[Gastric polyposis in surgical practice. Apropos 12 cases].

Personal experience is recorded in the surgical treatment of 12 patients with gastric polyposis at the Department of Propaedeutics of Surgical Diseases of the Research Institute of Surgery, for a period of 10 years (1981-1990). They accounted for 75 per cent of the benign neoplasms of the stomach and 4.08 per cent of all gastric tumors. The results of operative treatment, both in patients treated by gastric resection according to Billroth I technique (58.3 per cent) and in patients who were treated only by removing the polyps (41.7% per cent) were considered good within a follow up period of 1 to 5 years after the intervention. Proceeding from modern achievements and reports in the literature, the authors place accent on some important current aspects of treatment and observation on gastric polyps, their correlation with a number of hyperplastic diseases and malignant neoplasms of the stomach and colon.

[Duodenal carcinomas. Apropos 3 cases].

Three personal cases of duodenal cancer treated at the Department of Propaedeutics of Surgical Diseases of the Research Institute of Surgery in Sofia for the period 1981-May 1990 are reported. They accounted for 0.27 per cent of the patients with cancer of the gastrointestinal tract and 0.39 per cent of those with intestinal cancer. Two patients has adenocarcinoma and 1 leiomyosarcoma. Data available in the literature are adduced and analysis made of the symptoms, operative mortality and postoperative survival. The need of early diagnosis of duodenal cancer and the methods for this are pointed out. The major importance of the prophylaxis and competent approach to obligate and non-obligate precancerous duodenal diseases is emphasized.

Adenocarcinoma↗

[Semilunar ganglionectomy and the pain syndrome of pancreatic origin].

Personal seven-year experience (1984-1990) with the application of semilunar ganglionectomy in 25 patients with basic indication for pancreatic cancer (20 patients) and chronic pancreatitis (5 patients). Attention is called on the basic points in the original technique of Legé and the authors' own modifications of this technique. The indications and contraindications for performing the operative intervention were determined. The early postoperative results in 84 per cent of the cases (regardless of the type of diseases) were excellent as regards the pain; in the remote postoperative period the results were poor. The causes of the poor results of semilunar ganglionectomy are analyzed. The most common cause was the generalization of the malignant processes in the pancreas and the faulty performance of the intervention. The major importance of this operation in the treatment of chronic indurative pancreatitis with narrow pancreatic canals and its value when combined with other derivation methods in the operative treatment of pancreatic surgical diseases are pointed out.

Chronic Disease↗

[Cholelithiasis, cholecystectomy and colorectal cancer--a retrospective analysis].

The authors analyzed a series of 421 patients with colorectal cancer after preliminary review of the current literature and the modern theories of the origin of the disease. The patients have been treated at the Department of Propedeutics of Surgical Diseases, Research Institute of Surgery, for the period 1984-1990, and special accent was placed on the study of the relation of colorectal cancer to cholelithiasis and cholecystectomy. It was shown that 1.19 per cent of all patients had been cholecystectomized at mean 10.6 years back and 6.65 per cent had pre-existing or accompanying biliary calculosis with mean duration of symptoms 16.4 years. A major relationship was found between morbidity from colorectal cancer, on the one hand, and patient sex and biliary calculosis, on the other. Eighty nine per cent of the patients with colorectal cancer and with biliary calculosis treated by cholecystectomy were women and 20.3 per cent of the women with colorectal cancer had biliary calculosis and had been cholecystectomized. Attention is focused on the current practical requirements of the prophylaxis of colorectal cancer--adequate attitude to biliary calculosis, feeding habits, specifying cases at risk and their observation.

[Malignant non-Hodgkin's lymphomas in abdominal surgery: apropos 3 cases of Burkitt's lymphoma].

On the occasion of three observations of the extremely rare and malignant Burkitt's lymphoma, the authors record their 7-year experience in the operative treatment of 22 patients with non-Hodgkin's malignant lymphomas. Proceeding also from data available in the current literature, the most frequently encountered syndrome-complex is presented, the knowledge of which is of major importance for the early diagnosis, resp. for the results of combined treatment. The possibilities of the operative treatment are evaluated. In the authors' series it was radical in 31.8 per cent of the cases with 13.6 per cent early postoperative lethal outcome and 5.2 months average postoperative survival rate. Proceeding for the rarity of the disease in everyday surgical practice and that it is poorly known, but has become a nosologic entity of increasing interest, the authors have devoted the first part of their article to a discussion on the basic characteristics of non-Hodgkin's malignant lymphomas and the modern achievements of their treatment.

Abdominal Neoplasms↗

[3 cases of acute postoperative pancreatitis].

After a brief review of the literature the authors present three cases of acute postoperative pancreatitis in patient operated for different reasons with lethal outcome. In one patient the complication developed after duodeno-hemipancreatectomy and is the only patient with such complication (2 per cent) of 50 patients in whom radical operation was performed for cancer of the pancreas over the period Jan. 1983-May 1990. The current aspects of the etiology, treatment and above all of the prophylaxis of acute postoperative pancreatitis, which is a very serious postoperative complication with very high case fatality rate (up to 77 per cent, according to data in the literature) are discussed.

Acute Disease↗

[Transduodenal papillosphincteroplasty in the treatment of benign diseases of the terminal choledochus and papilla Vateri].

The modern state of the problem and the indications and contraindications for performing transduodenal papillosphincteroplasty are discussed. Personal experience is recorded with 67 transduodenal papillosphincteroplasties, performed for the period 1983-1989, against the background of 1091 operations on the hepatobiliary system, 171 of them on the extrahepatic bile ducts, performed during the same period. Proceeding from their own experience and from available data in the literature, the authors analyse and evaluate the surgical and nonsurgical methods of treatment of benign diseases of the terminal common bile duct and the duodenal papilla, manifested by stenotic lesions. The most important indications and contraindications for applying the different therapeutic methods are adduced. Remarks are made on the technique, which is of major practical importance and the respective conclusion is substantiated.

[The remnant cystic duct in the syndromal complex of the postcholecystectomy syndrome].

Some modern aspects of the postcholecystectomy syndrome and the importance of d. cysticus remnant for its arising are discussed. Analysis is made of 13 patients with postcholecystectomy syndrome surgically treated at the Department of Propedeutics of Surgical Diseases, Research Institute of Surgery, over the period 1983-1989. Results are reported of a study of 180 operated patients (cholecystectomized), in in 7.8 of whom low-grade postcholecystectomy syndrome developed. Accent is laid on the importance of the intraoperative diagnosis and on some characteristics of technical nature, requiring precision in processing d. cysticus. Conclusion is drawn, aimed at reducing to a minimum the percentage of postcholecystectomy syndrome.

[Our experience with nonanatomical liver resections].

The current status of the problem of nonanatomical liver resections is presented. Some characteristics of the technique applied in them are recorded and the indications for application of the operative technique are specified. Personal five-year experience (1983-1988) with 21 nonanatomical liver resections, applied in gall bladder cancer (4 patients), metastatic tumors (8 patients), high-grade biliary obstruction (7 patients), hemangioma (1 patient) and multiple echinococcosis (1 patient). The early postoperative results were very good, with early postoperative death of one patient. On the basis of these results, the authors advise extension of the indications for application of nonanatomical liver resections.

[Changes in the thromboxane B2 level of the blood plasma following partial resection of the pancreas].

The present study was devoted to the still poorly exploited aspect of pancreas surgery, from a standpoint of postoperative divergences in the plasma prostaglandin level. Experiments were carried out with dogs. The authors apply proximal (right) resection of pancreas and eliminate the exocrine function of the remaining part by occlusion of the pancreatic canal. Radioimmunologic investigations were carried out by extraction method for determining the plasma thromboxane B2 level. Postoperative rise in TxB2 level was found. The authors apply modern drugs, proceeding from recent report in the literature about correction of this abnormality aimed at optimal re-establishment of the functions of the remaining pancreas after the operative intervention.

[The effect of pancreatic resection with occlusion of the pancreatic remnant on the morphology of the gastrointestinal system].

Modern scientific problems are discussed on some still moot aspects of resection surgery of the pancreas from viewpoint of the postoperative divergencies in all other organs of the gastrointestinal system. Experiments were carried out on dogs. Proximal (right) resection of the pancreas was applied and the endocrine function of the remaining part of the organ was eliminated by occlusion of the pancreatic canal. Histomorphologic studies were carried out on different organs of the gastrointestinal system, 4, 5 and 11 months after the pancreas resection. Serious abnormalities and interrelations were found between the glandular apparatus of the gastrointestinal organs and the pancreas resection. Histomorphological proof of the changes are adduced.
