[A new approach to raising the efficiency of drug therapy for erectile dysfunction].

AIM: To try combined treatment of erectile dysfunction with viagra and alprostadil in case of failure of their monotherapy; to compare effectiveness of viagra in dynamics of treatment, in various doses and dose adjustment. MATERIAL AND METHODS: 82 patients with ED of different genesis received a course of intracavernous injections of alprostadil followed by a course of viagra; 25 patients received combined treatment with alprostadil and viagra. Each course lasted for 3 months. Viagra efficiency was also assessed in long-term use (12 months) and different initial doses (50 or 100 mg). RESULTS: Monotherapy with alprostadil or viagra was effective in 73.2 and 75.6% patients, respectively. Their combination was more beneficial--88.0%. When used for a long time, viagra lost efficiency in psychogenic ED by 17.7%, in organic ED--by 16.9%. In an initial viagra dose 50 mg efficiency reached 70.3%, 100 mg--80.0%. CONCLUSION: Combined treatment of ED is a method of choice in monotherapy failure and in severe ED. Lowering of viagra efficiency in long-term administration may be explained by disappearance of placebo effect.

[Sildenafil and alprostadil in the combined drug therapy of erectile dysfunction].

Forty-four patients with erectile dysfunction (ED) aged 21-72 years aged 21-72 years (mean age 61 years) were examined and treated with sildenafil and alprostadil monotherapy or combined therapy. ED was psychogenic in 9(20.5%), arterial in 12(27.2%), vein occlusive in 9(20.5%) and neurogenic in 14(31.8%) patients. Monotherapy was most effective in psychogenic ED (alprostadil--100%, sildenafil--88.9%), least effective in vein occlusive ED (alprostadil--33.3%, sildenafil--22.2%). Alprostadil was more effective in arterial and neurogenic ED (83.3 vs 66.7 and 78.6 vs 57.1%, respectively). Combination of the two drugs produced much high response: 100, 85.7 and 55.5% in arterial, neurogenic and vein occlusive ED, respectively. Thus, combined treatment with sildenafil and alprostadil is a method of choice in the treatment of ED in failure of monotherapy with these drugs or in vein occlusive ED. In the combined treatment dose of the drugs, number of side effects and cost of therapy are lower.

[An open non-comparative study of impase for treating erectile dysfunction].

Clinical efficacy and safety of the drug impase in erectile dysfunction (ED) were studied in 30 patients with ED of various genesis (mean age 52.09 +/- 4.75 years). IIED questionnairing was employed, combined andrological examination and standard laboratory investigations. Impase was taken 1 tablet a day each other day for 12 weeks irrespective of the intercourse. The number of attempted intercourses was at least 4 times a month. The treatment efficacy was assessed by changes of IIED parameters, subjective and objective effects. ED was psychogenic in 5 (16.7%) patients, organic in 25 (83.3%) patients. Blood hormones were normal for age in all the patients. According to IIED, "erectile function" mean score after 12 week treatment rose from 18.89 +/- 5.25 to 22.50 +/- 4.86. In 11 (36.7%), 4 (13.3%), 5 (16.7%) patients the response was rated as excellent, good and satisfactory, respectively. No response was stated in 10 (33.3%) patients. Side effects were absent. Tolerance and safety were excellent. Thus, efficacy of impase therapy varied from 60 to 66.7% depending on ED etiology and pathogenesis. Cost-efficacy, absence of side effects, possible parallel therapy of concurrent diseases, course administration are advantages of the treatment.

[Viagra, cialis, impase--which of them, to whom, when and how?].

The study has been performed of the efficacy in the treatment of erectile dysfunction (ED) of oral drugs affecting nitric oxide: impase and phosphodiesterase-5 (PDE-5) inhibitors--sildenafil citrate (viagra), tadalafil (sialis)--alone and in combination with impase. A total of 218 ED patients aged 21-73 years (mean age 58.1 +/- 13.2 years) were divided into 3 groups comparable by the number of the patients, age, suspected etiology, pathogenesis and ED severity. Group 1 (n = 81) took viagra in the individually adjusted dose for 6 months; group 2 (n = 64) received sialis in a dose 20 mg for 6 months; group 3 (n = 73) took impase 1 tablet each other day sublingually for 6 months. Overall efficacy made up 77.8, 81.3 and 56.2% for viagra, sialis and impase, respectively. In view of different mechanism of action of PDE-5 inhibitors (viagra, sialis) and impase we combined the drugs in those who failed monotherapy or had drastic side effects. The combination raised efficacy of pharmacotherapy from 56.2 to 92.2%. We came to the conclusion that in psychogenic, isolated neurogenic, compensated and subcompensated arteriogenic ED of a mild or moderate degree, the treatment can be started with impase. If it was uneffective, in severe ED or moderate venoocclusive ED it is better to use PDE-5 inhibitors (viagra, sialis). If one of the latter fails, the other should be administered. If the inhibitors have low efficacy or in side effects, it is indicated to use their combination with impase.

[Levitra-test in diagnosis of vasculogenic erectile dysfunction].

Dopplerography in artificial erection induced by intracavernous injection of drugs is now the most common method of diagnosis in examination of patients with erectile dysfunction (ED). We studied possibility of inducing erection by the novel potent and selective oral drug levitra (vordenafil) from the group of inhibitors of phosphodiesterase of the 5th type. The comparison of the dopplerographic results of two methods (erection induced by intracavernous injection of prostaglandin E and levitra) showed no statistical differences between the two methods. Thus, levitra-test is a highly informative technique in diagnostic examination of patients with erectile dysfunction with the same pharmacodopplerographic efficacy as intracavernous injection of vasoactive drugs.

[Prediction of clinical efficacy of levitra (vardenafil) in patients with arteriogenic erectile dysfunction].

Oral inhibitors of phosphodiesterase of type 5 (PDE-5) are drugs of choice in therapy of erectile dysfunction (ED). However, 20-30% patients do not respond to them. We studied potentialities of ultrasonic investigation (UI) of postcompression increase in cavernous arteries diameter (PICAD) after intake of vardenafil (levitra, Bayer&GlaxoSmithKline) which is a PDE-5 inhibitor in prediction of clinical efficacy of this medicine. A total of 75 males with ED participated in the study. Significant correlations were found between PICAD after vardenafil administration and clinical efficacy of the drug in patients with arteriogenic ED. The highest response to the drug was observed at PICAD value > or = 50%. High sensitivity and specificity of the PICAD threshold value obtained indicate high potentialities of the method in decision making on treatment policy in patients with organic ED.
