Stress and psoriasis: the importance of insight and empathy in prognosis.

The literature concerning the relationship between psoriasis and stress is reviewed. It is concluded that emotional stress is a significant precipitating factor. Treatment of stress is prognostically beneficial when patients understand the stress flare phenomenon and gain insight into the nature of any emotional trauma they have suppressed. Details of such insight are given and a consultation setting is described that has helped gain such rapport. It is shown that a significantly improved prognosis follows early and complete clearing of the rash.

Short contact dithranol therapy: advantages of a stiff emulsifying ointment base.

In a single blind trial, 16 in-patients with psoriasis were treated with short contact dithranol in stiff Lassar's paste and in a stiff emulsifying ointment base of equivalent stiffness. The preparations were equally effective in clearing psoriasis and well tolerated. Perilesional treatment burns, reported with soft ointments, were not seen with the stiff emulsifying ointment preparation. Furthermore, unlike the paste, excess stiff emulsifying ointment can be removed by simply wiping with a moistened cloth, thus greatly facilitating home management of psoriasis.

Psoriasis and stress. II.

A retrospective assessment of insight was undertaken in 61 patients whose psoriasis was considered to have been precipitated by stress. A significantly better prognosis was observed in those patients assessed as having had insight.

Psoriasis and stress.

132 psoriasis patients who had been completely cleared with dithranol were assessed and then followed up over a 3-year period. The incubation time from specific incidents of stress to the development of psoriasis was between 2 days and 1 month. Specific stress within a month before the first attack was recalled by 51 patients (39%); chance association was excluded by studying a control group where there was significantly less specific stress. The prognosis for the psoriasis patients who recollected specific stress a month prior to the onset was significantly better than for the rest of the patients where none had been recalled.

Simplified dithranol treatment for psoriasis.

Progressive simplification of dithranol therapy has made treatment at home more acceptable. This has included ommitting ultra-violet light, talc and dressings, and substituting an ordinary bath instead of one containing tar. A stiffened dithranol 0.5% ointment was found to be slightly more effective than the best paste hitherto employed. It was also very much easier to use.
