A pilot study of tele-ophthalmology outreach services to primary care.

We have begun a randomized study of tele-ophthalmology. General practitioners in two participating health centres in the Negev region of Israel, serving a local population of 40,000, were invited to participate. The study group consisted of 50 consecutive patients attending the general eye outpatient clinic and 50 consecutive patients suffering from various retinal diseases attending a hospital outpatient clinic. The initial assessment was carried out via the tele-ophthalmology system. There was then a face-to-face re-examination of the same patients, in randomized order, by the same ophthalmologist on the same day, at an outpatient ophthalmology clinic.

Quantified EEG in patients with vertigo of central or peripheral origin.

Computerized EEG frequency analysis was performed in 18 patients with peripheral vestibular dysfunction (PVD), 17 patients with central vestibular dysfunction (CVD) and 22 aged matched controls. Increase in the relative theta activity in the centrotemporal region was found in patients with PVD. The decrease of centroparietal beta activity in both groups of patients, as compared to controls, did not reach statistical significance. We conclude that quantified EEG may be a sensitive method to show changes of cortical electrical activity in patients with vertigo, reflecting the imbalance in afferent vestibular input to the cortex.

Progressive deterioration of brainstem auditory evoked potentials in Creutzfeldt-Jakob disease: clinical and electroencephalographic correlation.

Brainstem auditory evoked potentials (BAEP) were recorded and correlated with the clinical and electroencephalographic (EEG) findings in three verified cases of Creutzfeldt-Jakob disease (CJD). In contrast to previous reports, there was progressive deterioration of the BAEPs, through initial asymmetry of wave latencies to abnormal wave morphology, and increased interpeak latencies. Consequently, in two of the patients, disorganization of the wave pattern and generalized conduction-time slowing appeared by the time there was a fully developed clinical picture of CJD with periodic EEG waves. In the third, clinically mild CJD, asymmetry of wave latencies in the BAEP emerged, correlating with the appearance of periodicity on EEG. Our findings are consistent with the late, progressive involvement of the brainstem by the pathological process in CJD.

Optimal input design: case study of the metabolic process.

An optimal input of nutrients into the metabolic process of the individual subject so as to effect desired therapeutic results is particularly important for the critically ill. A patient-related, individualized nutrients optimization procedure is proposed here. The procedure is applicable to any time-invariant and deterministic metabolic model that is bound by one or more quantitative limiting criteria. As a case in hand, the procedure is used to optimize the individual metabolic needs of critically ill patients. The results indicate that, given a proper metabolic model, the patient may be treated on an individual appropriateness basis rather than on the traditional statistical intuitive approach.
