Hypofrontality in schizophrenia: influence of normalization methods.

1. Studies with Single Photon Emission Computed Tomography (SPECT) in schizophrenia research have utilized different approaches to normalization of data, such as cerebellar ratio and whole brain ratio methods, leading to conflicting findings. 2. The authors compared these two methods to test the hypofrontality hypothesis of schizophrenia. 3. Eighteen chronic and medicated DSM-IV schizophrenic patients and 10 healthy controls underwent two SPECT examinations using 99mTc HMPAO as a tracer at baseline and during frontal activation while applying the Wisconsin Card Sorting Test. 4. The hypofrontality hypothesis was supported with both indexes of relative perfusion, although the whole brain ratio method appeared to be more reliable and specific than the cerebellar ratio method. 5. Further studies are required to confirm these preliminary results on the specificity and sensitivity of both methods.

[Relationship between psychosocial stress and mental disease: community study].

INTRODUCTION: We investigate the relationship between life events stress and psychiatric disorders in a random sample of urban population older than 16 in the Tenerife Island. METHOD: A two-stage cross-sectional study was conducted. In the first stage 660 persons were interviewed using the GHQ-28 as a screening questionnaire and the DSM-III-Axis IV for the psychosocial stressors valoration. In the second stage were interviewed all persons screening positive on the GHQ and similar number of the screening negative ones, using the Clinical Interview Schedule. RESULTS: The psychosocial stressors prevalence was 36,5% and the pathology psychiatric prevalence was 12, 90%: among 3.3% of the people who do not refer psychiosocial stress and 29.5% of the people who do refer. CONCLUSION: We confirmed a significant relationship between psychosocial stress and psychiatric disorders in both sexes.

[Subtypes of schizophrenics patients with attempted suicide].

INTRODUCTION: During the last decades, numerous research works have tried to determine what risk factors are associated with the schizophrenic patient's suicidal behavior but they have yielded contradictory results. Thus we hypothesized that these results' variability could be due, among other reasons, to the heterogeneity of suicidal schizophrenic population. MATERIAL AND METHOD: Thirty-two Schizophrenics patients who had made Attempted suicide were studied. They were assessed through a battery of Clinical scales for negative, positive, hopelessness, insight and depressive symptoms. RESULTS: Two suicidal subtypes were identified according to the reasons that patients stated for attempting suicide. These two subtypes showed significant differences in two of the analyzed variables, namely hopelessness and PANSS positive subscale. DISCUSSION: Clinical implications coming from these results are discussed in the present paper.

[Symptomatic profile of depression].

There is no precise documentation about an universal nuclear composition of the depressive syndrome. Moreover, secondary manifestations of depression are even more controversial. In this paper the symptomatic profile of a sample of depressive patients attended in the Hospital Universitario de Canarias is analyzed. The characteristics, profiles and frequencies of the symptomatic pattern are discussed and commented.

[Based on one case of dissociative disorder: a conceptual review].

Based on the description of a clinical case treated by the authors, and diagnosed of dissociative disorder, a review of the diagnosis of dissociative disorder and its polemics is carried out. The authors discuss concepts such as dissociation and hysteria, their historic evolution and their relationships. Some modern cognitive theories on dissociative disorders and their relationship or opposition to psychodynamic theories are presented. The differences between dissociation and repression with these two different approaches are also mentioned. The authors conclude that at the present time important questions must be solved in the area of dissociative disorders in order to progress in the psychiatric knowledge of dissociative processes.

[Quantification of the stress reactivity in psychiatric patients admitted to an acute care unit].

We studied stress reactivity in a sample of 48 psychiatric acute inpatients of a psychiatric ward in a general hospital. Stress Reactivity Questionnaire (IRE-32 items) was used, this instrument supplies an index of the subject's individual response to stressful situations. The mean Stress Reactivity Index of the sample was 13.02 (s.d. 6.31), which is significantly higher than that found by us in general population--9.67 (s.d. 5.47)--and similar to the score obtained by psychiatric outpatients in our Department--13.30 (s.d. 6.07). According to gender, women reached the highest score (13.76). The various scores obtained in the Stress Reactivity Index by the different diagnosis are outlined.

[Stress reactivity index-32 in a student population].

We present here the preliminary results of the 32-item version of the reactivity to stress questionnaire. The sample consisted of 84 medical students. The mean score was 9.9. There were no sex difference in the total reactivity to stress index score or in any of the subscales.

[Insomnia in asthmatic patients].

In this paper, in order to assess the prevalence and features of insomnia as a subjective complaint in a sample of asthmatic outpatients. (n = 43; mean age = 36.39 +/- 12.85), items 44, 64 and 66 (that deal with insomnia complaints) of Derogatis' SCL-90-R (Symptom Check List 90 REvised), that was applied to the patients of the sample, are studied. A high prevalence of insomnia complaints is observed in our patients; over 70% of the sample manifests complaints of some type of insomnia. Insomnia complaints were higher (p < 0.0001) than in general population. No significant differences were evidenced according to sex and no correlation with age was observed in any of the items taken into account for insomnia complaints. A high positive intercorrelation was evidenced among the three items that we considered. Various factors associated with insomnia in asthma are discussed.

[Life events and self-destructive behavior].

The role of life events in precipitating self-destructive behaviour, especially in suicide attempts, needs to be studied in this paper the social readjustment rating scale (Holmes and Rahe), in its spanish version, in a sample of 100 suicide attempters are reported. The mean social readjustment index was 255.2 +/- 137.01, which is a higher score than that observed in medical inpatients (180.69 +/- 72.03) in our context. The mean life events index was 8.69 +/- 4.32, higher valves than were recorded in the somatic in patients sample (5.97 +/- 2.99). No sex differences were found in any of the two indexes. The finding of a greater number of unpleasant life events by different researchers is discussed.
