enflurane on the nuclear metabolism of bronchial mucosa cells of the dog (author's transl)].

The influence of halothane and enflurane on the nuclear metabolism of bronchial mucosa cells was investigated over ten hours on 5 dogs each anaesthetized with halothane or enflurane. Desoxyribonucleinic acid (DNS) and, for the arginine and lysine-rich histone fraction the basic amino-acids arginine and lysine were measured. During the ten hours of anesthesia there was a decrease of all patterns, reversible in a control 48 hours after the end of anesthesia. The decrease of DNS and arginine was similar to each other, but that of lysine was delayed. The results indicate a reversible retardation of the nuclear metabolism. The site of action is probably in the activated zone of nucleus.

[Recto-vaginal fistulae: causes, treatment, results (author's transl)].

Among recto-vaginal fistulae in 41 patients 22 were due to radiotherapy, 6 to inflammatory disease, 4 occurred as a postoperative complication and 9 were carcinomatous fistulous tracts. Passage of stool or air per vaginam is a pathognomonic sign. The fistula can usually be diagnosed by routine gynaecological examination. Barium enema, barium meal with follow-through or colpography often demonstrate the fistula. Except for a few cases in which the fistula closes spontaneously (most frequently those which occur postoperatively), treatment is surgical. Intestinal resection is often necessary in the inflammatory fistulae and those after radiotherapy. In fistulae due to carcinoma colostomy will improve symptoms: radical removal of the tumour is rarely possible. 31 of the 44 women are cured, while in two a fistula has persisted.

[A contribution to the weight determination of the fetus by combined kephalo- and thoracometry (author's transl)].

By comparison of the sectional area of the fetal head of the fronto-occipital level in the height of the biparietalic diameter and the cross sectional area of thorax in the level area of the ventrical it was tried to determine the exspected birth weight. A mean deviation of 8,9% was found. Below a birth weight of 2500 g the presented calculation is only valid to some extend as in these cases the dominance proved in fetal development grounds of the head of the fetus becomes apparent.
