Substance use among Hispanic and non-Hispanic adolescents.

This study compared lifetime prevalence data on substance use with demographic and psychosocial variables among 2,821 Hispanic and non-Hispanic white students from two Southwestern cities. Study findings revealed that ethnic-racial background factors were not the strongest predictors of substance use. Rather, when the analyses accounted for school grades and for maternal education, non-Hispanic youth had higher lifetime substance use rates than Hispanic youth. Study findings suggest that preventive intervention and treatment efforts for substance abuse problems among adolescents should consider sociodemographic risk factors as well as ethnic-racial factors.

[Traumatic pancreatic pseudocyst, 2 cases].

Two patients with traumatic pancreatic pseudocyst are presented. One patient was treated with percutaneous drainage and the other was operated on to perform a cystoenterostomy. Both patients recovered successfully. Diagnosis and treatment of this condition is discussed.

[The fall of the Jericho walls].

Archeologic and medical evidence and biblical texts were analyzed in search of support for the hypothesis that the fall of Jerico walls might have been related to an epidemic caused by Schistozoma hematobium. There is evidence of the presence of Balinus trunctus, a snail acting as an intermediary host, but not of the disease itself. However, the hypothesis could still be tenable if all inhabitants left the city, which is suggested by all available evidence. The text in Re 2: 19-22 may not be taken as literary evidence of infection by the agent under discussion.

Hemodynamic changes in underweight pregnant women.

Twelve normal-weight and 12 underweight women were compared to test whether fetal growth retardation in underweight gravidas is related to inadequate maternal hemodynamic adjustments. Plasma volume (+/- standard error) was 3227 +/- 103 mL in normal-weight and 2731 +/- 84 mL in underweight women (P less than .002). Cardiac output was 6340 +/- 167 mL/minute in controls and 5689 +/- 213 mL/minute in underweight women (P less than .03). Total peripheral vascular resistance was lower in controls than in underweight subjects (1025 +/- 31 versus 1198 +/- 58 dyne/second/cm5). Mean birth weight was 2837 +/- 125 g in underweight women and 3362 +/- 106 g in controls (P less than .005). Similarly, placental weight was reduced in the underweight group. All infants delivered by control mothers had a normal birth weight, whereas six infants from underweight gravidas were growth-retarded. In all cases combined, maternal plasma volume correlated significantly with both birth weight (r = 0.6, P less than .002) and placental weight (r = 0.56, P less than .01); total peripheral vascular resistance also correlated significantly and inversely with newborn weight and placental weight. Cardiac output correlated only with placental weight (r = 0.54, P less than .02). These results are consistent with the hypothesis that underweight mothers are at higher risk of fetal growth retardation because of a smaller plasma volume and lower cardiac output.

[The fall from height of king Ocozhyia of Israel].

An historic and literary analysis was performed to evaluate the clinical characteristics of the fall from height experienced by Israeli King Ocozhyia (book of kings 2: 2-17). We conclude that the clinical course was brief, there was no brain injury and it is likely that he developed a fracture but no retroperitoneal, soft tissue or vascular injury. Most likely hypovolemia and respiratory insufficiency derived from thoraco-abdominal injury caused his death. The biblical writer apparently was close to the action.

Accidental Falls

[Glycosylated hemoglobin in normal pregnancy].

In a prospective and transverse mode, HbA1 levels were determined in 60 normal pregnant women distributed in 5 groups according to gestational age. Also, total hemoglobin and glycemia levels one hour after a 50 g oral glucose load were determined. No statistically significant differences were found in HbA1 mean level of the different gestational age groups of our study. Correlation of glycosylated hemoglobin and gestational age, total hemoglobin and glycemia was found to be not significant.

Studies on the microheterogeneity of chorionic gonadotrophin secreted by the human cytotrophoblast in culture.

In the present study, we investigated the biological characteristics of different molecular forms of chorionic gonadotrophin (HCG) secreted by the human cytotrophoblast during its morphological and functional differentiation in culture. Highly purified cytotrophoblasts were prepared from term placentae and cultured for 24 to 96 h in the absence or presence of 8-bromo-3',5'-cAMP. Media were collected at 24 h intervals and the secreted isoforms of HCG were then separated by polyacrylamide gel isoelectric focusing (pH range 8.0-3.0) and quantified by radioimmunoassay. The secretion of HCG was significantly increased by 8-bromo-cAMP (from 23.5 +/- 6.3 ng/ml at 24 h to 1619 +/- 835.8 ng/ml at 96 h; controls, 9.3 +/- 0.1 ng/ml at 24 h and 26.6 +/- 3.5 ng/ml at 96 h, mean +/- SD). Analysis of media concentrates from cAMP-stimulated cultures by isoelectric focusing revealed the presence of several distinct peaks of HCG within the pH range of 7.3-4.8; major peaks consistently exhibited isoelectric points (pI) of 7.3-7.0 (peak 1), 5.6-5.4 (peak 2) and 5.1-4.8 (peak 3). The relative HCG content of the most acidic peak (as % of total on gel) progressively increased with time of exposure to the cAMP analogue (from 19.8 +/- 1.6% at 24 h to 34.4 +/- 4.3% at 96 h, mean +/- SEM, P less than 0.01). HCG recovered from peak 1 exhibited the highest receptor-binding capacity and in-vitro biological potency.(ABSTRACT TRUNCATED AT 250 WORDS)

Effects of neonatal androgenization on the chromatofocusing pattern of anterior pituitary FSH in the female rat.

Anterior pituitary glands were removed from neonatally androgenized (100 micrograms testosterone propionate) female rats and normal controls at 5, 10, 18, 21, 30, 60 and 90 days of age, and the multiple forms of FSH present within them were separated by chromatofocusing (pH range 7.5-4.0). Additional pituitary glands from intact adult males (90 days old) were also studied for comparative purposes. All animal groups exhibited multiple forms of immunoactive FSH within a pH range of 7.5-4.0, as well as an additional FSH form obtained after the addition of 1.0 mol NaCl/l to the chromatofocusing column (salt peak). In animals 5-30 days old (controls and androgenized) the majority of FSH applied to the chromatofocusing columns was recovered within the salt peak (45-85% of total FSH immunoactivity recovered). However, as the animals aged, more FSH immunoactivity focused within less acidic regions (isoelectric point (pI) 5.9-5.0); pituitaries from animals 60 days old contained the greatest proportion of FSH focused within this pH range (controls, 39.2 +/- 0.6%; androgenized, 23.1 +/- 0.9% of total immunoactivity recovered; P less than 0.03 vs animals 30 days old for both experimental groups). This shift towards less acidic FSH was attenuated in androgenized animals compared with the controls (P less than 0.01). In control adult rats, the chromatofocusing distribution pattern of pituitary FSH varied according to the day of the oestrous cycle. Pituitary extracts from control rats decapitated during the morning of pro-oestrus, oestrus and day 1 of dioestrus exhibited the highest proportion of immunoactive FSH (23.2-28.8% of total) focused within a pH range of 5.9-5.0, whilst only 10.4-11.6% of FSH from androgenized rats and those on day 1 of dioestrus was recovered within this pH range (P less than 0.05). In control animals decapitated during the morning of pro-oestrus and oestrus, 10-26% of FSH focused within the most alkaline region (pI 7.5-6.0); the chromatofocusing pattern of pituitary FSH from the neonatally androgenized animals was characteristic, in that no more than one peak (1.5 +/- 0.5% of total) was detected in this alkaline region. In the adult male rats, the majority of pituitary FSH eluted from the chromatofocusing columns within a pH of 4.9-4.0 (52.4 +/- 1.2% of total FSH immunoactivity) and the salt peak (pH less than 4.0) (33.1 +/- 2.4 of total). All FSH isoforms obtained after chromatofocusing represented alpha and beta dimers as disclosed by size exclusion chromatography.(ABSTRACT TRUNCATED AT 400 WORDS)

Performance of six different commercial assays to demonstrate antibodies to HIV-2 among immigrants from high-endemic areas.

Four HIV-2 and 8 HIV-1 infections were detected when serum specimens from 422 persons from high-endemic areas were examined with 6 different commercial ELISA tests. 41 specimens showed a positive result in at least one of the assays. 12 of these were confirmed as anti-HIV positive. One of the anti-HIV-2 specimens was negative in the Abbott recombinant HIV-1 test but positive in the ELAVIA II and the 4 HIV-1/HIV-2 combination tests. The 4 HIV-2 positive individuals originated from West Africa.

[Famotidine in the treatment of duodenal peptic ulcer].

A multicenter, open study was designed to evaluate the efficacy and tolerability of a novel and potent H2 receptor antagonist, Famotidine, in the treatment of duodenal ulcer. Thirty-four patients with endoscopically proven active duodenal ulcer were treated with Famotidine, 40 mg, one tablet, at bedtime, for six weeks. Follow-up endoscopic exams were done every two weeks until ulcer healing was obtained. One patient was excluded from further analysis as he failed to follow the protocol. Healing rates achieved were 48.5%, 91% and 97% by the second, fourth and sixth week of treatment respectively. After 72 hours 52% of the patients were asymptomatic and 79% at the end of the first week of treatment. Famotidine was well tolerated, no clinical neither laboratory adverse events were observed during the study. Famotidine, a novel H2 receptor antagonist appears to be effective and well tolerated in the treatment of most patients with duodenal ulcer.

[Cryptococcosis in acquired immunodeficiency syndrome].

The diagnosis of cryptococcal infection was established in 3 patients with AIDS. Infection was meningo-encephalic in all and presented a violent outset. The chemical characteristics of the CSF were normal. Detection of cryptococcal antigen, presence of encapsulated yeast forms revealed by india-ink staining and positive cultures allowed a definitive diagnosis. Response to therapy with amphotericin B alone or in combination with 5-fluorocytosine was poor. A maintenance therapy with fluconazole is recommended.

Acquired Immunodeficiency Syndrome

[Wolff-Parkinson-White syndrome: late results of surgical treatment].

We followed 25 patients operated on for Wolff-Parkinson-White syndrome between August 1985 and October 1989. Their mean age was 37 +/- 12 years and arrhythmia had been present for 5 to 30 years. A mean of 3.3 +/- 1.2 antiarrhythmic agents had failed in controlling recurrences. Tachycardia was orthodromic in 21 patients and antidromic in 1, while 3 patients presented rapid atrial fibrillation with hemodynamic deterioration. Drug refractoriness (n = 23) or intolerance (n = 2) were the main surgical indications. The location of accessory pathways was lateral in 19 patients, anteroseptal in 3, posteroseptal in 2, postero lateral in 1 and right lateral in 1 patient. One patient had a double pathway. There was no surgical mortality. After a follow-up period ranging from 1 to 50 months recurrence of arrhythmia was observed in one patient and electrophysiologic evaluation showed persistence of a left lateral pathway in another. The remaining 24 patients are free of symptoms at the end of follow up. Thus, surgical treatment is a curative therapy for most patients with WPW.

[Contusion of the abdominal aorta and massive embolization from the use of a seat belt. A clinical case].

Abdominal aortic lesion following blunt abdominal trauma is infrequent. Parmley quotes 347 cases of aortic lesion among 8710 necropsies. Of these, only 4.6% involved the abdominal aorta. Probably less than 100 cases have been reported afterwards. We report a case associated to the use of safety belt, who developed the serious complication of massive embolization.

[Albendazole in the treatment of hepatic and intra-abdominal hydatidosis].

Albendazole, in doses of 10 mg/kg/day for 28 days with 14 days rest periods between series and series, with a minimum of 3 and a maximum 6 series was administered to 16 patients, 13 women and 3 men, with hepatic and/or of the abdominal cavity hydatid cysts. Clinical, immunological and ultrasonographic criterion were used to evaluate the response, obtaining the following results: complete cure in 6 patients (37.5%), improvement in 7 patients (43.7%) and without change in 3 (18.7%). Drug was tolerated, occurring only one case of reversible total alopecia. There were not abnormalities in liver and renal function tests or in the haematological indices. As conclusion, Albendazole is a good therapeutic alternative in cases of inoperable or multiple hydatid cysts.

Ultrastructural characteristics of different stages of human chagasic myocarditis.

Myocardial damage was analyzed in the different stages of Chagas' disease. Myocardial biopsies from chagasic patients, whose clinical histories were initially unknown to the examiner, were examined and evaluated by electron microscopy using a table in which 244 characteristics were considered. When the ultrastructural results were associated with their respective clinical reports, it was found that Chagas' disease stages showed significant myocardium damage between stages IA and II: IA (normal EKG and cineventriculogram), 13.0%; IB (early left ventricular damage with normal EKG), 14.5%; II (advanced left ventricular damage, abnormal EKG), 25.5%; stage III (congestive heart failure) showed a decrement to 22.5% if compared with stage II. The acute stage of the illness was characterized by the presence of the parasites within the myocytes which were surrounded by inflammatory cell infiltrate. During Chagas' disease evolution the most affected organelle was the sarcoplasmic reticulum.
