OBJECTIVE: To seek a relation between pregnant women's psychosocial condition, assessed clinically in general practice, and prematurity and birth weight of the child. DESIGN: Clinical assessment early and late in pregnancy of psychosocial condition based on all accessible information, which included an extended history on psychosocial condition supplemented by background information about the woman and her social network and clinical observations. SETTING: Antenatal care in a general practice. RESULTS: The women assessed as having psychosocial difficulties were younger, had less education, and gained less weight in pregnancy. Birth weight was related to parity, body mass index before pregnancy, and total weight gain in pregnancy. A relationship was found between difficult psychosocial conditions and prematurity and low birth weight of the child. If the assessment of psychosocial condition had been based on the women's verbal descriptions of their problems alone, no relationship would have been found between psychosocial difficulties and prematurity and birth weight of the child. CONCLUSION: A clinical assessment of a woman's psychosocial condition, in which all accessible information is used, and an emphasis on emotional and social support for women when needed, may be necessary for improvement of antenatal care.
Pregnant women's ailments may cause anxiety and reduce the quality of life. Sixty-five pregnant women were followed prospectively in antenatal care in general practice, and spontaneously reported ailments not associated with serious pregnancy complications were registered prospectively. The vast majority of pregnant women spontaneously reported one or more ailments. A higher number of ailments was reported by women with psychosocial problems and heavy physical work. Abdominal pain was more often reported by women assessed as having psychosocial problems. Based on background knowledge, clinical observations and a somewhat extended history on the psychosocial situation, it is possible to identify a subgroup of women who experience a more than 50% increase in the frequency of such ailments. Reporting of a high number of ailments in antenatal care may provide the clinician with important clues regarding need for psychosocial support.
In pregnancy the woman's emotional sensitivity and also her receptiveness of emotional support increase. Antenatal care that is exclusively focused on somatic conditions may lead to ignorance of pregnant women's psychosocial problems, thus missing a chance of intervention. A psychosocial questionnaire, used as a clinical tool in history taking during the first two antenatal consultations, is described and evaluated. The purpose of the questionnaire was to elucidate the pregnant woman's psychosocial assets as well as problems. The information collected by means of this questionnaire is compared with the information on psychosocial conditions collected by means of the traditional antenatal care form. Screening for psychosocial conditions in pregnant women may reveal information about important life aspects that are not detected in the antenatal care form.
In clinical work in general practice decisions are based on biomedical data as well as on clinical knowledge rarely analysed in research. In this study all accessible clinical knowledge was used by the doctor in an assessment of pregnant women's psychosocial conditions and needs for intervention during antenatal care. Components of clinical knowledge (verbal and non-verbal communication, background knowledge about the woman and her network, and the clinician's own feelings during consultation) are described and discussed. The analysis of the material showed that, when there was a discrepancy between the GP's assessment (clinical observations and background information) and the woman's own verbal description of her psychosocial situation, the GP gave precedence to the former. Different information sources seem to be closely linked. The doctor's own feelings and evaluations will influence the atmosphere in the consultation, and through this the quality of information collected.

331 (54%) of Norways 609 female general practitioners responded to a questionnaire about their research experience, interest in research on general practice and the barriers experienced to doing research. 41 were actively engaged in research, five at a Ph D level. 38 had completed research earlier. 27 had collected material without publishing any results and were not actively engaged in research at the time of the investigation. 223 were interested in doing research if the conditions could be adapted to their needs. Half the respondents regarded caring for the family and professional strains at work as barriers to research. It is necessary to systematically recruit female general practitioners to research in order to enrich research on general practice by female perspectives and experience, and in order to influence the development of methods and the areas of research.

A 17-year-old girl swallowed at least 50 Duroferon duretter. Each tablet contains ferrous sulphate equivalent to 0.1 g Fe2+. Thus, a total of 5 g Fe2+ was ingested. Vitamin K-dependent coagulation factors dropped within the next hours to very low levels. Thrombotest showed less than 3% of normal coagulation activity 8 h after oral intake. Recovery was uneventful except for laboratory evidence of transient liver damage. The rapidity by which the early coagulation deficiencies developed and the lack of evidence of disseminated intravascular coagulation suggested a direct effect of iron on coagulation factors. In vitro studies confirmed that iron in concentrations that may have been attained in vivo, altered the functional activity of several coagulation factors. Monitoring the vitamin K-dependent coagulation factors may be a simple and useful parameter in acute iron intoxication.
