[The palliative intubation of malignant esophageal stenoses by means of a spiral tube. Indications--technic--results].

This is a 15-year-report on experience in the use of a special designed esophageal tube for the treatment of inoperable carcinoma of the esophagus and gastric cardia in 59 patients. Indications and operative procedures are described in detail. The operative mortality was 23.7%. The result was good in 66%, satisfactory in 27%, and poor in 7% of all patients. The longest survival time was 15 months. Complications of the endoesophageal intubation were dislocation, obstruction, penetration, perforation, and bleeding.

Arterio-venous fistulas constructed with formalin-fixed human umbilical grafts.

Since July 1976 we have implanted 18 umbilical vein grafts as haemodialysis shunts. Our experience shows that the umbilical vein is suitable for all shunt localisations. The formalin-fixed umbilical vein graft can be considered for vascular replacement. These can be removed in any clinic, prepared and stored under sterile conditions. The formalin conservation procedure is simple, inexpensive and possible everywhere without elaborate technical equipment.

[Does flow-limited portal arterialization avoid the disadvantages of portacaval shunts in healthy and cirrhotic rats? (author's transl)].

The disadvantages of portacaval end-to-side shunt concerning hepatic function can be avoided by additional portal arterialization in healthy liver and cirrhotic rats. The maintenance of total liver perfusion is the cause of good RES clearance, nearly normal capacity of synthetic functions and normal biliary excretion. Flow and thereby pressure-adapted technique of arterialization is necessary.

[Lessening the impairment of wound healing by pre-or postoperative quantitative factor-XIII substitution (author's transl)].

In the light of listed references, a report is given on the influence of the clotting factor XIII in experiments with animals as well as with patients. Through own examinations in 95 patients with severe abdominal and vessel surgery, a clear lessening of impairment of wound healing could be achieved by the quantitative pre- and postoperative substitution of the fibrin stabilizing factor.

[Experience in treating hepatic coma by extracorporal liver perfusion (author's transl)].

19 extracorporeal liver perfusions have been performed for treating hepatic coma in 11 patients. No improvement was found in 3 patients, improvement in 8.5 of these 8 patients recovered full consciousness. All 7 patients with pre-existent liver disease died. Of 4 patients without pre-existent liver disease, 2 were discharged and are still alive. At present, extracorporeal liver perfusion seems to be the most promising treatment for hepatic coma. It should be undertaken at Stage IV and at Stage III with increasing prothrombin time. Pre-existent liver disease should be excluded before by laparoscopy.

[Immunoreactive parathyroid hormone in samples from thyroid veins (author's transl)].

Blood samples were obtained from 16 patients with primary or secondary hyperparathyroidism by selectively catheterising cervical and thyroid veins. Immunoreactive parathyroid hormone was measured with two aminoterminal (anti-N)- and three carboxyl-terminal (anti-C)-specific antisera. Two anti-C sera and one anti-N serum were useful in localising the parathyroid tumors. In addition, the parathyroid hormone fragments were separated by gelfiltration and the immunoreactivity in the effluent was estimated with anti-N and anti-C sera. The results suggest that the usefulness of antisera for parathyroid tissue localisation is determined by their affinity for the intact hormone, regardless of their anti-N or anti-C qualities.

[Results of operative treatment of acute perforating gastroduodenal ulcer (author's transl)].

In this report of 264 operatively treated gastroduodenal perforations, differentiated, and where possible definitive, therapy is attempted. By omitting simple oversewing in favor of primary resection, bionomic operation or the combined procedure, primary lethality is reduced to one third and the late results of the survivors significantly improved. Resections are indicated in benign pyloric stenosis and gastric ulcer. The bionomic operation is particularly to be preferred in young patients.

[Swine liver ultrastructure following hypothermia and hyperbaric oxygenation preservation].

The livers of 20 pigs were preserved for up to 16 hours, applying hypothermia (4 degrees C) and hyperbaric oxygenation (3 ata). As a preparation solvent we used in one group the solution by Lie (17) which is rich in potassium and glucose, and in the other group the cryoprecipitated serum by Belzer (5). After 8 and 16 hours of preservation, the fine structure was examined light- and electronmicroscopically. After 8 hours of preservation, there were only little reversible changes in both groups. After 16 hours, both groups showed distinct changes of the endoplasmatic reticulum e. g. desorganisation, vesicular dilatation, and dissociation of the mural ribosomes. In addition, there were swellings of mitochondriae and Kupffer's cells. These changes were particularly distinct after preservation in cryoprecipitated serum. In this group a cellular depletion of glucogen was noticeable.

[Giant cavernous hemangioma of the liver. Case report and review of the literature (author's transl)].

The cavernous hemangioma of the liver is frequently seen as small tumor without any symptoms, but only rarely its diameter is more than 4 cm (161 cases in world literature). We can report on an additional patient. The peculiarities in this case were the tumor size, the presence of numerous phleboliths and the contrasting of the tumor taking place mainly from the venous part of the vascular bed of the liver. The main indication for a surgical intervention is the danger of rupture, but disturbances of the coagulation as well as intestinal suppression symptoms have also to be taken into consideration. The intraoperative tactics ought to be a standardised resection of the liver. The cavernous hemangioma must be differentiated from the hemangioendothelium: this usually occurs in children, causes cardiac failure due to a. v. shunts, and responds to conservative therapy (corticosteroids, radio-therapy).

[Pathogenesis, clinical application and therapy of chylothorax (author's transl)].

With the growth of traffic density and the spread of thoracic surgery, the clinical picture of the chylothorax can be observed with increasing frequency. Above all, the spontaneous or symptomatic chylothorax presents diagnostic problems, since, in about 70% of the cases, it only represents the concomitant symptom of a malignant tumor. Only after exact diagnostics and the exclusion of a malignant primary disease can one be content with the cursory diagnosis of an idiopathic chylothorax. However, this requires strict postoperative control and observation of the patients. Two patients are presented in whom the genesis of the chylothorax still remained unclear 3 1/2 years after the successful ligature of the thoracic duct.

[Clinical signs and therapy of the abdominal aortic aneurysm (author's transl)].

The abdominal, infrarenal aneurysm of the aorta shows a very unfavorable spontaneous outcome. Therefore, every patient should be operated on without delay. The surgical procedure consists of a partial resection of the aneurysm and the reconstruction of the vascular flow by means of a synthetic prosthesis. The most dangerous complication of the aneurysm is its rupture. Emergency surgery of a ruptured aneurysm is accompanied by essentially higher risks: The mortality rate lies between 34 and 90%. The prognosis of the operation varies with the patient's age, coronary sclerosis, and renal insufficiency. The diagnosis of the aneurysm can clearly be made with sonography, angiography, and computer tomography. In the case of a ruptured aneurysm, clinical signs are of primary importance. Angiography is not indicated here. From May 1969 until November 1976, 41 patients were operated on in our clinic: 23 electively and 18 after rupture. The mortality rate of the elective cases amounted to 17% and that of the ruptured to 72%.

[Hyperkinetic portal hypertension. Arterioportal fistula: problems--case reports--review of the literature].

Hyperkinetic portal hypertension is caused by pathological arterioportal shunts. Clinical differentiation is necessary between extrahepatic fistulas, splenoportal hypertension (arteriovenous anastomoses at the level of the pre-penicilary arteries) and intrahepatic fistulas in "active" cirrhosis and malignant tumors. This paper reports the clinical and angiographic features of eight patients with this type of fistula. A review of the literature is also presented (144 cases). Because of the severity of this disease, surgical intervention is necessary. The surgical technique depends on the organ-related necessity of vascular preservation.

[Prevention of postoperative thrombo-embolism by heparin/dihydroergotamine (author's transl].

Using the radiofibrinogen test and perfusion lung scanning (2-phase pharmacoscintigraphy), the incidence of postoperative thrombosis and pulmonary embolism was determined in a group of 362 operated patients. Low-dose heparin administration (twice 5000 IU daily) was given to 162, combined heparin and dihydroergotamine administration (5000 IU heparin and 0.5 mg dihydroergotamine daily) to 150, while 50 patients received no prophylactic treatment. There was a significant decrease in thrombo-embolism in the heparin/DHE group (8.7% deep-vein thrombosis; 2.7% pulmonary embolism) compared with the heparin group (19.8% and 5.5%, respectively) and the control group (30% and 14%, respectively).
