Induction by sodium butyrate of cytomegalovirus replication in human endothelial cells.

Cultured endothelial cells are shown to be induced in regard to permissiveness to human cytomegalovirus by temporary treatment postinfection with sodium butyrate (1-2 mM). Drug-treated cells are demonstrated to exhibit expression of immediate early and early viral antigens, synthesis of viral DNA and viral structural glycoprotein B. Progeny virus could be visualized by electron microscopy.

[Continuing education of professionals in rehabilitation is coordinated].

In its programme of action "Rehabilitation in the Eighties", the Federal Government declared that coordinated further education for professional workers in rehabilitation administrations and rehabilitation facilities was to received increased attention in the future. Efficient rehabilitative work presupposes adequate qualification for these tasks in those entrusted with providing rehabilitation services to persons with disabilities. On-going further education of professional workers is indispensable in achieving this objective, in addition to their initial qualification. Hence, the financially responsible rehabilitation administrations and the Federal Rehabilitation Council, on the initiative of the Federal Minister of Labour and Social Affairs, agreed to carry out inter-agency education seminars. For professional workers employed in rehabilitation facilities, the working groups of rehabilitation facilities have started such inter-facility further education in 1986, in coordination with the Federal Minister of Labour and Social Affairs.

[Can the number of thromboembolisms incidents in total hips replacement be further reduced?].

QUESTION: Is it possible to reduce the rate of thrombembolic events during total hip arthroplasty (THA) by the intraoperative use of a mechanical device for thrombembolic prophylaxis (A-V Impulse System). METHOD: The efficacy of additional mechanical thrombembolic prophylaxis was investigated in a prospective randomised trial on 104 patients who underwent cementless THA. All patients received low dose heparin. In 52 patients the additional application of the A-V Impulse System was introduced intraoperatively for the next 24 h. A perfusion scintigram of the lung was performed before and 7 days after the operation in both groups to detect embolic patterns. RESULTS: Perfusion deficits typical for thromboembolic events were detected postoperatively in 8 patients (15.4 %) of the controls and 5 patients (9.6 %) of the A-V Impulse System group. CONCLUSIONS: With the perfusion lung scan we saw postoperatively a total of 13 patients (12.5 %, n=104) with an asymptomatic microthromboembolism of the lungs. Hence it is necessary to increase the protection against thromboembolism. Although statistically not significant (p=0.374), there is a tendency that the additional use of the mechanical thrombotic prophylaxis can reduce thromboembolic events in THA. The impact of the intraoperative introduction of the A-V Impulse System is not really clear. However, for a higher velocity of blood flow during the operation it makes sense to start the procedure intraoperatively.

[The subacromial impingement syndrome].

The subacromial impingement syndrome describes the pathological contact between the rotatory cuff and the acromion. It is only a symptom and not a generic term for the different pathological diseases of the subacromial region. It is the diagnostical aim to verify the pathogenesis of the impingement syndromes and it is essential to differentiate structural versus functional causes. The diagnosis "impingement syndrome" should not have an acromioplastic procedure as general consequence. Because the expression "impingement syndrome" does not show causal approach it should no longer be used as a diagnosis. The term subacromial syndrome, in conjunction with the underlying pathological cause, acquires more therapeutic and prognostic clarity.

[Long-term results of surgically treated scaphoid bone pseudarthrosis].

204 cases of pseudarthrosis of the scaphoid bone have been treated by several operation methods. Reviewing our patients after an average time of 6 years we are able to estimate the results. The Matti-Russe method was the most frequent operative procedure causing good and excellent results. Concerning to the worse blood circulation of the proximale fragment most problems came from the treatment of the necrotic proximale fragment. Using several methods of operation only the early results (up to 3 years) were satisfactory. Within the following years collapsing of the spacers or dislocation of the silicone-implants appeared and loss of function and pain followed. We thought that it is indispensable to find new surgery methods to treat the problematic cases of necrotic proximal fragments by using bone graftings, so that the late results also will be successful.

[Dysphagia as a symptom of a ventral cervical prolapsed disk].

Ventral osteophytes, which occur frequently, usually cause no symptoms. They are sometimes identified as the cause of dysphagia. Cervical disc prolapses which cause symptoms of dysphagia are far less common. This paper describes a patient with dysphagia in whom ventral cervical disc prolapse was finally diagnosed by MR tomography, after she had been through a "diagnostic odyssey". The result was subsequently confirmed at surgery. The relevant literature is also reviewed.
