[Polymyalgia rheumatica--clinical observations on 90 patients].

The clinical and laboratory data recorded during a several month study of 90 patients with polymyalgia rheumatica are presented. Especially with elderly women who lack other symptoms it should be borne in mind that muscular and articular pain in the scapular and pelvic regions together with an excessive increase in the blood sedimentation rate--more than 100 mm during the first hour are frequent--may indicate this disease. It is often accompanied by an arteritis of the temporal vessels (giant cell arteritis), which increases the danger of uni- or bilateral loss of sight. In our case material unilateral loss of sight occurred in 3% of the patients, bilateral loss of sight was not observed. A general disturbance in the patients well-being, occasional attacks of fever, non-specific signs of inflammation in the haematological findings and symptomatic anaemia mask the clinical picture and make diagnosis difficult. Initial high doses of cortisonoid and following long-term therapy make all signs and symptoms disappear. The disease can last for several years. The prognosis is favourable. The cause is still unknown.

[Research on the psychomotor performance of patients with vertebral syndromes at spas].

The influence of radon balneotherapy on the psychomotor performance of 169 male and female patients with vertebral syndromes was investigated. The speed and skill of motor response to various stimuli was measured by the Schoppe apparatus and the Viennese determination apparatus. The analysis of "steadiness" brought no results, whereas all the other parameters tested by the Schoppe and the Viennese devices showed highly significant improvements in the course of spa treatment. It was not possible to differentiate to which specific factors of spa treatment this positive influence can be ascribed.

Efficacy and safety of piroxicam and indomethacin in the treatment of musculoskeletal disorders. A double blind comparative study.

A total of 17 patients suffering from musculoskeletal disease were included in a double blind study to compare the efficacy and safety of piroxicam and indomethacin. Seven patients received a single dose of 30 mg piroxicam daily for two weeks, an eight patient stopped treatment after two days because of thrombocytopenia of unknown origin. Nine patients received 50 mg indomethacin tid for two weeks although treatment was interrupted for 2 days in one patient when a pulmonary embolus (unrelated to trial treatment) developed. Both treatment groups showed significant reductions in pain scores assessed by the patient from day 2 in the indomethacin group and day 6 in the piroxicam group. There were significant differences between the groups in favour of indomethacin at days 5, 8, 11, 12 and 13, although the groups were not well matched at baseline. Other efficacy measurements including the patient's and physician's overall assessments showed an improvement in some patients in each group, but there were no significant differences between the two groups. Side effects recorded as due to trial treatment occurred in one piroxicam and four indomethacin treated patients. Minor abnormalities in laboratory test results without clinical significance occurred in both groups. One piroxicam treated patient had a low platelet count prior to treatment and a subsequent reduction on day 3 requiring treatment to be stopped.

[DNA-synthesis and DNA-repair in chronic rheumatism influenced by small doses of natural radioactivity (radon)].

Elevated natural radioactivity during a treatment at Badgastein (Austria) caused an increase in DNA-repair of lymphocytes of patients with ankylosing spondylitis and osteoarthritis. Semiconservative DNA-synthesis was remarkably lower after the treatment in patients with ankylosing spondylitis and a small decrease was obtained also in lymphocytes of osteoarthritis patients. These effects are discussed in relation to late effects.

[Behavior of the complement components C3 and C4, C-reactive protein and blood sedimentation rate for activity control in arthritis patients during thermo-hydrotherapy].

21 male patients suffering since years from classical rheumatoid arthritis were treated with daily thermal baths of 37 degrees Celsius temperature during a 3 week period. To control the activity of the rheumatoid inflammation we used the complement components C3 and C4, the C-reactive protein and the blood sedimentation rate. No changes in the different activity parameters could be found during the course of the treatment. Joint function improved significantly. Therefore the combined treatment of rheumatoid arthritis with physio- and pharmacotherapy is recommended.

[DNA metabolism in lymphocytes of experimental subjects during thermotherapy (sauna, Turkish bath)].

The unprogrammed DNA synthesis (UDS) in the lymphocytes of the peripheral blood was significantly higher in regular sauna-users than in those who had not had a sauna for some time. Sedimentation velocity of the supercoiled DNA in the lymphocytes was decreased 1 h and 24 h after Turkish bath, but the difference from values before the bath was not statistically significant.

[Blood coagulation in hyperthermia].

Young healthy volunteers were treated with physical hyperthermia (baths) in order to investigate changes in blood coagulation. Such therapy is used in the treatment of rheumatic diseases. Single hot baths (mean body temperature 38,2-39,9 degrees C) resulted in a rise of fibrinogen, factors IX and XII, maximal amplitude of the thrombelastogram and hemoglobin and in a decrease of plasminogen. In a series of hypothermic baths no additional changes of coagulation or fibrinolysis could be found. The results suggest that hyperthermia causes a tendency to thrombosis.

Alterations of haemostasis after exposure to sauna.

Blood coagulation and fibrinolysis were examined in young healthy male volunteers before and after sauna baths. No consistent or periodical changes could be found. However on several days the increases of haemoglobin, platelet counts and factor VIII associated antigen and to a lesser degree of haematocrit, factor VIII, IX and XII reached statistical significance after sauna. The results suggest a tendency to an increased activity of the blood coagulation system.

[Prostaglandins E2 and F2 alpha concentrations in the synovial fluid in rheumatoid and traumatic knee joint diseases].

Prostaglandin E2 (PGE2) and F2 alpha (PGF2 alpha) concentrations in synovial fluid samples from 15 patients with various knee joint disorders were measured by radioimmunoassay. High concentrations of PGE2 wee found in all the samples obtained from patients with knee effusions due to various inflammatory processes as compared to the very low PGE2 levels in patients with osteoarthritis or traumatic effusions. The highest PGE2 levels were measured in knee joint effusions from patients with severe forms of rheumatoid arthritis. Two patients suffering from rheumatoid arthritis commenced with indomethacin (150 mg/day) after the first synovial fluid sample collection only. Some weeks later in a new sample a partial reduction of PGE2 concentration was found. The results of our investigations ar discussed with regard to possible correlations between prostaglandins and inflammation on the one hand and bone resorption associated with various pathological disorders on the other hand.
