A simple and reliable method for the calibration of stenoses in intestinal Crohn's disease, using a marble, is presented. With this procedure, unnecessary resection or stenoplasty can be avoided and adequate passage of intestinal contents throughout the whole length of the small intestine is secured.
The solitary rectal ulcer and colitis cystica profunda are different manifestations of the solitary rectal ulcer syndrome. The cause of solitary rectal ulcer syndrome remains unknown. Since defecation disorders are common among patients with solitary rectal ulcer syndrome, defecography is indicated. Defecography was performed on 19 patients with solitary rectal ulcer syndrome. In five patients, the spastic pelvic floor syndrome had occurred. Twelve patients had internal intussusception of the rectum, and one patient had an anterior rectal wall prolapse. In one patient, no abnormalities could be detected. These abnormalities led to severe straining, which can damage the anterior rectal wall. Findings strongly support the hypothesis that solitary rectal ulcers are traumatic lesions caused by straining. Defecography is a suitable procedure for detecting the causative disorder of defecation and for selecting patients for treatment.
A continent ileostomy for patients with ulcerative colitis or polyposis coli is one of the alternatives to a conventional ileostomy. Construction of the continent ileostomy is a complex operation that can be followed by multiple, serious, and rather intractable surgical complications. In a small series of ten patients, a temporary diversion loop ileostomy was used to promote uncomplicated healing. The lack of any immediate surgical postoperative complications supports our point of view that, at least in centers with little experience of continent ileostomy, a two-stage procedure is preferable to a one-stage procedure.
A patient with anorectal varices is reported. Anorectal varices are a rare cause of rectal bleeding and are often erroneously diagnosed as bleeding hemorrhoids. However, the subcutaneous extension of the varices should alert the clinician. Although local treatment such as ligation of the varices has been advocated, in the presence of portal hypertension a more radical operation is recommended. A portosystemic shunt, preferably between the inferior mesenteric vein and the vena cava or renal vein, is then the treatment of choice.

A prospective comparative study between liver enzymes, scintigraphy, ultrasonography and computed tomography for detection of liver metastases was performed in 113 patients who were eligible for operation for a carcinoma in the gastrointestinal tract. Twenty-three of the 113 patients (20 per cent) had liver metastases. The accuracy of scintigraphy was 79 per cent, of ultrasonography 85 per cent and of computed tomography 88 per cent. The results of these tests showed no statistical differences. Ultrasonography and computed tomography were significantly more accurate than alkaline phosphatase and gamma-glutamyl transpeptidase (respectively P less than 0.01 and P less than 0.05). Taking into account cost effectiveness, simplicity and patient tolerance, ultrasonography is the test of choice for the detection of liver metastases.

In a dog model, both transfusion of leucocytes and of erythrocytes induced lymphocytotoxic antibodies. The results, obtained in DLA identical littermate and DLA nonidentical nonrelated beagle recipient pairs, suggest a correlation between DLA-type and lymphocytotoxic immune response. Two weeks after the last of three transfusions from different blood donors, kidneys from unrelated DLA-mismatched donrs were transplanted to the beagle recipients. A standard postoperative immunosuppressive regimen was given. No correlation between erythrocyte or leucocyte induced immune response was observed, but both erythrocyte as well as leucocyte transfusions significantly prolonged renal allograft survival as compared with nontransfused controls.
