[Effectiveness of psychoanalytically founded brief and dynamic psychotherapy].

OBJECTIVES: This study has been designed to investigate the effectivity of psychodynamic therapies. METHODS: In a naturalistic prospective longitudinal study, 117 patients are compared with 116 untreated probands. RESULTS: The improvement of the general psychosocial impairment of patients of clinical relevance and significance. The effective magnitude of change is high. Untreated patients show on the average no remission of impaired well-being and symptoms six months later. CONCLUSION: Analytically orientated short-term psychotherapy and dynamic psychotherapy are effective in the treatment of many dysthymic and anxiety disorders as well as in the treatment of a number of personality disorders.

[Psychological complaints and disorders of students].

UNLABELLED: Objectives of our investigation are the assessment of the prevalence of psychological disorders of students, the kind of symptoms and complaints and the frequency of treatment required. Secondly it is investigated whether the disorders of the clients consulting a counseling service are clinically relevant. The method has been a multidimensional field study of 346 students at the University of Heidelberg compared with 342 patients of the Psychotherapeutic Counseling Service of the University. RESULTS: 16% of the students assess themselves as severely mentally disordered and in need of treatment. This group shows objectively and clinically significant more frequent and more distinct psychological symptoms. The patients consulting a psychotherapeutic counseling service show the same degree of psychological symptoms as the persons of the control group who assess themselves as being in need of treatment. Depressive syndromes, working disorders, anxieties and interpersonal conflicts are the dominant areas of complaints. In the provision of counseling and treatment for students with psychological disorders, a psychotherapeutic counseling service plays its specific role.

The creative aspect of dynamic psychotherapy: parallels between the construction of experienced reality in the literary and the psychotherapeutic process.

Aesthetic experience and psychotherapeutic practice are compared. Beginning with the assumption that memories, imaginings and feelings are aesthetic categories, it is shown that aesthetic experience is elementary in the shaping and structuring of the experiences of human beings, both in psychotherapeutic communication and in the process of production and reception in literature. The aesthetic shaping of events into integrated experiences in the course of the therapeutic encounter might have a healing effect in its own right. To this extent, the therapeutic process should be regarded both as a joint process of creation and as a process of interpretation. As in writing and reading a literary work of art, the therapeutic process consists of special functions of productivity/poiesis, receptivity/aesthesis, and communicative interaction/catharsis. The psychic mechanism of projective identification may help us understand this emotional and cognitive interchange psychodynamically. Further graphic associations and visual imaginings that arise in the patient and in the therapist are considered, like literary works of art, to function as "transitional objects." These creations of patient and therapist, artist and recipient serve to consolidate formerly nonintegrated and therefore pathogenic experiences. A fruitful psychodynamic work is always situated in the tense relationship between the aesthetic shaping of emotions and the scientific interpretation of inner and outer facts.

[Psychotherapeutic counseling and treatment of students within the scope of a psychotherapy counseling center].

The field of psychotherapeutic counseling in germany is rather heterogeneous and scientifically scarcely investigated. As an example the author describes the organisation and methods of a psychotherapeutic counseling service for students. Statistical material of the kind of complaints and disorders is given and first results of an evaluation study are reported.

[Hermeneutic aspect of analytically oriented (short-term) psychotherapies].

The present paper describes the experience of the "significance" a crisis or psychic disorder can have in the context of analytical (short-term) psychotherapy as a temporal and spatial revelation of inner world by means of--unelaborated--projective and introjective processes. This proceeding is called "hermeneutic" because firstly, a part of the whole, for example a symptom, a phantasy or any personal expression is to be understood in the "here and now" of the therapeutic "Sprachspiel" (Wittgenstein). Secondly, the hermeneutic understanding helps to open the "transitional space" (Winnicott) where the significance which is formed in the listener's imagination responds to the speaker and changes his conception in a continuous--frequently unconscious--dialogic exchange. Hence the "mere past that happened" gains significance in actual understanding. Thus hermeneutics is to be considered both as a methodology of the Humanities and as an art to participate experientially in the manifestations of human life. The therapeutic encounter can be concluded when patient and therapist are sure that the patient can continue to develop the contact to "significant others" and to himself in his inner world without the presence of the therapist.

[Structural correlation of schizophrenic thought and language disorders with delusional perception and variations of intentionality].

This study originated from a phenomenological and speech-act theoretical concept of schizophrenic concretism. An experimental study was performed showing a highly significant lack in the schizophrenic patients' ability to use metaphors correctly. Basing on the interpretation of proverbs, the hypothesis is rejected that false interpretations of schizophrenic patients are due to intermingling of personal conflicts. On the other hand, it could be shown that concretistic interpretations of proverbs represent an avoidance of conflicts. The concepts of "substitution" and "transfer" enabled us to measure pathological concreteness and "deconflictualisation". The differentiation between schizophrenic and nonpsychotic patients was found to be highly significant. In a complementary study it could be shown that the chronic schizophrenics' disability to transfer images of proverbs to an interpersonally relevant context does not differ significantly from that of patients with their first schizophrenic episode. Discussing our empirical findings, we try to show that the concretistic reduction of thought and speech is also a paradigma of delusion. The "incorrigibility" of schizophrenic delusion was seen to be based on reification of verbal signs and metaphors. After trying to show a connection between the concretistic "Lebensform" (Wittgenstein) and the disordered intentionality of schizophrenic patients, pointers towards psychotherapeutic implications are given.

[On the aesthetical evidence in the psychotherapeutic meeting].

The therapeutic function of visual ideas and imaginations is focussed. Reminiscences, dreams and fantasies are not only understood as indications of an underlying truth, which is to be explored. They have an own truth content, which is esthetic. Their creation in the psychotherapeutic encounter unfolds curative affects, beyond analytic exploration of psychodynamic reasons.
