Retinal break type and proliferative vitreoretinopathy in nontraumatic retinal detachment.

In a retrospective study of 1180 consecutive eyes operated for retinal detachment, vitreous traction on the rent was the determining factor for the development of proliferative vitreoretinopathy (PVR). Round, multiple, small holes in equatorial degeneration (retinogenic) and macular holes in which no vitreous traction on the rent was found did not complicate with PVR. Retinal detachment caused by horseshoe or crescent-shaped tears with evidence of vitreous traction (vitreogenic) developed PVR to a variable degree: in 171 (25.8%) senile myopic, 19 (44.2%) senile-myopic aphakic, 23 (20.2%) typical aphakic, and 32 (78.1%) patients with giant tears. We also found that retinogenic retinal detachments affected younger age groups more than did vitreogenic retinal detachments.

Unsuspected mycobacterial endophthalmitis with increased aqueous lactate dehydrogenase levels in a child.

A 12-year-old boy had chronic uveitis resistant to medical therapy OS. Clinical examination and serologic tests did not show any infectious or systemic disease. Cytologic study from aqueous paracentesis revealed atypical cells. Ultrasonography disclosed diffuse posterior chorioretinal thickening. The diagnosis of diffuse retinoblastoma was considered, and an aqueous aspirate was assayed for lactate dehydrogenase. It contained six times the level present in a matching sample of serum. The blind eye was enucleated, and pathologic examination showed a necrotizing granulomatous endophthalmitis containing acid-fast bacilli.

Myopic and nonmyopic aphakic retinal detachment: time interval and location of breaks.

Of 471 retinal detachments occurring in aphakic patients after undergoing intracapsular cataract extraction, we reviewed 318 eyes with uncomplicated aphakia (no vitreous loss). Myopic eyes had equatorial breaks 38% of the time versus 19% for nonmyopic eyes. The time interval between aphakia and detachment was shorter for myopic than for nonmyopic eyes. Our results showed a significant incidence of: (1) equatorial breaks in myopic eyes versus nonmyopic eyes (P less than 0.005), and (2) earlier detachments for myopic eyes versus nonmyopic eyes (P less than 0.005). The presence of equatorial breaks seemed to be associated with earlier retinal detachments.

Retinal telangiectasias associated with longstanding retinal detachment as a prognostic sign.

The authors present 26 cases (28 eyes) of rhegmatogenous retinal detachment, lasting more than 6 months, in which retinal telangiectasias appear in the periphery of the retina as a secondary phenomena, seemingly related to hypoxic factors. A comparison is made with a control group of 25 cases of longstanding retinal detachment taken at random in order to establish statistical differences. Considerations regarding their pathogenesis and their value as a prognostic sign are made.

The influence of new surgical procedures on retinal pathology in aphakia after congenital cataract surgery.

After reviewing the incidence of aphakic retinal detachment more specifically on eyes that have been submitted to surgery for congenital cataracts, the authors make some remarks and discuss the value, the advantages, and the yet unknown risks that the new methods of lens removal may have as regards retinal pathology. Special consideration is given to the balance of the advantages: more complete elimination of the opacities, lower incidence of immediate morbidity, better visual results and postoperative possibilities of retinal examinations; against the, as yet unknown, late retinal complications with new procedures such as ultrasonics, phacoemulsification, pars plana approach for lens removal, etc. Moreover, bearing in mind the typical late onset of retinal detachment in aphakic eyes after congenital cataract surgery with the older methods, such as linear extraction and multiple discussions, special attention is given to this feature as a comparative landmark for a prospective evaluation of recent methods of extracapsular lens surgery.
