[Acute inflammatory diseases of the extremities].

A total of 208 patients with diseases of the extremities, 56 of whom with diabetes mellitus, were included in the study. Septicemia developed in 6 patients, in 4 of them being caused by anaerobic pathogens. The following surgical interventions were performed: 193 incisions, 32 amputations of varying extent and 15 large-scale necrotomies. The case fatality rate was 1.44 per cent. To achieve superior therapeutic results and a better life for patients with diabetes mellitus, it is necessary to change the former approach to their long-term medical surveillance.

[Insulin receptor function in subjects with above-normal body weight].

The insulin receptor binding ability was studied in 26 persons with above normal body mass (15 women and 11 men), mean age 44.15 +/- 10.1 years without family history of diabetes mellitus. According to the degree of obesity they were classified into 3 groups. In the persons with I-II degree of obesity parallel with the strongly reduced number of insulin receptors (total and the high affinity) an increase of the receptors affinity appears as a compensatory mechanism which ensures appropriate insulin receptor binding. In the persons with III-IV degree of obesity the number of insulin receptors is strongly reduced but the receptor affinity does not differ from that of the controls with normal body mass. The receptor changes in the persons with excessive obesity are similar to those found by the authors in patients with newly discovered non-insulin dependent diabetes mellitus. This allows the suggestion that these persons are in a potential risk of developing diabetes mellitus.

[Benign and malignant hemopathies in elderly patients].

108 patient over 75 years of age, treated in an internal diseases ward during a five-year period, were studied. 62 of them were with benign blood diseases, the women prevailing. The malignant hemopathies were found more frequently in men than in women, the ratio being 2.83:1. 35 of the patients (76%) were with non-Hodgkin's lymphoma and 10.8% of the patients were with Hodgkin's lymphoma. Only single patients suffered from chronic myeloid leukemia, myelofibrosis and blastic leukemia. The survival of the patients with benign hemopathies was shortened considerably because of the polymorbidity in elderly people. The survival of the elderly patients with malignant hemopathies did not differ from that of the patients from the other age groups.

[New toxicokinetic exposure tests for metals based on atomic absorption analysis of the nails].

The up-to-date methodology development for analysis of microelements permits the determination of many of them in small nail samples. The nails of the feet are chosen as biological bioptic material offering some advantages: stability in preserving; higher levels of most toxic elements (in comparison with the levels in serum, blood and urine); a lower rate of surface pollution (in comparison with the hair and nails of the hands); receiving information on the average level of exposure for a long period of time. For the purpose of using nails from the feet in biological control are developed direct AAC - procedures for defining As, A1, Cd, Co, Cr, Cu, Fe, Mn, Ni, Pb, Se, Zn, Mo, Sb and Sn on the basis setting the samples in solution with tetra-alkylammonium hydroxide. The levels of these elements are determined in unexposed persons. The analysis of nails from feet of workers exposed to Pb, Cr and Mn proves, statistically, that the level of these metals gives valuable information on the rate of exposure (the quantity entered into the organism) of groups and individuals. This analysis permits longitudinal control on Pb, Cr and Mn - status of the organism by painless sampling, accessible and reliable analytical procedure.

[Epidemiologic studies of the prevalence of arterial hypertension among industrial workers in Stara Zagora].

An epidemiologic study was carried out in June 1986 among 1702 industrial workers from the machine-building and metal-manufacturing industry--956 men and 746 women. 57.9% of the workers were in the 30-39 and 40-49 age groups. Arterial hypertension was found in 16.6% of the examined and borderline hypertension was found in 12.9%. With advancing age the hypertension frequency increases. Men suffer more frequently (18.4%) than women (14.2%). In younger age this difference is statistically significant. A relation was established between hypertension and obesity in both sexes. The characteristics of the hypertension structure and the relation of borderline hypertension with age, sex and body mass are discussed.

[Clinical efficacy of a Minidiab preparation (glipizide) in an acute experiment].

The hypoglycemic action of Minidiab (Glipizide) and maninil (Glibenclamide) were studied in an acute experiment. 10 patients with type 2 diabetes mellitus (6 men and 4 women, mean age 49.1 years, were examined. The blood sugar level and the insulinemia were followed up after a 10 mg morning dose of each drug. The hypoglycemic action of both drugs is identical but is reached in different ways. Minidiab has a faster, shorter but stronger insulinotropic action. Maninil exerts a better peripheral effect.

Cistiside-M, a new protective agent.

The toxicological and radiopharmacological properties of the agent Cistiside-M (2-mercaptoethylamine adenosine-5-phosphate) were investigated on male mice, lines C57Bl and H. The compound was administered intraperitoneally in doses from 1000 to 125 mg/kg, 15 min prior to lethal gamma-irradiation using a 137Cs source in the 6 to 15 Gy range. The radioprotective effect was assessed by the integral indices of 30-day survival, mean survival time, coefficient of protection alpha, S30 and the dose-reduction factor (DRF). The compound was characterized by low toxicity and good antiradiation effect in a wide dose range. Maximum degree of radioprotection (75% survival) was established when the agent was administered in a dose of 1000 mg/kg. DRF was 1.6. The dose of 250 mg/kg was interesting--with 50% survival rate and increase of the therapeutic index to 5.6. The low toxicity and the considerable radioprotective effect of the compound are due to its complex structure.

[Isolation and receptor activity of mammalian lymphocytes].

The possibilities of lymphocytes from the blood of pigs, cows, sheep and rabbits, using Lymphoprep, Polysep, and Histopaque were investigated. The conditions given by the producer firms were strictly observed when dealing with the separation media, and in cases when the results were unsatisfactory modifications were made with regard to the velocity and duration of centrifugation in carrying out the separation. The functional state of membrane receptors of lymphocytes was checked by rosette tests with erythrocytes for the detection of T-lymphocytes. It was demonstrated that lymphocytes from pig's blood are successfully isolated with all three separating media, when observing the conditions of manipulation as indicated by producer firms. With ruminants the best results were obtained with Lymphoprep. Polysep is not suitable medium for lymphocyte isolation from cow's blood; Histopaque can be used if the separation is carried out at 1880 r. p. m. for 40 minutes. Good results are obtained with Polysep with sheep's blood when the separation is performed at 1880 r. p. m. for 40 minutes. All three media can be used in isolation of lymphocytes from rabbit's blood. The isolation of lymphocytes with Histopaque must be performed by centrifugation at 1520 r. p. m. for the duration of 40 minutes. The separation media investigated do not harm the membrane receptors of lymphocytes.

[Malignant hemopathies combined with latent tuberculosis (a report of 2 cases)].

Two cases of a combination of a malignant blood disease with latent tuberculosis are presented. One of the patients was with acute leukemia and the other one was with non-Hodgkin's malignant lymphoma (Lymphosarcoma). The blood disease dominated the clinical picture and determined the severity of the course and the lethal outcome. The post mortem examination revealed hepatosplenic form of miliary tuberculosis in both patients. In one of the patients caseous tuberculosis of the bronchopulmonary, peribronchial and periportal lymph nodes was found, too. The tuberculous process had a latent course without characteristic manifestations but it also led to worsening of the patients' condition.

[Ultrastructural changes in the uterine epithelium and in the immunocompetent cells of the lymph nodes draining the uterus during pregnancy in swine].

The studies were carried out with both nonpregnant sows aged 1-2 years and pregnant sows from the 30th to the 40th day and on the 60th, 90th, and 100th day of gestation. Three types of cells were found in the epithelium covering the uterus of nonpregnant sows--with kinocilia and microvilli at the apical pole, with microvilli only, and with no microvilli and kinocilia. During pregnancy the ultrastructure of the epithelium were subjected to essential changes which consisted chiefly in increasing the compactness of microvilli, the amount of lysosomes at the apical part, and the basal deposition of fatty globules in some of the cells. The basal membrane on which the epithelial cells lay became considerably thicker. The draining lymph nodes showed a clear trend toward increasing the number of lymphocytes and cells of the plasmatic order. The ultrastructure of the plasmatic cells and lymphocytes spoke of enhanced processes of synthesis.

Dynamics of T-suppressor and T-helper lymphocytes and haemolytic plaque-forming cells during normal pregnancy in the sow.

T gamma and T mu lymphocytes and haemolytic plaque-forming cells (PFC) were quantitatively determined in peripheral blood and uterine draining lymph nodes ( DLN ) from 36 nonpregnant and 45 pregnant sows. A considerable increase in the number of T gamma cells occurred both in the blood and the DLN during pregnancy (P less than 0.001). This increase was higher at the beginning of pregnancy (20th-40th day) and lower at the end (80th- 110th day). No significant quantitative changes were found in T mu cells. During pregnancy there was a marked drop in the number of cells in the peripheral blood manifesting direct erythrocytolytic activity (DECA), as well as of haemolysin-producing cells.

[Characteristics of immunoglobulin dynamics and absorption in newborn calves].

A study on protein dynamics in calf blood serum from the time of birth to age of 60 days was carried out by use of electrophoresis on 1% agar gel, immunodiffusion and immunoelectrophoresis with rabbit immune serum against cattle blood serum, colostrum whey and pure nu-globulin. Investigations were performed on the 4th and 24th hour, and on the 3rd, 6th, 8th, 10th, 15th, 20th, 30th, 45th and 60th day after birth. Resorption of colostral gamma globulins, applied intravenously and orally in the form of the preparation lactoplasmin and 10% normal gamma-globulin injected intravenously was followed under experimental conditions. It was established that calves are born with a comparatively low protein level in the blood serum and after feeding with colostrum begins, specific quantitative and qualitative changes in their profile take place, but the changes concerning gamma-globulins are most pronounced. gamma-globulins are best resorbed after oral application of colostral globulin preparations, and not so well after intravenous application. Noral blood gamma-globulin injected intravenously is least resorbed. Eventual causes for this fact are discussed.
