Coronary thrombosis in non-cardiac death.

BACKGROUND: Coronary thrombosis is the major cause of acute myocardial ischaemia but can be, albeit rarely, clinically silent. We investigated a series of autopsy hearts from hospitalized patients who died from non-cardiac causes, to detect and study coronary thrombosis. METHODS: The series consisted of 132 autopsy cases (81 men and 51 women, age range 32-39 years, mean 63 +/- 14), in whom cause of death was confirmed as extracardiac. Major epicardial coronary arteries were isolated from the hearts and routinely processed for histopathological study. We evaluated the presence of coronary atherosclerosis and thrombosis. Plaque size was histologically graded with low magnification lenses. RESULTS: Coronary atherosclerosis, which was found in 110 hearts, caused critical stenosis or occlusion of at least one major vessel in 55 (41.6%) cases. Coronary thrombosis was found in 10 vessels from nine different hearts. One coronary tree presented two thrombi in two different vessels. Thrombi were mural in all but one vessel. We did not observe either deep sub-thrombotic ulceration or atheromatous material mixed with thrombus. Deep thrombus layers often presented organizing features. There was no correlation between thrombosis and degree of vessel stenosis, which was only mildly increased by thrombus. Plaque rupture without thrombus was found in five coronary arteries of five different cases. CONCLUSIONS: Coronary thrombus may overlay the intima of a diseased vessel independently of plaque type and severity. Moreover, thrombosis is more frequent than expected, although it is rare when compared with the spread of coronary atherosclerosis. It may represent a plaque progression mechanism in the natural history of coronary atherosclerosis.

Rioprostil and the gastric mucus-bicarbonate barrier.

The effects of a single oral dose of rioprostil, 150 micrograms, rioprostil, 300 micrograms, or placebo on gastric mucus and bicarbonate secretion were investigated in humans under basal conditions and during pentagastrin infusion. The study was carried out in healthy male volunteers according to a double-blind crossover design. Rioprostil, 300 micrograms, significantly stimulated basal mucus secretion and induced a significant increase in mucoprotein and bicarbonate output during pentagastrin infusion. The results suggest that rioprostil enhances the gastric mucus-bicarbonate barrier in man.

Effects of kallikrein on the male reproductive system and its use in the treatment of idiopathic oligozoospermia with impaired motility.

A single-blind study on the effects of kallikrein in idiopathic oligozoospermia was carried out. 8 of the 16 patients admitted to the trial were treated with kallikrein 100 K.U. t.i.d., the remaining patients received vitamin E 100 mg t.i.d. Treatment lasted 120 days in both groups. Kallikrein led to an increase in sperm concentration, total sperm count and sperm penetration capacity. Patients receiving vitamin E failed to show any improvement. It is postulated that the increase in sperm concentration could be explained by the positive action of kallikrein on spermatogenesis, due to a stimulatory effect of the drug directly on the Sertoli cell function and on the circulation in the vascular system of the testis.

Gastric effects of a prostaglandin E1-derivate (rioprostil) on acid, alkaline, and mucus secretion.

In a double-blind crossover study, the gastric effects of a single oral dose of rioprostil (150 and 300 micrograms) or placebo were compared in basal conditions and during a 120-minute pentagastrin infusion. A slight decrease in basal acid output and a nonsignificant rise in basal bicarbonate secretion were observed with the 300-micrograms dose. Pentagastrin-stimulated acid secretion was reduced by the prostaglandin derivate, especially with the 300-micrograms dose. Compared with placebo, 300 micrograms of rioprostil significantly stimulated basal mucoprotein secretion and bicarbonate and mucus output during pentagastrin infusion. The results suggest that 300 micrograms of rioprostil exerts both a moderate antisecretory activity and a strengthening effect on the gastric mucus-bicarbonate barrier.

The treatment of male subfertility with kallikrein.

In a controlled double blind trial the effectiveness and tolerability of Kallikrein in patients suffering from astheno-oligozoospermia was examined in comparison to placebo. 30 patients with astheno-oligozoospermia of idiopathic origin entered the study; they were subdivided into two homogeneous groups of 15 patients each. Group A was treated with 600 K.U. daily for 3 months, group B received placebo tablets. In the Kallikrein group all parameters improved, as sperm motility, volume of semen, sperm concentration, total sperm value, and rate of normal spermatozoa. No changes of endocrinological findings were observed. Three pregnancies occurred in this group. No conceptions were observed in the wives of those patients treated with placebo (group B). Tolerability was very good in all patients.

Spermatic cord liposarcoma: a report of four cases.

Spermatic cord liposarcoma is a rare pathology (1-4); currently about one hundred cases are documented. The therapy of choice is surgery, followed sometimes by radiotherapy. We herein describe our experience of 4 cases between 1995 and 2000, with median follow-up of 34 months (mean 48 months, range 28-95 months), in order to stress the role of orchifuniculectomy, even when mass-ablation first procedure may seem radical.
