Removal systems of immunoglobulin A and immunoglobulin A containing complexes in IgA nephropathy and cirrhosis patients. The role of asialoglycoprotein receptors.

BACKGROUND: Whereas the complex removal routes hypothesized for IgA containing immune complexes (IC) and macromolecules can be adequately analyzed by a recently proposed IgA1-IgG aggregate probe (Lab Invest, 66: 86-95), the relative significance of the asialoglycoprotein receptors in IgAIC clearance is still uncertain. EXPERIMENTAL DESIGN: The removal kinetics of 99mTc diethylenetriamine-pentaacetic acid-conjugated asialo alpha 1 acid glycoprotein (AAGP) and 123I-labeled IgA1-IgG aggregate were analyzed in 11 cirrhosis patients and 13 IgAN patients of comparable age. RESULTS: IgA1-IgG aggregate mean plasma clearance rate was delayed in IgA neuropathy (IgAN) patients (slope 0.038 minutes-1, range 0.027 to 0.053) compared with normals (0.047 minutes-1, range 0.038 to 0.053, p = 0.05). The liver was the main organ involved in the IgA1-IgG removal. When compared with normals, (34.3 minutes, range 29.8 to 42.2), the liver mean transit time (MTT) was significantly (p < 0.02) prolonged in IgAN patients (41.3 minutes, 33.6 to 52.3). Participation of spleen in clearance was observed in some patients and was almost invariably concurrent with normal clearance parameters. Conversely, 9 out of 11 cirrhosis patients had a remarkable splenic uptake, but the blood clearance rate was invariably delayed (0.022 minutes-1, 0.014 to 0.028, p < 0.003) and liver MTT extremely prolonged (122.4 minutes, 52.4 to 400, p < 0.003). In IgAN patients with delayed clearance of the IgA1-IgG aggregate, a distinct trend of progression towards renal failure was noted. AAGP clearance was also delayed in cirrhosis patients: slope = 0.166 minutes-1, 0.108 to 0.247, p = 0.05 as compared with both normals (0.230, 0.173 to 0.289) and IgAN patients (0.250, 0.184 to 0.254). Liver MTT in cirrhosis patients was extremely prolonged: 240.6 minutes, 132.5 to 400 minutes, p < 0.007 compared with both normals (90.0 minutes, 82.7 to 96.6) and IgA patients (92.2 minutes, 70.3 to 107.1). AAGP clearance parameters in normals and IgAN patients were not statistically different. MTT values of AAGP and IgA1-IgG aggregate were strictly related (p = 0.008), suggesting that asialoglycoprotein receptors are partially involved in the clearance of the IgA1-IgG aggregate probe. CONCLUSIONS: Some patients with IgAN have a prolonged circulation of an IgAIC miming probe, probably due to an impaired macrophage function. Other possibilities of prolonged circulation of IgAIC in these patients should imply an abnormal IgA glycosylation pattern that allows IC to escape from an effective asialoglycoprotein receptor system. In cirrhosis patients, all of the removal routes of IgA and IgA containing IC are greatly altered suggesting a causative role in the development of an associated, often clinically inapparent, glomerular disease.

Sumatriptan does not affect vasopressin secretion in humans.

The aim of the study was to investigate the effects of a new serotoninergic drug, sumatriptan, on arginine vasopressin secretion in humans. Plasma vasopressin concentrations were determined in eight healthy volunteers, before and after administration of 6 mg of sumatriptan, or placebo. No changes in hormone levels were found after sumatriptan or placebo administration. The results suggest that in humans serotoninergic mechanisms, which modulate vasopressin secretion, do not involve the serotonin receptor activated by sumatriptan.

Twenty-four-hour beta-endorphin secretory pattern in Alzheimer's disease.

A chronobiological study was carried out in 6 male patients (67-71 years), suffering from Alzheimer-type dementia (ATD) and 6 male patients (52-74 years) suffering from multi-infarct dementia (MID), to evaluate their 24-hour beta-endorphin and cortisol secretory patterns. Six healthy male adults (28-37 years) and 6 healthy elderly male subjects (78-84 years) constituted the control groups. Blood samples were drawn every 4 h from 8.00 to 20.00 h and every 2 h from 24.00 to 6.00 h. Mean 24-hour beta-endorphin levels were significantly (p < 0.05) higher in the ATD patients (39.2 +/- 1.5 ng/l) than in the other groups (33.8 +/- 1.1, 30.1 +/- 1.6 and 33.2 +/- 1.1 ng/l in the elderly subjects, the adults and the MID patients, respectively). The circadian rhythm was absent in the ATP patients, in the elderly subjects and the MID patients. No differences in plasma cortisol circadian rhythm were observed among the four groups. Our data indicate that changes in circulating beta-endorphin concentrations and circadian pattern may be due to the aging process.

Thyroid hormones and atrial natriuretic hormone secretion: study in hyper- and hypothyroid patients.

Plasma atrial natriuretic hormone (ANH) values were evaluated in 28 hyperthyroid patients and in 11 hypothyroid patients and compared with 20 healthy subjects. In hyperthyroid patients plasma ANH basal levels were significantly (p less than 0.01) higher (14.2 +/- 1.6 pmol/l) than in controls (7.8 +/- 0.4 pmol/l) and in hypothyroid patients (6.4 +/- 0.3 pmol/l). No significant differences were found between controls and hypothyroid patients. The propranolol-induced decrease in heart rate in hyperthyroid patients did not significantly affect the plasma ANH values. Conversely, after the methimazole-induced euthyroidism a return within the normal range of ANH values was observed. The thyroxine replacement in hypothyroid patients determined a small but significant (p less than 0.05) increase in plasma ANH values. Observed data suggest that in humans thyroid hormones may influence plasma ANH concentrations independently of their effect on heart rate.

[The effect of the use of sinusoidal modulated currents on the recovery of athletes].

Modular sinusoidal currents were used to optimize the recovery process of the force of athletes' muscles. 113 elite athletes were analyzed. They were participants in rowing, cycling, tennis and Greco-Roman wrestling. In all cases the modular sinusoidal currents were applied to the dorso-cervical and lumbar regions. It was found that applications to the dorso-cervical regions normalize the altered parameters of the cardiovascular system and increase the capacity of the humeral muscles to contract. Furthermore they stabilize the adrenergic sympathetic zone and stimulate the immune system.

[Meteoropathy: a syndrome continuously on the increase].

The authors describe the different kinds of meteoropathies, pointing out how these disorders are becoming ever more frequent in countries belonging to the consumer and welfare society due to the progressive lowering of body resistance and immunologic responses. These "unfavourable meteosyndromes", as meteoropathies should be called, develop when wave disturbances arrive, and are more manifest and persistent in subjects with neurologic disturbances, i.e. in the presence of anxiety, stress, hypertension, coronary artery disease, arthropathy, hyperthyroidism, etc. The different moments in which meteoropathies are more frequent and the possible therapies are also discussed.

[Correction by physiotherapy of immune disorders in high-grade athletes].

It is well known that during intense training and competitions athletes show a remarkable reduction of their immunological responses. As a matter of fact, during these periods athletes very frequently (five to ten times more than average) fall ill. In order to correct this situation, in the USSR immunomodulator drugs are used together with high frequency electromagnetic waves which are applied on the body surface corresponding to the adrenal glands. In this way body desensitisation is accelerated. One hundred high-level athletes received an application of electromagnetic waves on the body surface corresponding to the thyroid gland and thymus, so as to lower glucocorticoid activity and activate lymphoid tissue. Indices of immunologic reactivity and hormonal levels after electromagnetic treatment were measured in all athletes.

Effects of ursodeoxycholic acid (UDCA) on serum liver damage indices in patients with chronic active hepatitis. A double-blind controlled study.

The effects of ursodeoxycholic acid (UDCA, 450 mg daily) in patients with histologically proven chronic active hepatitis (CAH) have been evaluated in a randomized, double-blind, placebo-controlled study. Twenty-six patients with serum alanine aminotransferase (ALT) values at least twice the normal upper limit in two of three pre-treatment tests received UDCA or a placebo for twelve weeks. In all UDCA-treated patients, serum aspartate amino-transferase (AST), ALT, gamma-glutamyl transpeptidase (GGT) and alkaline phosphatase (AP) fell significantly after 4 weeks of treatment. There was a further decrease at the end of therapy, as well as a small but significant fall in total serum bilirubin. Conversely, 4 weeks after suspension of therapy, serum enzyme levels had increased, reaching values not much lower than those recorded before treatment. Total serum protein, albumin and gamma-globulin did not change after UDCA treatment. In the placebo group no significant variation in the test results were found. The results indicate that UDCA therapy in CAH, as has been observed in primary biliary cirrhosis and primary sclerosing cholangitis, is able to improve several indices of liver damage, without producing any toxic adverse effects.

Plasma somatostatin response to an oral mixed test meal in cirrhotic patients.

Ten patients with non-alcoholic cirrhosis and ten control subjects were studied in basal conditions and after ingestion of a standard mixed test meal. Plasma somatostatin, blood glucose, plasma insulin, C-peptide and glucagon were determined before and 15, 30, 45, 60, 90, 120 and 180 min after the start of the meal. Basal somatostatin levels in patients (31.9 +/- 1.8 ng/l) were significantly higher (p less than 0.01) than in controls (12.5 +/- 0.9 ng/l). The time-course of the somatostatin secretory response after the meal was similar in the two groups, but the increase, evaluated as incremental area above baseline, was significantly smaller (p less than 0.01) in cirrhotics (804 +/- 134 ng/l per min) than in controls (1482 +/- 149 ng/l per min). Data indicate that elevated basal plasma somatostatin concentrations in cirrhosis may be consequent to elevated gastrointestinal and/or pancreatic secretion, whereas the blunted somatostatin response to the mixed test meal may derive from the hyperinsulinemia which occurs in the postprandial period.

An improved synthesis of the inert, diffusible blood-flow tracer, [18F]fluoromethane.

An improved procedure for producing [18F]fluoromethane ([18F]FM) in batches of several hundred mCi is reported. 18F prepared by the 18O (p, n) 18F reaction in a H2(18)O target is trapped on a small column of Dowex 1 (x 10) resin to allow recovery of H2(18)O. One new feature is elution of 18F- from the column with 3 mL of CH3CN containing 67 microL 1.5 M aqueous (Bu)4N+ OH-, after residual H2(18)O has been removed with dry CH3CN. This 18F- solution reacts with CH3I in the presence of Ag2O directly to give [18F]FM, which is swept out of the reaction vessel with a stream of air, from which CH3I and other vapors are removed with a C18 SEP-PAK at room temperature. Another new technique is trapping of [18F]FM on a second SEP-PAK cooled in ethanol/dry ice. After warming the SEP-PAK to room temperature, the trapped [18F]FM can be recovered with either H2O or air. The improvements speed the preparation and minimize hands-on operations. The product has no detectable radiochemical impurities, and a specific activity of greater than 1 Ci/mumol. Non-radioactive CH3CN, CH3I and CH3OH are present at less than 0.2 mumol per batch.

Influence of suckling and continuous cow-calf association on the resumption of post-partum ovarian function in Bos indicus cows monitored by plasma progesterone profiles.

The effect of suckling on post-partum ovarian function was monitored by weekly plasma progesterone determinations using enzyme immunoassay in 16 Small East African Zebu (Bos indicus) cows maintained with a fertile bull. Five cows continuously suckled their calves (COS), but starting 30 d after calving 6 cows remained with their calves which were prevented from suckling using nose plates (NOS) while 5 calves were separated and penned (PEN). The mean (+/- SE) interval from calving to first behavioural oestrus was 81 +/- 11 d and was significantly (P less than 0.001) shorter for PEN (43 +/- 10 d) than NOS (72 +/- 9 d) or COS cows (133 +/- 11 d). COS cows nonetheless weaned heavier (P less than 0.001) calves at 5 months. Ovarian activity determined by plasma progesterone levels greater than or equal to 1.3 ng/ml indicated that "silent ovulation" was initiated in 44% of cows by 40 +/- 6 d post-partum. The results indicate that continuous suckling or constant cow-calf interaction extend the post-partum anoestrous interval in Ethiopian Zebu cows. However, early-weaned calves or those on restricted suckling would need extra nutritional supplementation to ensure continued growth.

Metoclopramide increases plasma but not cerebrospinal fluid vasopressin levels in man: study in hydrocephalic patients.

Arginine vasopressin (AVP) concentrations were determined in plasma and in cerebrospinal fluid (CSF) in 8 adult male patients suffering from hydrocephalus of various etiologies, before and after intravenous administration of 10 mg metoclopramide. Metoclopramide was able to increase the plasma (2.6 +/- 0.2 ng/l in basal conditions and 6.1 +/- 0.6 ng/l at 30 min) but not the CSF AVP levels. The results suggest that the neurons which secrete AVP into the CSF may be functionally different from those secreting into the peripheral circulation.

Influence of suckling on plasma concentrations of somatostatin, insulin and gastrin in lactating women.

The effects of suckling on plasma somatostatin, insulin and gastrin values were evaluated in eight nursing women on the 3rd to 4th day postpartum. Plasma prolactin levels were also determined. Prolactin levels increased in response to suckling, as expected. Insulin levels also rose, whereas somatostatin and gastrin concentrations did not change after suckling. It is possible that the suckling-induced hyperinsulinemia blunts the somatostatin response to suckling in humans.

Intramedullary osteosynthesis. 4. Küntscher nailing in the ulna.

Intramedullary osteosynthesis with Küntscher nailing is an appropriate and extremely effective operation in the treatment of diaphyseal fractures of the ulna. Like the other long bones of the limbs (femur, humerus, tibia), in the ulna, as well, the advantages of the method, particularly when performed in closed surgery, are undeniable, so much so that this bone has a shape which is particularly favourable to nailing. The authors present and discuss their personal series of cases reporting results which are excellent. Thus, once again, the great reliability of this simple means of synthesis, in use for more than 20 years, is emphasised, and the subject is completed with a series of previous technical suggestions on how to obtain the best results.

The association of two or more surgical methods in the tibia.

The authors propose the association of two or more surgical methods in the treatment of aseptic pseudarthrosis of the tibia with the object of increasing the probability of union and reducing the period of disability. Based on the anatomo-pathological features of the pseudarthrosis, methods aimed at obtaining mechanical stability (fibula pro tibia, Küntscher) and methods which stimulate biological repair (decortication, transplants) are associated. The results obtained confirm the theoretical presuppositions, revealing a relatively short amount of time required for healing.
