Mental health. Making the right links.

Intensive care management has not been widely adopted, partly because of the initial costs involved. Control of inpatient beds is crucial to successful initiatives. Implementing this approach in part of Newport, including appointing a link nurse who liaises with acute and community teams, has led to reduced bed use.

Autoerotic asphyxia: a case report.

Autoerotic asphyxia is the induction of a state of oxygen deficiency to enhance sexual excitement and orgasm. It has been practiced for centuries, and there are many reports of fatalities. The authors report a case of a living participant that shows a number of characteristics different from those of fatalities. The case study also raises doubt about the conventional view that participants have no wish to die. We feel that further case studies of living participants will lead to a greater understanding of the practice and will facilitate treatment.

Psychological effects of colliery closures.

A 'factory closure' type study was conducted by postal survey to assess the impact of the colliery closure programme on individuals. Two collieries were studied, one 'threatened' and one 'unthreatened'. Replies were received from 30% of potential subjects. Miners at the threatened colliery had higher mean GHQ scores and reported greater alcohol consumption and more recent contact with their general practitioners. This study suggests that threat of unemployment and actual unemployment has a significant effect on psychological morbidity, potentially harmful behaviour and use of general practitioners.

Olfactory acuity in the positive and negative syndromes of schizophrenia.

The olfactory thresholds of 46 schizophrenic subjects were measured. This group yielded 11 patients with a stringently defined positive syndrome and 12 with a negative syndrome when rated with the Positive and Negative Syndrome Scale. The negative group had a significantly (p less than 0.01) higher olfactory threshold than the positive group although neither of the groups differed significantly from a control group. Implications of this finding are discussed.

Hypersexuality, fetishism and multiple sclerosis.

Hypersexuality and fetishism appeared in a patient with multiple sclerosis whose MRI scan showed frontal and temporal lesions. The hypersexuality may have been the presenting symptom of the multiple sclerosis.
