[Collagen colitis. A new disease picture or merely a histological abnormality?].

"Collagenous colitis" has been recently defined as a new entity characterized by protracted diarrhoea, associated with subepithelial broadening of the collagen layer. A report is presented on two patients fulfilling the above criteria. Based on a critical analysis of these two cases and on prospective study of biopsies in 50 consecutive coloscopies it is concluded that a causal relationship between the enlarged collagen layer and the diarrhoea cannot be unequivocally established and hence "collagenous colitis" should not yet be considered a new entity.

[Therapy of gastro-esophageal reflux disease].

Gastro-esophageal reflux is discussed with reference to pathogenetic factors and therapy. Over 90% of patients can be successfully managed by conservative treatment, though it requires much endurance on the part of both patient and physician.

[Adult duodenal diaphragm].

A case of intestinal obstruction in an adult due to congenital diaphragma of the duodenum (DD) is presented. Aetiology, pathology and diagnostic problems are discussed and the literature is reviewed. Various therapeutic procedures are evaluated. Excision or duodeno-duodenostomy are the only measures recommended. The correct choice depends on the relation of the DD to the ampulla of Vatter. Duodeno-duodenostomy was successfully applied in the reported case.

[Mechanical obstruction of the large intestine by wheat bran].

Since a 1971 a high daily fibre intake has progressively won a place as therapy and prophylaxis for various diseases. Thus far there have been no reports of severe adverse side effects from high residue diet. Four patients are presented in whom wheat bran in the usual dosage caused mechanical obstruction in the lower rectum.

[Occult blood in feces--a prospective study for the comparison of Hemoccult and Fecatest].

Stool samples of 150 unselected patients were tested for occult blood by the Haemoccult test and the Fecatest. The results show that the Haemoccult test is more suitable for screening for cancerous and precancerous colonic lesions than the Fecatest. Less than 10% of the patients had positive stool samples with the Haemoccult test, while bleedings lesions were subsequently found in about 2/3 of the cases. In contrast, the Fecatest was positive in 50% and in 70% after storage. These findings show clearly that the Fecatest cannot be recommended for the screening of colonic tumors.

[Effect of vasopressin on the lower esophageal sphincter. Study on the action mechanism of vasopressin in bleeding esophageal varices].

The effect of vasopressin on the lower esophageal sphincter has been investigated in 12 healthy volunteers. Peripheral infusion of 0.2 U/min resulted in a significant (p less than 0.05) decrease in sphincter pressure. Accordingly, vasopressin-induced myogenic compression of bleeding varices has been ruled out as a potential additional hemostatic mechanism of vasopressin.

[Tetracycline ulcer of the oesophagus: clinical features and in-vitro studies (author's transl)].

During treatment with doxycycline capsules three patients (aged 24, 34, and 39 years, respectively) developed acute oesophageal ulcers with dysphagia as the cardinal symptoms. They became symptom-free and the ulcers healed promptly on symptomatic treatment. In-vitro experiments in rats indicated that doxycycline accumulates within the erythrocyte membrane and epithelial cells of the oesophagus. This accumulation is followed by cell destruction. It is assumed that doxycycline capsules remain within the oesophageal lumen and release doxycycline, causing epithelial destruction. This will occur more often after dry swallows of the capsules before going to bed.

[Pneumatosis cytoides coli. Conservative treatment with oxygen breathing (author's transl)].

Investigations in a 51-year old female patient suffering from severe tenesmus and therapy resistant diarrhoea led to a diagnosis of pneumatosis cystoides coli. After 4 days of treatment with continuous oxygen breathing there was dramatic clinical improvement and an incomplete radiological remission. This new method is an important step forward in the treatment of pneumatosis coli. The only potential side effect is pulmonary oxygen toxicity. Thus limitation of oxygen treatment to the shortest time necessary for abolishing symptoms is recommended. Oxygen breathing is a safe, simple and effective treatment of pneumatosis coli and may replace the currently performed surgical resection of the involved intestinal segments.

[Buffering capacity and cost of fluid antacids in 1977].

The 12 liquid antacids most frequently used in Switzerland underwent in vitro determination of their buffering capacity, which was then related to their price. There was a twentyfold difference in buffering capacity between the most potent and the weakest antacid, and a 36-fold difference in price between the most expensive and the cheapest preparation. Based on these results and on the literature the guiding principles and effectiveness of antacid therapy in medical practice are discussed.

[Gastro-esophageal reflux disease. A review of pathogenesis, diagnosis and therapy].

Gastro-esophageal reflux disease is caused by peptic damage from gastric contents to the distal esophagus. The principal cause of reflux is a decrease in pressure of the lower esophageal sphincter, which normally separates the esophagus from the stomach. Various hormonal, pharmacological or toxic agents are capable of altering resting sphincter tone. Treatment of esophageal reflux disease is a rewarding task for the general practitioner. An algorithm on appropriate use of diagnostic and therapeutic resources is presented.

[The measurement of gastric emptying of solid food. Comparison of 2 solid phases by means of double isotope technic].

The suitability of a simple method of measuring the rate of gastric emptying of solids using 99mTc-marked filter paper pieces has been evaluated. In five healthy volunteers emptying of the filter paper pieces and of 198Au-tagged liver was measured simultaneously. In every proband the filter pieces left the stomach at a faster rate than the liver, but showed a linear emptying pattern in each experiment. Based on these observations and on recent results of other authors, a new concept of the gastric emptying of solids is presented. Within the framework of this model the use of the filter pieces in clinical studies to simulate a solid meal appears to be justified.

[Gastric emptying].

Gastric emptying can be measured by three basically different means: Intubation -x-ray - Isotopes, X-ray methods may be neglected as they do not permit quantitative measurement of the exptying process. Intubation methods offer the advantage of exact determinations of emptying rates, but disadvantages are the limitation to liquid meals and patient discomfort. Radioisotopes can be used as marker both of liquid and solid foodstuffs with minimal patient discomfort. However problems often neglected wtih isotopes are reliability of their attachement to the marked meal and radiation dose to the probands. The most important indications of measuring gastric emptying is determination of alterations induced by peptic ulcer surgery and drugs.
