Economic analysis of computed tomography units.

All operating CT installations in the United States were surveyed in January 1976; data were obtained from 98 of 140 installations. Although the respondents represented 80 head units and 18 head and body units, the overwhelming experience was with head CT studies. CT equipment was installed in an average of 1.3 months, operated 64 hr per week and examined 50-55 patients per week. A downtime of 7 hr per week was reported. Radiologists are responsible for 92 of 98 installations, and 90% of installations are in a hospital. The scheduling delay averages 1.6 days for inpatients and 11.5 days for outpatients. The delay is increasing in many installations. The estimated total yearly technical cost is +325,000-+371,000 per installation, depending upon patient volume. The estimated technical cost per patient (when 50 patients per week are studied) compares favorably with the estimated net revenue per patient from the average basic technical charge (+130 compared to +138). A separate billing method is used by 59% of installations, and 76% have an extra charge for contrast injection and additional studies; 60% of patients receive contrast. The reported total charges during the last 3 months were higher in installations that (1) charged additionally for contrast, (2) were located in outpatient settings, and (3) had nonradiologists as the responsible physician. It should be emphasized that most CT installations are not independent activities and should be considered an integral part of a diagnostic radiology department or office.

Thyroid imaging in lymphocytic throiditis.

The thyroid radionuclide images of 51 adult patients with a histological diagnosis of either Hashimoto's thyroiditis (HT) or chronic lymphocytic thyroiditis (CLT) were reviewed. The findings were variable but similar in the two groups, the most common being a multinodular pattern and the presence of hypofunctional nodules. Thyroid carcinoma was found in 2 of 41 patients with CLT and in 4 of 10 with HT. In a patient with clinical or laboratory evidence of HT and the radionuclide image finding of a cold nodule, thyroid carcinoma should be excluded by surgical biopsy.

Phagocytic capability of the kidney: a possible mechanism for renal uptake of colloid in liver-spleen scanning.

Guinea pigs were given Trypan blue or India ink i.v. and i.m. to demonstrate whether proximal convoluted tubule cells of the normal kidney have potential phagocytic capability. Circulating colloidal particles of Trypan blue solution accumulated in these cells and the accumulation was related to the particle size, dose injection, and duration of experiment. These studies suggest an explanation for the clinical observation of renal visualization with 99mTc-sulfur colloid in patients with congestive heart failure.

Migration and embolization of macrophages to the lung--a possible mechanism for colloid uptake in the lung during liver scanning.

Estrogenic stimulation of the RES in animals will result in the mobilization of large numbers of phagocytic cells from their natural sites of storage, chiefly the liver, spleen, and bone marrow, into the intravascular space. These cells are trapped in the pulmonary capillary bed and retain their ability to phagocytize colloidal particles. This is suggested to be the mechanism whereby increased lung uptake of colloidal radiopharmaceuticals is observed in some patients with liver diseases or other conditions stimulating the RES. It is further postulated that the known increase of estrogenic hormones in some patients with liver disease may be the in vivo stimulation resulting in increased mobilization of RES cells to the lung and subsequent isotope entrapment. It is possible that prognostic information can be obtained from this finding; however, its exact significance cannot be determined from this present study.
