Ki-67 MIB1 labelling index and the prognosis of primary TaT1 urothelial cell carcinoma of the bladder.

AIMS: To evaluate whether ki-67 labelling index (LI) has independent prognostic value for survival of patients with bladder urothelial tumours graded according to the 2004 World Health Organisation classification. METHODS: Ki-67 LI was evaluated in 164 cases using the grid counting method. Non-invasive (stage Ta) tumours were: papilloma (n = 5), papillary urothelial neoplasia of low malignant potential (PUNLMP; n = 26), and low (LG; n = 34) or high grade (HG; n = 15) papillary urothelial carcinoma. Early invasive (stage T1) tumours were: LG (n = 58) and HG (n = 26) carcinoma. Statistical analysis included Fisher and chi2 tests, and mean comparisons by ANOVA and t test. Univariate and multivariate survival analyses were performed according to the Kaplan-Meier method with log rank test and Cox's proportional hazard method. RESULTS: Mean ki-67 LI increased from papilloma to PUNLMP, LG, and HG in stage Ta (p<0.0001) and from LG to HG in stage T1 (p = 0.013) tumours. High tumour proliferation (>13%) was related to greater tumour size (p = 0.036), recurrence (p = 0.036), progression (p = 0.035), survival (p = 0.054), and high p53 accumulation (p = 0.015). Ki-67 LI and tumour size were independent predictors of disease free survival (DFS), but only ki-67 LI was related to progression free survival (PFS). Cancer specific overall survival (OS) was related to ki-67 LI, tumour size, and p27kip1 downregulation. Ki-67 LI was the main independent predictor of DFS (p = 0.0005), PFS (p = 0.0162), and cancer specific OS (p = 00195). CONCLUSION: Tumour proliferation measured by Ki-67 LI is related to tumour recurrence, stage progression, and is an independent predictor of DFS, PFS, and cancer specific OS in TaT1 bladder urothelial cell carcinoma.

Prognostic factors in follicular carcinoma of the thyroid.

Although there are numerous publications on the nomenclature, morphological diagnostic criteria, and prognostic factors of follicular carcinoma of the thyroid, these issues remain controversial. We present the findings of a retrospective and comparative study of 82 patients who underwent thyroid surgery for thyroid neoplasms. Of these patients, 58 had follicular carcinoma, 12 had atypical adenoma, and the remaining 12 patients had adenoma with partial capsular invasion in the surgical specimen. The goal of the study was to determine diagnostic criteria and prognostic factors in follicular carcinoma. Our results showed that encapsulated thyroid follicular neoplasms should be considered malignant when they demonstrate total capsular invasion and/or vascular invasion. Statistical analysis of the cases of follicular carcinoma showed that higher morbidity rates were seen in the patients with metastasis at the time of diagnosis, with extensively invasive tumors, with age older than 35 years, and with tumors with simultaneous vascular and capsular invasion. The accumulated survival rate over 5 years based on mortality was 91%, and based on morbidity it was 71% (84% for patients with encapsulated tumors and 56% for patients with extensively invasive tumors). In the univariate survival study based on morbidity, we found a relationship with the following parameters: metastasis at the time of diagnosis, grade of local invasion, presence of capsular and vascular invasion, cellular atypia, histologic pattern (insular vs others), presence of thyroid nodule, and age of the patient at the time of diagnosis. In the multivariate survival study based on morbidity, the factors that showed prognostic value were metastasis at the time of diagnosis, grade of local invasion, and age of the patient at the time of diagnosis.

Mitotic activity of the endocrine cells in rat thyroid glands during postnatal life.

This paper presents the results of investigations into the mitotic rates of thyroid endocrine cells in normal postnatal rats, aged 1-120 days. Our study revealed considerable age-dependent shifts in the mean mitotic activity of follicular cells and C-cells. The maximum indices of endocrine cell renewal were reached during the first 10 days of life, decreasing gradually and significantly until 25 days. At 1 month, there was a significant recuperation of the division rate for both cell types, which declined in the adult rat. These results mean that the proliferation of C-cells and follicular cells is inversely proportional to age. The preferential zone of localization of mitoses of both cell types is the central region of the thyroid lobe. The present paper provides new evidence for the postnatal origin of C-cells and follicular cells from the preexisting endocrine cells.

Histometry of normal thyroid glands in neonatal and adult rats.

The present histometric study is on thyroid glands of Wistar rats ranging in age from 0 to 120 days. The mean volume of one lobe of the thyroid in 4-month-old animals was some 22-, 10-, 5-, and 3-fold greater, respectively, than the volumes in the newborn, 5-, 10-, and 30-day-old rats. At 4 months of age the mean length of the lobe was 3 times greater than at birth. The volumetric fractions (Vv) of the different histological components (follicular cells, C-cells, colloid, and interstitial tissue) changed considerably in the course of development. The Vv of follicular cells diminished from 61.4% at birth to 37.2% at 4 months. C-cells increased from 2.9% in the newborn to 4% at 15 days, with no further significant change at 4 months. Colloid and stroma together represented 35.7% at birth, increasing to 58.5% at 120 days. In the course of the first 4 months of life, the absolute volumes occupied by follicular cells, C-cells, colloid, and stroma increased 13.25, 30.75, 38.6, and 33.7 times, respectively; these changes reflect the variations that occur in the volume of the gland and the Vv throughout postnatal development.

The effect of HeNe laser radiation on the thyroid gland of the rat.

Although laser irradiation is becoming common practice in medicine, there is not always a clear understanding of the possible side-effects. The present report is a light and electron microscopic study of the effects of fixed low intensity doses of soft HeNe laser on the thyroid of Wistar rats. The immediate effects are mild multifocal degenerative changes; these lesions recover in less than 3 months. Long-term lesions are identified only by electron microscopy; they consist of an increased number of peroxisomes and free or intramitochondrial crystalline structures. We discuss the laser's hypothetical functions.

Mucosubstances in medullary carcinoma of the thyroid.

We have studied mucosubstances in 12 cases of medullary carcinoma of the thyroid using histochemical techniques and a number of different lectins. Immunohistochemical staining for calcitonin and carcinoembryonic antigen was also performed. We have found that the presence of mucosubstances is a constant finding in medullary carcinomas; they were present extracellularly in 100% of cases and intracellularly in 53.3%. In both these compartments there was a predominance of neutral over acid mucosubstances. Receptors for UEA-I, Con A, RCA-I, Succ-WGA and SBA were found in many cells in the majority of cases, but there were differences in the affinity from one case to another. Because of this lack of specificity lectin histochemistry is not of diagnostic value in medullary carcinomas of the thyroid. The possible relationships of the lectin-binding results to glycosylated hormone precursors and carcinoembryonic antigen are discussed.

Differential diagnosis between follicular adenoma and follicular carcinoma of the thyroid by marker features.

Morphometric features descriptive of nuclear morphology, nuclear chromatin texture and nuclear staining density were studied in tissues from encapsulated follicular carcinoma and follicular adenoma of the thyroid, whose differentiation is one of the most difficult tasks in thyroid pathology. Measurements were made on nuclei in both tumorous and normal-appearing areas in the same section from six patients with each diagnosis, as well as on nuclei in sections from six normal individuals. The average nuclear optical density (OD) was significantly lower in the follicular carcinomas than in the follicular adenomas while the nuclear area was significantly increased in the carcinomas. The variance of the nuclear OD values was higher in both tumor and normal-appearing areas in the sections from the follicular adenomas than in the follicular carcinomas; this variance was lowest in tumor areas from the follicular carcinomas. There were also significant differences in another chromatin feature (the average difference in OD values between adjacent pixels) between nuclei from follicular carcinomas and those from follicular adenomas, whether comparing measurements between the tumor areas or between the histologically normal-appearing areas of the sections. These results suggest that the high-resolution study of marker features, such as nuclear chromatin and staining density, may help in the differentiation between follicular adenoma and encapsulated follicular carcinoma of the thyroid.

Multiple endocrine neoplasia type 2b in twins.

The syndrome of multiple endocrine neoplasia (MEN or MEA) type 2b is characterized by the association of medullary carcinoma of the thyroid, phaeochromocytoma, ganglioneuromatosis and Marfan-like features. Though this disorder usually shows a familial distribution, it may also appear spontaneously as the result of a genetic mutation. This paper describes the second case of MEA type 2 in twins, and appears to be the first description in twins of the association of medullary carcinoma of the thyroid, Marfan-like features and multiple neuromas.

Extragenital endometrioid cystadenofibroma.

We present a case of extragenital endometrioid cystadenofibroma, whose rare pelvic localization is the motive for its publication, as no similar case had been described previously. Its possible histogenesis from a pelvic mesothelium, as well as the differential diagnosis from pelvic endometriosis is discussed.

Adrenal myelolipoma simulating a retroperitoneal malignant neoplasm.

We describe a case of a large myelolipoma (15 by 12 cm. maximum diameter) that was removed surgically and resembled clinically and semeiologically a malignant retroperitoneal tumor. The patient also had chronic pancreatitis and cholelithiasis. Experimental findings, as well as the frequent association of myelolipomas (usually small) with several chronic diseases, suggests that the adrenal myelolipoma is not a true neoplasm but, rather, a choristoma or, perhaps even more likely, a hyperplastic and/or metaplastic lesion.

'Benign' fibrous histiocytoma of the trachea.

Fibro-histiocytic neoplasms are uncommon in the respiratory tract. This paper presents a clinical and histological description of a case of fibrous histiocytoma at the level of the third tracheal ring. This cases shares some features with the four cases previously described, viz., the growth occurs mainly in young adults; recurrences are common; and all have shown similar benign histological patterns. Ultrastructural investigations confirmed the fibro-histiocytic nature of the neoplasm.
