Midazolam for induction of anaesthesia in patients with limited cardiac reserves. A comparison with etomidate.

The new water-soluble benzodiazepine, midazolam (Dormicum; Roche), was compared with etomidate as part of a balanced technique for induction of anaesthesia in 16 patients undergoing cardiac valve replacement. Monitoring revealed a significantly greater fall in blood pressure in patients receiving midazolam (reaching dangerous levels in 2 patients), which suggests that this agent is not suitable for induction of anaesthesia in haemodynamically compromised patients.

Evaluation of a new tracheal tube with a movable bronchus blocker.

A new tracheal tube which incorporates a movable bronchus blocker (the Univent tube), was evaluated in eight patients who underwent thoracic surgery. Difficulties encountered with accurate placement suggest that routine use should be made of a fibreoptic bronchoscope to confirm the position of the blocker.

Pneumonectomy in a patient with ventricular septal defect.

A young man with severe unilateral bronchiectasis and a ventricular septal defect presented for pneumonectomy. Intra-operative monitoring, which included continuous measurement of systemic and pulmonary oxygen saturations by oximetry, revealed transient reversal of the intracardiac shunt across the defect. The implications of this combination of cardiac and pulmonary disease for anaesthetic management are discussed.

Development of malignant hyperthermia obscured by cardiopulmonary bypass.

An episode of malignant hyperthermia occurring in a two-year-old child undergoing cardiac surgery is reported. The coincidental usage of hypothermic cardiopulmonary bypass obscured the classical presenting signs and symptoms of the syndrome. Once the clinical diagnosis was confirmed, rapid reversal was achieved with the administration of dantrolene sodium.

Cardiopulmonary Bypass

Amrinone after cardiopulmonary bypass surgery. A case report.

A 22-year-old woman underwent emergency aortic valve replacement. Administration of intravenous amrinone (Inocor; Sterling-Winthrop) enabled successful discontinuation of cardiopulmonary bypass after conventional measures had failed. The possible role of amrinone in this clinical situation is discussed.

Tracheal compression caused by aneurysms of the aortic arch. Implications for the anaesthetist.

Two cases of severe tracheal compression caused by aortic arch aneurysm are reported. There was a marked similarity in the clinical presentation of these patients but the different outcomes of surgical resection appeared to be related to the content of the aneurysms. The first patient had a fixed obstruction due to solid clot, which proved irreversible; while the second had an aneurysm containing fluid blood. The anaesthetic management of this complication is discussed.
