Electrocardiogram gated cardiac computed tomography (CT) was performed to evaluate the usefulness in the measurement of left ventricular volume and left ventricular wall thickness in 25 patients; five with angina pectoris, five with old myocardial infarction, eight with hypertrophic cardiomyopathy, four with dilated cardiomyopathy, and three healthy men. The left ventricular volume was calculated as the sum of the volume of each slice, which was area times thickness of the slice. The left ventricular wall thickness was measured in reconstructed short-axis view at the level of the mitral valve and papillary muscle. The left ventricular volume and left ventricular wall thickness determined by CT were compared with that by left ventriculography (LVG), and that by two-dimensional echocardiography (2 DE), respectively. The following results were obtained. There were good relationships between left ventricular volume, end-diastolic volume, end-systolic volume, ejection fraction by CT and those by LVG, but left ventricular volume by CT was smaller by about 20% than that by LVG. In the measurement of stroke volume, no relationship was recognized between CT and LVG. Concerning the measurement of left ventricular wall thickness, more available figures were obtained by CT than by 2 DE, particularly at the apical region, lateral wall, and posterior wall. In the other segments of the left ventricle, CT was also more suitable for the measurement of the wall thickness. Interventricular septal thickness was correlated between reconstructed short-axis view of CT (Y) and transverse view of CT (X), and the equations Y = 0.66X + 2.13 (r = 0.79) at the mitral valve level, and Y = 0.56X + 3.00 (r = 0.81) at the papillary muscle level were obtained. Therefore, reconstructed short-axis view should be used for the measurement of the left ventricular wall thickness not to overestimate the thickness.

Three cases of myocardial visualization on a routine perfusion lung scintigram with 99mTc-macroaggregated albumin were reported in patients with congenital heart diseases; two cases of tetralogy of Fallot and one case of truncus arteriosus type IV. Large right-to-left shunts greater than 39% and marked hypertrophy of the ventricle suggesting the presence of increased coronary blood flow were noted in all cases. In the two patients with tetralogy of Fallot myocardial activity appeared to be located in the hypertrophic right ventricles.

A new method for measurement of 24-hr whole-body retention (WBR) of 99mTc-MDP, using a thyroid uptake probe was established and its clinical significance was evaluated in various bone diseases. (1) Reproducibility of 24-hr WBR in 9 patients was very good. Correlation coefficient was 0.997 and coefficient of variability was only 1.83%. (2) Radiochemical purity of 99mTc-MDP was 97.8 +/- 0.7% (n = 5), indicating no significant inter-lot variations. (3) 24-hr WBR of normal adult males (n = 5) was 30.0 +/- 4.9%, which was significantly elevated compared to the reported 99mTc-HEDP WBR of 19.2 +/- 1.7%. Whole-body retentions of chronic renal failure, metastatic bone disease and hyperthyroidism groups were significantly elevated compared to that of the normal group. However, WBR of steroid-induced osteoporotic group was significantly decreased. Based on these results, this thyroid uptake probe method was simple, reproducible and accurate to measure 24-hr WBR of 99mTc-MDP. Quantification of WBR of 99mTc-MDP was of great clinical value to diagnose metabolic bone disease and to follow-up metabolic and metastatic bone diseases.

1) The experimental studies have demonstrated that 99mTc-gluconate reaches a high concentration in the kidneys within 1-2 hours after injection and its concentration in organs adjacent to the kidney is low. 2) The initial images of serial macroautoradiograms visualized the inner medulla as well as the cortex and outex medulla, whereas the delayed images revealed only the outer cortex and outer stripes in the outer medulla. These findings suggest that this renal agent consists of two components and the delayed component deposits in the convoluted and straight segments of proximal tuble. 3) This renal agent visualizes the calyceal system and excretory pathways in the initial study and the renal morphology in the delayed study. Therefore, using this renal agent, it may be possible to evaluate a variety of renal diseases, including obstructive uropathy, space-occupying lesions, congenital deformities, etc,.

Magnetic resonance (MR) angiographic studies of 14 patients with intracranial aneurysms (AN) were analyzed retrospectively as compared with contrast angiographic studies. Eighteen of 19 (95%) lesions were detected, and the relation between AN and their parent vessels was precisely assessed. The diameter of the smallest aneurysm detected in our case was 3 mm. However, small vessels around AN were not visualized. Therefore, MR angiography (MRA) is definitely not a substitute for preoperative contrast angiography. MRA is a useful screening tool for nonruptured AN, but further refinement is required to evaluate emergency ruptured AN.
