[The ventilatory response to CO2 during end-expiratory negative extra-thoracic pressure and PEEP].

The effects of end-expiratory negative extra-thoracic pressure (EENETP) and PEEP on the ventilatory response to CO2 were studied in seven healthy volunteers. The changes in functional residual capacity during EENETP -20 cmH2O and PEEP 5 cmH2O were 13.9 and 12.9 ml.kg-1, respectively. The slopes of CO2 response (minute ventilation/end-tidal CO2) during ZEEP, EENETP and PEEP were 0.913, 0.622, 0.693 l.min-1.mmHg-1, respectively. The slopes during EENETP and PEEP were significantly decreased. These results indicate that EENETP and PEEP could worsen the CO2 response in patients with respiratory failure, especially, with chronic obstructive pulmonary disease in which functional residual capacity is increased.

[A case of progressive systemic sclerosis with Sjögren's syndrome complicated by nephrotic syndrome caused by bucillamine].

A 53-year-old woman was admitted with complaints of edema in the lower legs. The patient had developed polyarthralgia in 1982 and had been treated with D-penicillamine and prednisolone under the diagnosis of rheumatoid arthritis. She then had developed sclerosis of skin as well as dry eyes and dry mouth. In November 1987, bucillamine treatment had started and proteinuria had appeared in February 1988. Her blood showed hypoproteinemia and was positive for anti-Scl-70 antibody test and for anti-SS-A antibody test. The skin biopsy and labial salivary gland biopsy revealed the increased collagen fibers in the subcutaneous tissue and the round cell infiltration around the salivary ducts. We diagnosed her as progressive systemic sclerosis (PSS) with Sjögren's syndrome (SjS) complicated by nephrotic syndrome. Subsequently a renal biopsy was performed. There were the granular deposition of IgG and complements in the glomerular capillary wall and electron dense deposits in the subepithelial region. These findings suggested that she had membranous nephropathy. Since such a renal lesion was rarely observed in PSS and SjS, it was highly probable that bucillamine induced the nephropathy. The bucillamine treatment was discontinued about 2 months after the nephrotic syndrome had developed. However, the nephrotic syndrome has continued for additional 8 months. Thus it is concluded that the renal damage is one of the important side effect caused by bucillamine.

The silent period in the masseter and the anterior temporalis muscles in adult patients with mild or moderate mandibular dysfunction symptoms.

The electromyographical silent period in the masseter and the anterior temporalis muscles during tooth tapping and jaw jerk were studied in patients with fairly mild temporomandibular joint dysfunction symptoms. The length of the silent periods in the patient group did not differ generally from that in a control group. During tooth tapping, however, patients with distinct muscular disorders had shorter silent period duration (7.7 ms) than patients with other symptoms or when compared with control subjects (10.5 and 11.3 ms, respectively). The duration returned to normal after correction of the muscular disorders. This finding suggests that the duration of the silent period is affected by the muscle condition. Patients with obvious muscular disorders of mild to moderate magnitude, thus, may show a shorter silent period duration during tooth tapping.

[A case of schwannoma of intrathoracic right phrenic nerve].

A 22-year old man was admitted because of an abnormal shadow on his chest X-ray film. Radiographic findings revealed that the tumor was located in the right middle mediastinum. At operation a smooth round tumor was found arising from the right phrenic nerve. The tumor was removed with phrenic nerve. Pathohistological examination revealed this tumor was benign schwannoma. Intrathoracic schwannoma usually arise from intercostal and sympathetic nerve and that arising from phrenic nerve is very rare. We removed the tumor by means of cutting his right phrenic nerve because of complete resection. Postoperatively he did well without any respiratory distress. And his respiratory function studies recovered normal 9 months after the operation. We think that schwannoma arising from phrenic nerve should be removed completely and in such a case cutting of the one side phrenic nerve is at ease if the patient have normal respiratory function.

The silent period in the masseter and anterior temporalis muscles in young adult subjects unaware of mandibular dysfunction symptoms.

In fourteen individuals unaware of mandibular dysfunction symptoms, latency and duration of the silent period in the masseter and anterior temporalis muscles at tooth tapping and jaw jerk were reasonably reproducible after 5 weeks and 5 months. Although the subjects felt no discomfort in the stomatognathic system, in some of them mild to moderate muscle tenderness, TMJ sound and occlusal disharmony were found at clinical examination. The relationship between these symptoms and the silent period was analysed. Muscle tenderness was the only factor to affect the silent period. The subjects with muscle soreness had a shorter duration of the silent period than the subjects without such symptoms.
