Allergic contact reaction to dexpanthenol: lymphocyte transformation test and evidence for microsomal-dependent metabolism of the allergen.

In a patient with contact dermatitis, dexpanthenol was found to be the causative allergen. There was a positive reaction to dexpanthenol on patch testing. Controls did not show any positive reactions to dexpanthenol on patch testing. Additionally, an LTT was performed. After preincubation with dexpanthenol-modified microsomes, we observed an increase in lymphocyte proliferation to dexpanthenol, in comparison to dexpanthenol without microsomes, suggesting that microsomal metabolism plays a rôle in the pathogenesis of dexpanthenol sensitization, because microsomes are known to possess drug metabolizing enzymes such as cytochrome P450.

Chemiluminescence microscopy reveals functional heterogeneity in single neutrophils undergoing oxygen burst.

Activated polymorphonuclear leukocytes (PMN) respond to various triggers with an oxygen burst, during which the release of reactive oxygen species (ROS) plays a key role in microbial killing. The biological function of the ROS-associated light emissions is not known. However, this particularly weak cell-derived chemiluminescence (CL) may serve as a parameter for the identification of PMN activation. In this study we describe a novel technique which we termed CL microscopy. A microscope-based low-light image-processing system was applied which was sensitive enough to detect single photons, capable of two-dimensional signal accumulation, and digital image analysis. This technique permitted, for the first time, the visualization of the oxygen burst in single cells. Furthermore, quantitative evaluation of cell-derived luminol-enhanced CL revealed functional heterogeneity. Single-cell investigations of activated living PMN of normal human donors showed clear differences in kinetics and intensity of the oxygen burst related to different stimuli. The chemical agent (phorbol 12-myristate 13-acetate) induced CL in 83% of PMN. In contrast, the complement-mediated phagocytic stimulation by opsonized zymosan gave much higher light intensities of individual cells, but only in part of the PMN population (30%). CL microscopy presents a new and highly sensitive technique with considerable potential for single-cell analysis in immunological research.

Assembly of nonneural microtubules in the absence of glycerol and microtubule-associated proteins.

Microtubule protein from Ehrlich ascites tumor cells purified by an in vitro polymerization process in the absence of glycerol and calcium chelators contains several accessory proteins but lacks the high molecular weight proteins which are present in neurotubulin. DEAE-Sephadex chromatography of two-times cycled tubulin removes these nontubulin proteins, resulting in pure tubulin, as critically examined by sodium dodecyl sulfate gel electrophoresis. This tubulin can readily assemble into microtubules in assembly buffer, at low magnesium concentrations, without glycerol and at tubulin concentrations above 0.8 mg/mL. Electron microscopy shows that the tubules are identical with normal microtubules. When the purified tubulin fraction was reduced and carboxymethylated, a significant minor protein component could be observed electrophoretically, migrating between alpha- and beta-tubulin. At present, the identity and function of this protein are not known. The results demonstrate that the in vitro assembly of tubulin from Ehrlich ascites tumor cells does not require high molecular weight proteins or gamma-like factor(s) as has been proposed for the neurotubulin system.

[Report of a case of aplasia of the cervix and discussion of the embryology and clinical significance of the condition (author's transl)].

A case of aplasia of the uterine cervix and partial atresia of the vagina in a 17 year old woman is reported. A hysterectomy was done and a new vagina created. The literature on the origin of the different parts of the female genital tract is reviewed and discussed. Interpretation of the embryologic origin of the different parts is difficult. The indication for the hysterectomy was prevention of further endometriosis by retrograde menstruation, high probability of infertility because of the lack of the cervix and high probability of ascending infection should a new vagina be connected directly to the endometrial cavity. The case is presented to generate discussion on the management of cervical aplasia.

Adolescent

[Partusisten as a possible cause for renal colics (author's transl)].

Two patients were treated with Partusisten and Isoptin to inhibit uterine contractions. One patient was in the 36th and the other in the 32th week of pregnancy. Both developed colic like complaints related to the urogenital system, which disappeared after disruption of treatment and reoccured after restarting. The case description seemed to indicate an as yet not described side reaction of Partusisten.

Colic

[Morbus Hodgkin in autopsies today (author's transl)].

In Hodgkin's disease, good clinical results are obtained by the modern combined treatment with irradiation and chemotherapy. An examination of autopsy findings show in the period 1955-1964 a mean survival time (measured from first biopsy until death) of 22 month, in the period 1965-1973 of 42 month. In contrast to first biopsies, autopsies have a prevalence to the reticular "Hodgkin-sarcoma" .41% of all cases are of this type. 12 of these cases had changed from the lymphocytic predominaut type or from nodular sclerosis, to the reticular type. This augmentation of the reticular type is seen as a consequence of modern therapy. In 18 autopsies, histologic classification was rendered impossible by massive scarring. 8 cases of the period 1965-1973 did not show any lymphogranulomatous tissue and could be morphologically defined as "cured". Therapeutic damages were found in 15 cases. 6 cases had diffuse pulmonary fibrosis. The therapy provoked scars could be differentiated histologically from the hyalinizations occurring in nodular sclerosis and in diffuse fibrosis.
