A comparison of scar quality in wounds closed under tension with PGA (Dexon) and Polydioxanone (PDS).

Wounds closed under tension have a tendency to stretch. To assess whether this stretching can be limited by inserting absorbable subcuticular sutures, 28 wounds, which involved elliptical excisions, were closed with PGA and PDS sutures in opposite halves of the same wound. PGA scars were found to be significantly wider than their PDS counterparts and were associated with greater hypertrophy at 6 months.

Plasticized polyvinyl chloride film as a primary burns dressing: a microbiological study.

Twenty-four patients were studied after the application of plasticized polyvinyl chloride (PVC) film as a temporary burns dressing. The burns were assessed as being partial thickness (nine patients) and full thickness (15 patients) in depth and involving 2 per cent to 35 per cent of the body surface area. Microbiological cultures were performed on the exudate beneath the plasticized PVC film and then at each dressing change until the burn was healed. Bacterial isolates showed no unexpected organisms and there was no predominant organism to suggest contamination from the plasticized PVC film. Indeed, bacteria were only rarely isolated from the initial exudate (three of 37 burn wounds), and subsequent bacterial cultures did not differ from those normally found in burn wounds. In addition, incorporation of the plasticized PVC film into nutrient broth and agar with Staph, aureus and Ps. aeruginosa did not influence the recovery of these organisms, indicating that plasticized PVC film does not exert any antibacterial effect.

Dorsal metacarpal flaps.

The history of dorsal metacarpal flaps and their anatomical basis is described. In thirty hand dissections, the fascial variety of the first dorsal metacarpal artery was present in 90% and the second dorsal metacarpal artery in 97%. The origin, course, branches and termination of these arteries are illustrated. Doppler ultrasonic flowmeter studies of the vessels in 52 hands are presented but shown to be unhelpful clinically. Two patients in whom the second dorsal metacarpal flap was used, are described and possible difficulties delineated.

Diffuse cavernous haemangiomas of the upper limb.

In the upper limb, diffuse cavernous haemangiomas occur infrequently but their treatment is often unsatisfactory and the result of surgical excision disappointing. Four cases of these diffuse haemangiomas are described and their management discussed.

The facial artery island flap.

A nasolabial island flap on the facial artery and anterior facial vein is described. This is a useful modification of the classic nasolabial flap for repair of defects in the floor of the mouth. The flap can be led directly into the floor of the mouth, and the pedicle can be passed deep or superficial to the mandible as required. This obviates the use of bulky carrier skin passing over the alveolus and allows all the nasolabial skin to be available for reconstruction of the defect.

Acne vulgaris associated with a latissimus dorsi musculocutaneous island flap.

A case is presented of the development of acne vulgaris in the skin island of a latissimus dorsi musculocutaneous flap used for breast reconstruction. The patient had acne scarring of the back but had had no active acne for twenty-five years. The latissimus dorsi musculocutaneous island flap is commonly used in breast reconstruction. The development of acne in association with skin flaps has not been described before and we speculate as to its cause.

MR and CT in the assessment of Poland syndrome.

Seven cases of Poland syndrome were investigated with CT and/or MR to assess the extent of the muscle abnormalities of the shoulder girdle. In all cases the absence of the sternocostal head of the pectoralis major was clearly shown, as were any associated abnormalities of the pectoralis minor, serratus anterior, and latissimus dorsi. The latissimus dorsi muscle has an important role in reconstructive surgery and can be difficult to assess clinically. Cross-sectional imaging with CT or MR will give useful information in patients with Poland syndrome requesting anterior axillary reconstruction and breast mound formation.
