[Diagnostic value of sonography (B-scan) in the upper abdominal region].

Minimal risk, easy applicability and high efficiency of ultrasonography, makes this method a valuable screening procedure in upper abdominal diagnostics. Case history, physical examination and laboratory diagnostics should be followed by ultrasound procedures as the first directed examination. This review intends to evaluate the efficacy of ultrasonography in different gastroenterological, oncological, hematological and angiological diseases of the upper abdomen.

[Chronic pancreatitis: diagnostic value of endoscopic retrograde pancreaticography, of pancreatic sonography, and of secretin-pancreozymin test corrected for volume losses (author's transl)].

Endoscopic retrograde pancreaticography (ERP) and secretin-pancreozymin test corrected for losses (SP test) were performed in 153 patients suspected to have pancreatic disorders in order to evaluate diagnostic significance of these procedures. Pancreatic sonography was done in addition in 110 of these patients. If pancreatic excretion was normal, ERP results turned out to be normal in the same patients as well. SP test involves rather extensive laboratory work-up, but it does yield the most precise results as far as diagnosis of chronic pancreatitis is concerned. For this reason, and because of the possible complications caused by ERP, ERP should be applied only, when surgery is considered. Results of SP test and sonography coincided rather well. Therefore, sonography, not doing any harm to the patient, does have its place in the diagnosis of chronic pancreatitis.

Ultrastructural changes of colonic mucosa in patients with chronic laxative misuse.

Laxatives are amongst the most widely used and misused drugs. Especially the "irritant chathartics" are associated with severe cell poisoning. Electrolyte disturbances like potassium deficiency may occur as a result of enhanced potassium secretion into the colon. Ultrastructural studies of patients with chronic laxative misuse demonstrate, that this functional disorder is correlated with damage of the colonic epithelial cells consisting in stereotyped lesions as changes in microvilli, reduction and structural defects of cytoplasmic organelles and an increase of lysosomes and dense bodies. The lesions have a focal distribution and may explain the potassium loss from a morphological point of view.

[Determination of splenic size by ultrasonic scanning (author's transl)].

Two dimensional B-Scans make it possible to examine and measure the spleen. In 62 patients the size of the spleen was evaluated by ultrasound; the results were compared with the size of the organ after extirpation: In 58 of 62 cases there were correct results. Therefore, the ultrasonic determination of the splenic size is a reliable method for clinical questions.

[Acute leukemia as terminal stage of Hodgkin's disease (author's transl)].

In a 61 year-old female patient with Hodgkin's disease treated with cytotoxic drugs and radiation therapy acute undifferentiated leukemia developed 10 years later. The possibility of a carcinogenic effect of the cytotoxic and actinic therapy is discussed. Leukemia complicating the course of Hodgkin's disease must be considered an independent disease and not a iatrogenic transformation of a preexisting lesion.

[Co-existence of chronic myeloid leukaemia and carcinoma of the breast (author's transl)].

Chronic myeloid leukaemia and carcinoma of the breast co-existed in three women. This combination is exceedingly rare. Although the cancerogenic properties of ionizing radiation and cytostatic drugs could be held responsible for the occurrence of two malignant tumours, no definitive conclusions can be drawn in the individual case. Co-existence of leukaemia and carcinoma does not occur more frequently than could be predicted statistically from the incidence of each.
