Use of partial cardiopulmonary bypass during the anhepatic phase of orthotopic liver grafting.

Femoro-femoral partial cardiopulmonary bypass has been used in two patients under going orthotopic liver allografting who had severe cardiovascular disturbances on trial clamping of their venae cavae. Flows of 2-3 litres/min resulted in satisfactory cardiovascular control during the anhepatic phase in each ase. A third patient who tolerated caval clamping withstood the grafting without bypass. Having bypass available for use in critical cases could increase the safety of orthotopic liver transplantation.

Polypeptides expressed in Escherichia coli K-12 minicells by transposition elements Tn1 and Tn3.

Escherichia coli K-12 minicells were employed to examine polypeptides encoded by plasmids carrying wild-type and mutant Tn1 or Tn3 transposition elements. Tn1- and Tn3-containing minicells express high levels of four transposon-specified polypeptides. Three, of molecular weights 30,000, 28,000, and 25,000, are related immunologically to beta-lactamase, the enzyme responsible for ampicillin hydrolysis. A fourth polypeptide of molecular weight 19,000 is encoded by the Tn1 or Tn3 region which spans the BamHI cleavage site. Mutant transposons which no longer produce this polypeptide transpose at higher than wild-type frequencies to give aberrant transposition products (Gill et al., J. Bacteriol. 136: 742--756, 1978; Heffron et al., Proc. Natl. Acad. Sci U.S.A. 72:3632--3627, 1975). No expression could be detected from a region of the transposons extending from the inverted repeat sequence distal to the beta-lactamase gene to more than half the distance into the Tn1 or Tn3 sequence.

Analysis of sequences transposed by complementation of two classes of transposition-deficient mutants of Tn3.

The Tn1 and Tn3 elements are closely related transposons which carry the structural gene for ampicillin resistance. Two classes of deletion mutants of the plasmid pMB8::Tn3 (RSF1050) are unable to transpose ampicillin resistance but can be complemented in trans by a coresident Tn1 or Tn3 element. The analysis of the sequences transposed upon complementation of one class of mutants (type I) showed that the mutant element had undergone bona fide transposition. Complementation of the type II mutants led to the transposition of a sequence analogous to bacteriophage mu-promoted integration of non-mu DNA. The transposed sequence consisted of two Tn3 elements which flanked a single copy of the pMB8 portion of the RSF1050 genome. Complementation data indicated that the type II mutants are defective in at least one trans-acting function which must be supplied for transposition to occur. The nature of sequence transposed from the type II mutant is the consequence of a defective cis-acting function (or site). In addition, the type II mutants were defective in a trans-acting function which regulated the frequency of transposition.

Alteration of the fatty acid composition of membrane phospholipids in mouse lymphoid cells.

A simple method is described for introducing exogenous fatty acids into the membrane phospholipids of the murine leukemia cell EL-4, and into the membrane phospholipids of resting mouse lymphocytes. The method involves culturing of the cells with free or methylated fatty acids at concentrations up to 50 microgram/ml. The presence of serum in the culture medium does not interfere with fatty acid uptake, but does increase the growth rate and viability of the cells. Membrane lipid composition returns to normal after the cells are grown in medium without exogenous fatty acid. Fractionation of the cell membranes confirmed that exogenous fatty acids were incorporated into the phospholipids of the plasma membrane.

The generation of a ColE1-Apr cloning vehicle which allows detection of inserted DNA.

A 3.2 Mdal sequence of DNA, TnA, which contains the ampicillin (Ap) resistance determinant has been translocated from an R plasmid to the plasmid ColE1. A total of 12 isolates were studied. There are at least 8 sites in ColE1 at which TnA has inserted. Insertion at five of these has resulted in a Col-phenotype. One ColE1-Apr plasmid, RSF2124, was examined further and its replication properties are found to be similar to that of the parent plasmid. RSF2124 appears to be a useful plasmid vehicle for the molecular cloning of DNA from diverse prokaryotic sources: it codes for readily detectable Ap resistance and contains a single EcoRI site in a gene affecting colicin biosynthesis so that it is unable to produce colicin upon ligation to other DNA.

Computer simulation model of an x-ray department.

A computer simulation model has been designed to predict the effects of changes in the work load and resources of an x-ray department. The model has been used to produce histograms of patient waiting times and to show the effect on these of introducing changes in the speed of processing films and in the numbers of cubicles and radiographers available. The predicted benefit of using a faster film processor has been confirmed in practice.

Sample handling and storage for the quantitative analysis of volatile compounds in blood: the determination of toluene by headspace gas chromatography.

The problems associated with the handling and storage of blood samples for the analysis of toluene in toxicological casework using headspace gas chromatography have been investigated. Significant losses of toluene and the internal standard, ethylbenzene, from solution may be observed during handling. Toluene may be lost into the headspace above the blood in a storage container while such headspaces can also be detrimental, leading to the formation of n-hexanal, a possible interference with the assay. Large differences in the stability of toluene were seen for blood samples in different types of storage container. Storage containers in which the sample was only in contact with glass, teflon, or aluminum foil were most satisfactory. It is recommended that blood samples should be stored in appropriate containers with minimum headspace under cold conditions (4 degrees C). Transfers of blood should also be made at cold temperatures. The applicability of these guidelines to the analysis of other volatile compounds is discussed.

Multiplane transesophageal echocardiography: our initial experience.

We present our experience in the diagnostic assessment of a wide spectrum of cardiovascular disorders using multiplane transesophageal echocardiography (MP-TEE). Two hundred and seventeen patients in the age range of 11-71 years were subjected to MP-TEE from January to November 1993. The male:female ratio was 1.1:1. One hundred and ten patients had predominantly mitral valve disease of rheumatic origin, eleven had mitral valve prolapse, twenty patients had aortic valve disease and thirty seven patients had more than one valve involvement. Six patients with suspected prosthetic heart valve dysfunction and ten patients of hypertrophic cardiomyopathy were also studied. Two patients had unexplained pulmonary hypertension, three had pericardial disease and three had proximal aortic dissections. Twenty six patients with congenital heart disease were studied of which nineteen had atrial septal defects, one had corrected transposition of great vessels with pulmonic stenosis and one adult had Ebstein's anomaly of the tricuspid valve. In our experience, MP-TEE enhances the versatility of TEE by providing incremental diagnostic information and enhancing delineation of pathology. The procedure was well-tolerated and no complications occurred.
