Human papillomavirus associated with adenocarcinoma and adenosquamous carcinoma of the cervix: analysis by in situ hybridization.

The incidence of adenocarcinoma of the cervix appears to be increasing. Recent reports have demonstrated an association between adenocarcinoma of the cervix and human papillomavirus (HPV) by Southern blot hybridizations. In situ deoxyribonucleic acid (DNA) hybridization was performed on paraffin-embedded specimens to localize the source of HPV DNA. In pure adenocarcinoma five of six specimens were positive for HPV DNA. Four specimens contained HPV type 18 and one HPV type 16. Only one of three adenosquamous lesions was positive and it contained both HPV types 16 and 31. These findings suggest an association between HPV and adenocarcinoma of the cervix.

Morphometric markers for the evaluation of preneoplastic lesions in the lung. Diagnostic evaluation by high-resolution image analysis of atypical cells in sputum specimens.

A review is presented of the Baylor College of Medicine/NASA/Lyndon Johnson Space Center high-resolution image analysis system for the detection of preneoplastic lesions of the lung in sputum specimens. For each specimen, 200 cells are graded as to their Atypia Status Index (ASI), a numerical classification based on a weighted composite of morphometric markers identified in the digitized images. The ASI values, which place individual cells within categories ranging from squamous metaplastic to carcinomatous, form the basis of the Cell Atypia Profile (CAP), which reflects the overall status of the patient's bronchial epithelium and can be used to diagnose and monitor epithelial atypias. Initial studies have shown the ASI and CAP to be accurate indices, whose application in the studies of sputum of individuals at high risk for the development of lung cancer (cigarette smokers over 45 years of age and industrially exposed workers) could lead to the early detection of preneoplastic lung lesions and to effective early clinical intervention, including the cessation of smoking or the application of beta-carotone or retinoids, which reportedly arrest the progression of bronchial epithelial atypias. Computer-assisted cell image analysis of morphometric markers in cells in sputum specimens appears to be uniquely applicable for surveillance of individuals at risk for carcinoma of the lung.

Hyperplastic polyps of the large bowel.

One hundred seventy-one hyperplastic colonic polyps were examined and found to be less homogenous histologically than has been generally considered. For example, 13% exhibited adenomatous changes, 7.6% contained Paneth cells, and 3% had distinct stalks. They are at times pedunculated and relatively large; some cannot be distinguished grossly from small sessile polyps of other histological patterns and removal is required for proper identification. Other epithelial polyps of equivalent size were compared with the hyperplastic polyps to determine the frequency of the various types. Hyperplastic polyps comprised 43.5% of all surgically removed epithelial polyps of the colon measuring 5 mm or less.

Relationship between multicentric reticulohistiocytosis and tuberculosis.

Some cases presently classified as multicentric reticulohistiocytosis may represent unusual manifestations of tuberculosis. Antituberculous chemotherapy may be justified in patients who have clinical and histopathologic evidence of multicentric reticulohistiocytosis in association with a positive tuberculin skin test.

The columnar-cell-lined (Barrett's) esophagus: a premalignant condition?

A 65-year-old man developed adenocarcinoma arising in Barrett's epithelium of the esophagus. He died three months after unsuccessful surgery to remove the tumor. The columnar-cell-lined esophagus may represent a premalignant condition, and it is suggested that patients with this condition undergo regular follow-up examination.

The significance of epithelial polyps of the large bowel.

We have reviewed 801 epithelial polyps removed from large intestines of 611 patients. Among these, there were 250 pure adenomatous, 304 adenomatous polyps with villi, 123 villous, and 124 hyperplastic polyps. Of the lesions classified as purely adenomatous, there were no observed carcinomas or carcinomas in situ. It would appear that a villous component may be an important indicator of the chance of the lesions harboring a carcinoma in situ or an invasive carcinoma. On the basis of our review, it would seem that it would be reasonable for a pathologist to indicate the estimated precentage of villous component in epithelial polyps of the large intestine.

Liposarcoma of the tongue.

Liposarcoma is a rare tumor in the head and neck. No previous report of its occurrence in the tongue is found at the time of this writing and this prompts its presentation. Significant in its management is wide surgical excision. Irradiation may be effective in some metastases. Prognosis is best in well differentiated forms.

Application of cell-image analysis to the diagnosis of cellular atypias in sputum: a review.

The incidence of carcinoma of the lung continues to steadily rise, and attempts at early diagnosis to improve prognosis have not yet been rewarding. The goal of our research is to decrease the incidence of lung cancer by detecting premalignant bronchial dysplasias in individuals in whom development of lung cancer is potentially preventable. To achieve this, we have developed an atypia status index (ASI)--the assignment of numerical values to the various stages of atypical bronchial epithelial cells in sputum, and a cell atypia profile (CAP)--an ASI-generated scale of 200 such atypical bronchial epithelial cells in a single sputum specimen. Computerized cell-image analysis techniques and statistical data analysis are used to generate the ASIs and CAPs for each subject. This study is a step toward the development of an automated cell-image analysis system for mass screening of premalignant atypias in sputum of those considered at high risk for lung cancer (i.e., men and women of 40 yr of age and older, with more than 20 pack-yr of cigarette smoking).
