[Liposomes in the combined treatment of patients with chronic obstructive bronchitis].

Liposome inhalation was included into intensive combined treatment of 78 patients with chronic bronchitis exacerbation and preasthma. As shown by investigations of respiration, central hemodynamics, lipid and protein metabolism, vitamin E concentration, chemiluminescence before, during and after treatment, liposomes promote elimination of obstruction in the airways, pO2 normalization, correction of impaired lipid and protein metabolism, an increase in vitamin E level in peripheral blood leading to antioxidant system recovery and inhibition of lipid peroxidation. The use of lipin reduces the time of treatment of bronchitis chronics 2-fold. Lipin proved also to display a cardioprotective action.

[The treatment of leptospirosis complicated by severe liver-kidney failure].

The authors examined 16 patients with severe forms of leptospirosis complicated by hepato-renal insufficiency. The treatment program included the following methods: hemodialysis, hemosorption, intraportal administration of drugs, gastrointestinal sorption via a heterospleen, hyperbaric oxygenation. As a result of this complex treatment lethality decreased by 15%.

[Diagnosis and differential therapy of edema and swelling of the brain in patients in critical conditions].

A total of 181 patients found in critical conditions were examined and treated. The use of a complex of neurophysiological studies including brain plethysmography++, rheoencephalography and echoencephalography in patients with encephalopathies made it possible to carry out early and differential diagnosis of brain edema and swelling and to exercise dynamic control over those processes. The basis of the treatment of patients with brain swelling was formed by the drugs that ameliorate microcirculation, cerebral blood flow and brain metabolism. In the treatment of brain edema, use was made of the drugs that normalize blood-brain barrier function and of dehydration therapy.

[The correction of functional disorders of the cardiovascular system in patients with acute kidney failure treated by hemodialysis].

A study is presented of the central hemodynamics in 38 patients with severe forms of acute renal failure of different genesis. Disorders were revealed related both to renal failure and use of active methods of extracorporal detoxication, namely, hemodialysis and hemosorption. Measures on the correction of disorders of the central hemodynamics are described.

[Detoxication in the critical state due to rhabdomyolysis].

An examination of 92 patients with the crush syndrome and the position compression syndrome was made. All the patients were examined by clinico-biochemical methods, the cardiovascular system being also studied. The leading place is occupied by endogenous intoxication. The metabolic disorders were of a total character involving different links of metabolism. It required inclusion of efferent methods in the program of treatment. The most effective program of using efferent methods of treatment was developed on the basis of the state of hemodynamics and clinico-biochemical peculiarities.

[Central and organic hemodynamics during the treatment of infection by splenosorption].

Patients with sepsis have been found to have two types of alterations of central hemodynamics: hyperdynamic and hypodynamic ones, as well as alterations of the hepatic blood flow. The use of splenosorption results in normalization of the parameters of the central and hepatic hemodynamics. The positive hemodynamic effect of splenosorption seems to be associated with the increased nonspecific resistance as well as the activation of the endogenous mechanisms of defense.

[Biological sorption in immunoneurologic diseases].

A study is presented of 17 patients--9 with myasthenia, 8 with the Guillain--Barré syndrome. The complex treatment program of intensive therapy and reanimation included in all patients the operation of extracorporal heterospleen. Forty-one perfusions were carried out. The operation of splenic sorption allowed to reduce treatment time under conditions of intensive care and reanimation 1.5 times, lethality--by 33%. Employment of biological sorption in patients with such immunoneurological diseases as myasthenia and the Guillain--Barré syndrome removes the humoral autoallergic factor and, thus, favours restoration of nervous system function.

[Diagnosis and intensive therapy of obstetric coagulopathic syndrome in parturients in the early post-resuscitation period].

Therapeutic management of 42 parturients with the syndrome of obstetric coagulopathy who had suffered hemorrhagic shock in labor or early postpartum period was analysed. The signs of consumption coagulopathy were revealed in all the examinees. A decrease in the blood fibrinolytic activity observed in the presence of hypocirculation led to a microcirculatory crisis and the development of polyorganic failure. Agents of fibrinolytic activation and disaggregation from the nicotinic acid group or its derivatives, which were given in the postresuscitation period potentiated the therapeutic efficacy and therefore decreased lethality rates.

[The complex diagnosis of brain edema--swelling in patients in critical states].

In an intensive care unit, 181 patients were electrophysiologically investigated (impedance measuring, rheoencephalography, echoencephalography) and ophthalmoscopied, their plasma and red blood cells electrolytes, red blood cells ATP, levels of histamine, serotonin, and monoamine oxidase activity measured and the state of the lipid peroxidation system determined. The combined investigation ensured the early detection of brain edema or swelling and assessment of preponderance of either process. Increase in serotonin, histamine levels and activation of lipid peroxidation system were shown to play an important role in the pathogenesis of these disorders.

[Changes in the function of the cardiovascular system and cerebral circulation in patients poisoned by carbon monoxide and methane].

Altogether 68 patients with carboxyhemoglobinemia and 58 patients poisoned with methane were investigated. Toxic encephalopathy was observed in all of them: I degree of severity--in 13 patients, II degree in 29, III degree in 24, and IV degree in 60. To control CVS function, indices of central hemodynamics were investigated using methods of whole-body rheography and ECG; a volumetric rate of cerebral circulation was determined. A decrease in volumetric cerebral circulation was noted in patients with encephalopathy, III-IV degree, by 37.5% (carboxyhemoglobinemia) and by 26.1% (methane poisoning). Central hemodynamic disorders were more noticeable in carboxyhemoglobinemia. Multimodality intensive therapy included HBO and drugs to restore cerebral circulation, microcirculation in the brain, cerebral metabolism, and CVS function. Of 126 patients CNS functional restoration without neurological deficiencies was noted in 108 patients, neurological and psychic deficiencies were noted in 9 patients; 9 patients died.
