This article examines the current management of trauma to the spleen. The incidence, mechanism, classification, diagnosis, treatment and complications of splenic trauma are reviewed. Modern radiological investigations are assessed in view of the recent vogue for non-operative management. The effects of splenectomy and particularly of overwhelming postsplenectomy sepsis are discussed. The role of non-operative management of splenic injuries in children and in adults without associated injuries is emphasized. Means of repairing and preserving the spleen are detailed. Prompt splenectomy is necessary in seriously traumatized patients, especially those with head or multiple injuries.
We report a case of postpartum rupture of a subcapsular hematoma of the liver. The etiology, pathology, diagnosis and treatment of this condition are discussed. We emphasize the modern concepts in treatment of hepatic hemorrhage. This knowledge and an awareness of the possible diagnosis will help to decrease the high morbidity and mortality rates associated with this disorder.
The purpose of this investigation was to obtain performance data from normal-hearing children using the written word portion of existing auditory-visual word identification materials, for example, the Picture Identification Task (Wilson and Antablin, 1980). The response foils, consisting of four written words (target and alternative words) were entered into computer memory so that responses were made by pointing to a computer monitor. Subjects were 24 fourth graders with normal hearing who repeated auditory-stimulus words in noise in oral conditions, and pointed to written words representing the stimulus words in pointing conditions. Mean percent correct performance scores were higher in the pointing conditions than in the oral conditions, reflecting closed- versus open-set tasks. The results indicated that the written word portion of the Picture Identification Task can be utilized to assess the word identification performance of children who read at a fourth grade level or above.

This review examines pancreatic trauma and its management in the light of recent experience. The incidence, mechanism, classification, diagnosis, treatment and complications of pancreatic trauma are discussed. The difficulty in establishing the diagnosis is addressed and possible solutions are provided. The case for conservative surgery in the absence of pancreatic duct damage is outlined. The importance of draining all pancreatic injuries is emphasized.

Diagnosis of hepatic injury may be difficult and specific investigations are suggested. Non-operative treatment and the strict criteria necessary to identify suitable patients for this are assessed. The recent trend to conservative surgery in hepatic trauma is reviewed. Aggressive resuscitation and early control of bleeding are crucial and are emphasized.
The NU No. 6 materials spoken by a female speaker were passed through a notch filter centered at 247 Hz with a 34-dB depth. The filtering reduced the amplitude range within the spectrum of the materials by 10 dB that was reflected as a 7.5-vu reduction measured on a true vu meter. Thus, the notch filtering in effect changed the level calibration of the materials. Psychometric functions of the NU No. 6 materials filtered and unfiltered in 60-dB SPL broadband noise were obtained from 12 listeners with normal hearing. Although the slopes of the functions for the two conditions were the same, the functions were displaced by an average of 5.8 dB with the function for the filtered materials located at the lower sound-pressure levels.
Cardiovascular disease is a significant health problem for the Zuni Indians of southwest New Mexico, in part because of high rates of non-insulin-dependent diabetes mellitus (NIDDM). The Zuni Diabetes Project was initiated in July 1983 to reduce rates of obesity and provide primary and secondary prevention of NIDDM. Two studies of the project's activities have been carried out to date. After 2 y of follow-up, diabetic participants in an exercise program compared with diabetic nonparticipants experienced weight loss, a drop in fasting blood glucose values, and reductions in the use of hypoglycemic medications. In a weight-loss competition, 45% (122/271) of the enrollees finished and lost greater than or equal to 2.3 kg. The results of these two studies demonstrate that 1) participation in a community-based exercise program can produce significant weight loss and improvement in glycemic control in Zuni Indians with NIDDM and 2) weight-loss competitions appear to be an important public health model for health-behavior change in communities similar to that of Zuni, NM.
The Department of Veterans Affairs recently produced a compact disc of speech audiometry materials. The compact disc, which is available commercially, includes the W-1 spondaic words recorded by a female speaker. Two experiments were conducted. The purposes of experiment 1 were to obtain normative detection and recognition data on the female recording of the spondaic words and to compare the detection and recognition functions for the original male speaker version of the W-1 words. No significant differences were found between the recognition functions for each speaker. The recognition functions for both speakers were displaced to higher sound-pressure levels by 8 dB above the detection functions. Clinically, the two versions of the W-1 spondaic words should produce equivalent results. In experiment 2, slopes of the individual spondaic word recognition functions for the female speaker were obtained from two listeners and are discussed in terms of interstimulus, intertrial, and intersubject variability.
From a rat genomic library in phage lambda Charon4A, a complete glutamine synthetase-encoding gene was isolated. The gene is 9.5-10 kb long, consists of seven exons, and codes for two mRNA species of 1375 nucleotides (nt) and 2787 nt, respectively. For both mRNAs, full-length cDNAs containing a short poly(A) tract were identified. The sequences of the entire mRNA and of the exon-intron transitions were determined. The smaller mRNA is identical to the 5' 1375 nt of the long mRNA and contains the entire protein-coding region. The position of the transcription start point was mapped. Within the first 118 bp of promoter sequence, a (T)ATAA-box, a CCAAT-box and an SP1-binding site were identified.
Two descriptive experiments were performed on a version of the Northwestern University Auditory Test No. 6 (NU No. 6) recorded by a female speaker that is included on an audio compact disc recently produced by the Department of Veterans Affairs. In Experiment 1, normative psychometric functions for the female speaker version of the NU No. 6 materials were established on 24 young adults for three monaural listening conditions (in quiet, in 60-dB SPL broadband noise, and in 60-dB SPL competing message). The 60-dB SPL broadband noise shifted the psychometric function for the NU No. 6 words 33 dB, whereas the 60-dB SPL competing message shifted the function only 18-22 dB. In contrast to the slopes of the quiet and noise conditions (4.5%/dB), the slope of the competing message function was more gradual (3.5%/dB). In Experiment 2, comparisons between the psychometric functions for the female and the original male speaker versions of NU No. 6 in quiet and in broadband noise were made on 8 young adults. In comparison to the psychometric functions for the male speaker version of NU No. 6, the functions for the female speaker version of NU No. 6 were displaced between the 10-90% correct points to higher sound-pressure levels by 10-13 dB in quiet and by 12-16 dB in noise. The difference in performance on the two versions of NU No. 6 is attributed to spectral differences between the two sets of materials that produced a calibration anomaly.
Injection sclerotherapy for acutely bleeding oesophageal varices has been used in Belfast since 1958. However, a chronic injection sclerotherapy programme with rigid oesophagoscopy under general anaesthesia commenced only in 1979. So far, 82 patients have entered the programme; 57 patients had already received 73 acute injections before commencing chronic sclerotherapy. Subsequently, the 82 patients received 221 chronic injections plus a further 29 acute injections for rebleeding episodes which occurred during the programme. There were 24 Child's grade A patients, 23 B and 35 C; 48 per cent had alcoholic cirrhosis. Forty-eight patients achieved variceal obliteration with a mean of four injections. During the programme 24 patients experienced 42 acute bleeds. There were only two bleeding episodes within 1 week of a chronic injection and eight within 4 weeks. In the 8-year period there have been four early deaths. One occurred 17 days after a chronic injection and three followed acute injections required for rebleeding during the programme. There were 21 late deaths, mostly due to progressive liver failure, and none from rebleeding. We conclude that chronic injection sclerotherapy using rigid oesophagoscopy under general anaesthesia is both safe and effective.

The detection, loudness, and discrimination performances of subjects with normal hearing were assessed using 3 five-component tonal complexes (1020-1100 Hz) with crest factors of 1.8, 2.6, and 3.2. The 3 five-component tonal complexes were: (1) equally detectable in broadband noise when presented at equal rms amplitude; (2) perceived as equally loud when presented at essentially equal rms amplitude; and (3) discriminable from one another when presented at equal loudness. The present data indicate that listeners can make discriminations on the basis of amplitude variations.
