Radioisotopes and their use in the diagnosis and management of thyroid disease.

The use of radioactive isotopes in thyroid evaluation and therapy gives important information in the endocrine assessment of the aged. Thyroid uptake imaging can be used to establish a hypothyroid or hyperthyroid state and can determine the functional status of a palpable nodule. Investigational agents, such as T1 201m DMSA, MIBG, FDG, and Gallium are reviewed in relation to the evaluation of neoplasia. A detailed discussion of thyroid carcinoma ablation and subsequent radioiodine therapy is described, as are radiation safety procedures as they relate to the elderly.

Clinical diaphanography--its present perspective.

The application of nonionizing radiations and magnetism in clinical medical diagnosis has been rapidly increasing. The incorporation of these techniques into the armamentarium of the diagnostic roentgenologist warrants a change in nonmenclature to diagnostic imageologists. This review of the clinical applications of diaphanography covers the period of simple light source examination of the 1930s to the present sophisticated imaging with infrared film or videcon T.V. computerized format. Light physics of the infrared spectrum is well known, however experimental data of the mechanism of transillumination or normal and abnormal breasts are being investigated. A short discussion of breast anatomy and pathology including benign and malignant is basic to the clinical evaluation technique. A realistic appraisal of the application, the limitations of the technique and the current ongoing investigative studies in the European and American literatures are reviewed. Other noninvasive breast diagnostic techniques of thermography, ultrasound, and NMR are included as well as mammography in the diagnosis of breast disease and cancer.

Preoperative work-up in breast surgery.

Our patients who are to undergo breast surgery are placed into one of four groups with a number of subdivisions. Each has a variable amount of preoperative work-up depending on the patient's age and constitutional status, the degree of suspicion as to the malignancy of the lesion, the type of anesthesia (local or general), the place where the biopsy is being done (in-patient or out-patient)and on the type of informed consent signed by the patient. Breast x-rays are used on a judicious and selective basis and thermography and ultrasound are used routinely.
