[Bilateral pheochromocytoma: laparoscopic surgery in 2 cases].

Laparoscopic adrenalectomy, if done by skilled surgeons, is now the first choice for treating most adrenal tumors, including bilateral pheochromocytoma. We report two women, aged 35 and 34 years old, with bilateral adrenal pheochromocytoma successfully excised by laparoscopic surgery. Both had severe hypertension, high urinary catecholamine values (epinephrine + norepinephrine: 528 and 1083 ug/24 h) and bilateral adrenal tumors at CT scan. After 4 weeks of doxazosin treatment, a laparoscopic transperitoneal adrenalectomy was done (Gugner's technique), with surgical times of 7 and 5 hours respectively. Both patients received hydrocortisone and only the second one required one unit of packed cells. Postoperative evolution was uneventful and both patients were discharged at the fifth postoperative day. At two months of follow up, both patients are asymptomatic and normotensive.

[Viral agents in patients with infectious processes of the central nervous system].

Local statistics report a great number of cases of neurological diseases of unidentified cause. The purpose of this research was to determine the presence of viral agents in patients with affection of the central nervous system, in Zulia State, Venezuela. In different health centers throughout the state, we collected 129 randomized samples of cerebrospinal fluid (CSF) and serum from patients ranging from 1 day to 41 years of age, showing symptoms of CNS disease and whose bacteriological test resulted negative. The serum/CSF albumin ratio was determined to exclude contaminated CSF samples, thus 54 optimal samples were obtained for the research. We determined serum IgM and CSF IgG specific for Venezuelan equine encephalomyelitis (VEE), Herpes simplex (HSV), Epstein Barr (EBV), Dengue, Rubella and Measles virus, through ELISA and indirect immunofluorescence techniques. Of the 54 samples studied, 33 (61.11%) were positive. %). The etiological agents implicated were VEE 14 cases (42.42%); HSV 11 cases (33.33%); EBV 5 cases (15.15%) and Dengue 3 cases (9.09%). We did not detect Rubella o Measles. Regarding the age distribution, the most affected group was that of children under 12 (83.34%). We analyzed the clinical manifestations and the CSF characteristics and did not find differences suggestive of an specific etiological diagnosis, even though certain signs could be considered early indications of CNS alterations due to viral causes. Our study shows that a large percentage of CNS affections of unknown etiology are produced by viral agents, indicating that in our region, most of this cases are not registered.

[Prostate neoplasm prevalence in Talca, VII Region of Chile].

BACKGROUND: In 1998, there were 1,218 deaths in Chile caused by prostate cancer. This figure results in a death rate of 16.6 per 100,000 males for this disease. AIM: To assess the prevalence of prostate cancer in the Seventh Region of Chile. MATERIAL AND METHODS: A probabilistic sample of 327 males aged 40 to 59 years old was studied. In all, a codified questionnaire was applied, a digital rectal examination was performed and a blood sample was drawn to measure prostate specific antigen. All digital rectal examinations were performed by the same observer. Patients with an abnormal rectal examination or prostate specific antigen were subjected to a prostatic biopsy under ultrasound guidance. RESULTS: In 14 subjects, the digital rectal examination was considered abnormal and in seven, prostate specific antigen was over 4 ng/ml. All subjects with elevated prostate specific antigen had an abnormal rectal examination. In three of the 14 subjects, the biopsy showed a well differentiated adenocarcinoma. All three were aged over 50 years old. The resulting calculated prevalence of prostate cancer was 9.2 per 1,000 males (CI 4.2-14.1). CONCLUSIONS: The cost effectiveness of screening for early diagnosis of prostate cancer must be calculated, to decide its incorporation in preventive medical examinations.

[Pheochromocytoma and von Recklinghausen neurofibromatosis: postpartum crisis and renal artery thrombosis].

Pheochromocytoma is observed with higher frequency in patients with von Recklinghausen neurofibromatosis. We report a 36 years old female with von Recklinghausen neurofibromatosis who developed mild hypertension during the fourth month of pregnancy. A cesarean section was performed at 37 weeks of pregnancy. Thereafter, the patient presented severe hypertensive and hypotensive crises, sinus tachycardia and fever. No evidences of an infectious disease were found. Abdominal ultrasound examination showed a right adrenal mass of 7 x 5 cm. High levels of urinary cathecolamines confirmed the diagnosis of pheochromocytoma. After three weeks of prazosin therapy, the patient was operated. During the surgical procedure, an encapsulated pheochromocytoma was found and excised. A right renal atrophy and renal artery thrombosis were also found and a nephrectomy was done. Postoperative evolution was uneventful and the patient remains with normal blood pressure levels six months after the operation.

[Reproductive history of Chilean puerperal women hospitalized in a public hospital of the metropolitan region].

BACKGROUND: Maternal and infantile morbidity is closely related to the features of women's reproductive history. AIM: To study the reproductive behavior of women hospitalized due to labor or abortion in a public maternity of Metropolitan Santiago. SUBJECTS AND METHODS: One thousand women were interviewed about their reproductive behavior, 24 to 72 hours after pregnancy resolution. RESULTS: Ages of interviewed women ranged from 13 to 47 years old. Eighty seven percent had three children or less. The proportion of single women with a first pregnancy was 62% and decreased to 9.3% in those with a second pregnancy. At the first pregnancy, 1.3% of women were using contraceptives, notwithstanding that 27% did not want to become pregnant. Most women had a birth spacing of 24 to 59 months. There was a low number of stillbirths in this sample. CONCLUSIONS: This work shows that studied women have pregnancies at low risk ages and an adequate parity.

[1st sexual intercourse and 1st pregnancy in puerperal women at a hospital of the metropolitan area].

One thousand women hospitalized at a maternity were surveyed after their delivery or abortion about their first sexual intercourse and pregnancy. Mean age at the first sexual intercourse was 18.7 years and only 9.2% used a contraceptive method in spite that 20% were not willing to become pregnant. The mean age of sexual partners was 22 years old. Women of less than 32 years old generally had older sexual partners, but among those older than 25 years old the proportion of younger partners increased. The first pregnancy occurred at 20 years old as a mean, 62% of women were single, only 1.3% used a contraceptive method, and 27.3% of resultant pregnancies were not desired.

Purification of FKBP-70, a novel immunophilin from Saccharomyces cerevisiae, and cloning of its structural gene, FPR3.

A novel protein, belonging to the yeast family of FKBPs (FK-binding proteins), FKBP-70, was isolated from Saccharomyces cerevisiae by its interaction with the immunosuppressive agent FK-520. Its structural gene, FPR3, was cloned and the protein expressed and purified from Escherichia coli. This third member of the FKBP family in yeast is homologous to the other FKBPs at its carboxy terminus, showing conserved ligand binding and proline isomerase regions. It is, however, a longer acidic protein with several potential nuclear targeting sequences and a region of homology to nucleolins. Yeast strains deleted for FPR3, as well as a triple deletion mutant of this family of genes, FPR1, FPR2 and FPR3, are viable under normal conditions of growth, indicating that the FPR genes are not essential for life.

[Expectant management of missed abortion].

A hundred cases of consecutive missed abortion under expectant management are analysed. The average age for these patients was 29.1, 81% were multiparous, and in 65% of the cases they were not using any contraceptives in the last year. The mean score for haematocrit has no variations. These cases were resolved in an average of 12.8 days. In 42% of the cases, the abortion is complete and with no need for instrumentation. In 43% of the cases, curettage is necessary but in only 7% dilatation is necessary. In 16% of the cases there are some types of complications but in only 1% it is serious, not by the expectant management. Concluding that the conservative management in missed abortion cases represents a good alternative focussing classic dilatation and curettage in selected patients in which you can obtain an adequate continuation and subsequent control.

Prenatal ultrasonographic diagnosis of acardiac twin.

Acardiac twinning is a rare complication of monozygotic twin gestation occurring in 1/35,000 pregnancies or in 1% of all monozygotic twins. This condition is characterized by partial or complete lack of development of the heart in one of the twins, and requires that the normal twin (pump twin) provides circulation for itself as well as the acardiac sibling (perfused twin) by means of reverse circulation through large artery-to-artery and vein-to-vein anastomoses. The acardiac anomaly is uniformly fatal for the perfused twin, and the perinatal mortality for the normal twin is about 50%, usually as the result of congestive heart failure, polyhydramnios, and preterm delivery. In this report we present two cases of acardiac twin prenatally diagnosed by ultrasound. The principal sonographic features for prenatal diagnosis and the clinical management are briefly discussed.

[Podalic version in multiparae. Vaginal or cesarean?].

A review of maternal and perinatal results of multiparous patients with breech presentation was done during two period; two years before and two years after establishing the rule of cesarean section for all multi-parous with breech presentation. Seventy cases were considered at the first period in which 47.1% were vaginal delivery, proportion that significantly decreased (15%) at the second period. No significant differences on maternal results by delivery form were found and the same was found in perinatal results at comparing vaginal delivery and cesarean section. A different situation was reported studying only elective cesarean section, in which a significant decrease of severe neonatal depression was found comparing to vaginal delivery (24.2% and 0%). Because of rapid recovery there were no differences at fifth minute. As a conclusion, it appears as an exaggeration the option of cesarean section for all multiparous with breech presentation. Secondly, the adequate selection of cases to resolve by vaginal delivery may not produce adverse maternal and perinatal results.

Isolation and sequencing of NOP1. A yeast gene encoding a nucleolar protein homologous to a human autoimmune antigen.

We have identified the gene for the yeast nucleolar protein p38 and deduced the primary structure of p38 from its sequence. We propose the name NOP1 (nucleolar protein 1) for this gene. NOP1 encodes a 327 amino acid protein of 34,470 daltons and is flanked by potential promoter and polyadenylation sequences. Blot analyses indicate that the mRNA transcribed from NOP1 is approximately 1.3 kilobases in size and that there is one NOP1 gene per haploid genome. The amino-terminal sequence of p38 is homologous with the 31 known amino-terminal residues of the autoimmune antigen fibrillarin, confirming the previously observed similarity between p38 and this mammalian nucleolar protein. Consistent with this, p38 cross-reacts with serum from a patient with the autoimmune disease scleroderma. A putative nuclear localization signal can be identified in p38. Interestingly, a repetitive amino acid sequence motif begins near the amino terminus of p38. This motif is approximately 80 residues long, is rich in glycine and arginine, and shows striking sequence homology to mammalian nucleolins and certain nucleic acid binding proteins.

[Prolonged intergenesic period. A high risk factor?].

A prospective study is effected in 115 pregnant patients with an intergenesic period (IGP) of 7 years or more and it is compared with an equal number of patients with a minor IGP to this age. From these patients, 86 (74.8%) do not present pathology and 29 (25.2%) present it. These groups are divided in turn in two categories, from 35 years old to more and minors to this age. Differences are not found in the form of beginning parturition, way of this form of membranes breakage, duration of working of parturition and expellant between both independent groups to the maternal age. The perinatal results are alike except a minor average weight in mothers with prolonged IGP children. The patients with pathology were managed according to this, independent to their IGP. In conclusion only patients with prolonged IGP must be managed as high risk, when they present furthermore associated pathology and those patients with prolonged IGP independent to this duration have raised possibilities of success in vaginal parturition in absence of pathology.
