[Role of accelerated electrons in the therapy of advanced-stage skin epithelioma advanced stage: analysis of 31 cases].

Electron beam therapy is the superior modality of treatment for advanced skin cancer or in specific anatomical situations. The authors report the results for the electron beam therapy of 31 patients with advanced skin carcinoma. There were 15 squamous cell carcinomas, 11 basal cell carcinomas and 5 mixed, basosquamous cell carcinomas. All lesions were 3 cm or greater in maximum diameter. With follow-up ranging from 1 to 6 years, 28 patients have not evidence of disease, 1 is dead with disease, 1 is alive with suspicious residual disease and 1 is alive with recurrence. Acute and late complications are acceptable; bone, cartilage and soft tissue necrosis are not reported. The treatment results are similar to those in the literature and show the advantages of the electron beam irradiation for advanced skin carcinomas.

[The treatment of cancer of the anal area with radiotherapy. An analysis of the authors' personal experience].

The treatment of anal canal carcinoma is very difficult and great care must be taken in planning therapy because this type of cancer is potentially curable. From 1982 through 1989 we treated 16 patients with carcinoma of the anal canal. All patients were evaluable for toxicity and 14/16 for response after treatment. Of the 12 patients who underwent irradiation alone and were N0 M0, 8 are alive with no evidence of the disease, 4 had a recurrence which was treated with radical surgery, and 2 underwent palliative treatment alone. Therapy toxicity was not relevant in most cases and our results are in agreement with those reported in the literature.

A new technique for endocavitary irradiation of the esophagus.

Recently, high-dose rate intracavitary irradiation has been used as a boost therapy or a palliative therapy in the treatment of the esophageal carcinoma. The new intraesophageal device is based on a modified Sengstaken-Blackemore tube. On the surface of the esophageal balloon of the S-B tube Cyponil tubes are fixed as iridium containers. The device is placed at the level of the neoplastic lesion. Treatment tolerance is good.

Recurrent primary hydatidosis of sartorius muscle. Case report.

A case of recurrent primary hydatidosis of sartorius muscle is reported. Initially an intact cyst was enucleated and the cavity irrigated with hypertonic saline, but 41 months later the cyst had recurred. Cysto-pericystectomy and saline irrigation were now performed, followed by mebendazole for 21 days. The patient is recurrence-free after 19 months.

Ovarian function after pelvic lymph node irradiation in patients with Hodgkin's disease submitted to oophoropexy during laparotomy.

Staging laparotomy for Hodgkin's disease was carried out on 110 women in fertile age at the General Surgery Department of Padua Medical School from 1972 to 1977. An oophoropexy was done during the operation. According to the stage the patients underwent radiation therapy that was associated in some cases to chemotherapy. Complete evaluation was possible only in 66 patients. Eighteen out of the 66 had mantle field and para-aortic lymph node chain radiotherapy. One case of amenorrhoea (5.5%) and five pregnancies (27.8%), were found among them. Nineteen had the same type of radiotherapy with MOPP. 3 cases of amenorrhoea (15.8%) and 3 pregnancies (15.8%) were found. Five had total nodal irradiation (TNI) according to Kaplan and all of them had amenorrhoea (100%). Twenty four had TNI associated to MOPP with 24 cases of amenorrhoea (100%). Our results slightly differ from those of other Authors. They showed: 1) that the oophoropexy is not effective using standard surgical technique (probably because of the size of the central shielding) as it causes amenorrhoea in 100% of the cases; 2) the incidence of permanent amenorrhoea from chemotherapy with MOPP is the same as reported by other Authors; 3) the absence of teratogenic effects by chemotherapy in newborns of these patients.

Electrophysiologic properties of intravenous oxprenolol in man.

Electrophysiologic modifications produced by intravenous administration of 0.1 mg/Kg Oxprenolol were studied in 16 subjects with estimated normal impulse formation and conduction. Significant effects were sinus bradycardia, mild increase of sino-atrial conduction time, depression of intranodal conduction and prolongation of A-V node refractory periods. Sinus node recovery time and atrial refractory periods were unchanged. Infranodal conduction and the refractory periods of the His-Purkinje system, as well as of the bundle branches, were unchanged. These effects are compared with those observed after intravenous Propranolol and Prindolol.

Lack of correlation between sex hormone binding globulin, adrenal and peripheral androgens in precocious adrenarche.

Sex hormone binding globulin (SHBG) is a specific steroid-binding plasma glycoprotein regulated by several different factors. Sex steroids are currently considered to be the main physiological regulators of this protein. Testosterone (T) in adults seems to be the main hormone active in lowering SHBG. The role of dihydrotestosterone (DHT) in such regulation, particularly in the prepubertal age, is not well understood, and no data exist about the role of 3 alpha-androstanediol (3A alpha) and its glucuronide. In adulthood, in addition to T, 5-ene steroids seems to play a role in the regulation of SHBG plasma concentration. To assess the effect of adrenal and peripheral androgens in modulating SHBG levels in the prepubertal age, we studied subjects with precocious pubarche secondary to precocious adrenarche (PA). PA represents, in fact, a good model of study as it is characterized by an increased production and action of adrenal androgen in females under 8 yr of age and in males under 9. Sixty-five subjects (55 females and 10 males; chronologic age: 3.6 - 8.2 yr (6.9 +/- 1.3, SD); bone age: 3.6 - 11 yr (7.6 +/- 1.9); BMI 17.9 +/- 3 kg/m2) were studied. Fifteen age-matched normal children (BMI 15.2 +/- 0.8 kg/m2) were studied as controls. Androstenedione (A), dehydroepiandrosterone (DHA) and its sulphate (DHA-S), T, DHT, 3Ad and its glucuronide (3AG) and SHBG were evaluated in all subjects. In PA cases serum SHBG levels (50 +/- 27 nM) were significantly lower (p less than 0.05) with respect to normal prepubertal patients.(ABSTRACT TRUNCATED AT 250 WORDS)

Identification and localization of atrial natriuretic factor receptors in human spermatozoa.

Specific binding sites for atrial natriuretic factor (ANF) have been detected and localized in viable human spermatozoa through radioreceptor analysis and autoradiography, respectively. Radiotracer uptake was time and concentration dependent. Scatchard analysis of saturation data showed a single class of ANF receptors with a kd of 2.5 nM and a Bmax of 1.03 fmol/10(6) sperm 2.5 min-1, corresponding to about 620 molecules per sperm. Nonreducing SDS-PAGE analysis after covalent cross-linking of sperm bound 125I-ANF evidenced a single displaceable (i.e., specific) band with an apparent molecular weight of 135-140kD. In 125I-ANF bound spermatozoa, optical autoradiography showed an exclusive distribution of silver grains covering the midpiece region. The effects of ANF binding on ionic homeostasis and cyclic nucleotide metabolism, which modulate a number of sperm cellular processes, could make this factor play outstanding roles in gamete physiology.

Adenocarcinoma and epidermoid carcinoma of the lung in the elderly.

Primary lung carcinoma, in the elderly, presents some interesting epidemiologic and clinical characteristics. The authors, after reviewing the international literature, stress the differences of behavior and incidence of epidermoid carcinoma and adenocarcinoma occurring in elderly patients as compared to those found in younger patients. They report their experience which is similar to that reported in the medical literature.

[Venous bypass for treatment of chronic obstructive venous insufficiency of lower limbs].

About 1 to 5 percent of patients affected by chronic obstructive venous disease of the lower extremity, are eligible to surgical treatment, by veno-venous bypass, for the relief of unerely invalidating symptoms. Indications to operation include a class II to III chronic venous insufficiency of the deep venous system, failure of medical treatment continued for at least 12 months, good patency of outflow axis, a significant gradient of pressure between discared and outflow axis, a good quality saphenous vein to be utilized for bypass. The case of an aged patient, successfully treated with a femoro-femoral venous bypass, associating an artero venous fistula, is presented and indications to the operation in the older patients, together with indication to and technical details of performing the fistula are discussed.

[Surgical treatment of inguinal hernias with local anesthesia].

Fifty-nine cases of inguinal hernia repair with local anesthesia are shown. The authors stress the advantages of this safe technique permitting an immediate postoperative ambulation and an early discharge of patients, thus reducing complications related a long term bed-stay.

[Pulmonary hamartoma in geriatric surgery].

The pulmonary hamartoma is a kind of disease frequently shown in elderly. The diagnosis is often difficult and fine needle aspiration (FNA) can be used for histological features. CT scan can be also a such useful technique in differential diagnosis. A case is presented and its problems are discussed.
