[Effect of the morphological structure of lung cancer on the incidence and type of regional metastases].

A correlation between the frequency, nature of metastatic spreading and histological pattern of lung cancer was established on the basis of a study of 2,107 cases of morphologically confirmed cancer, 1,251 specimens of resected lung tissue and 145 exploratory thoracotomies. Poorly-differentiated cancer of the lung was found to involve frequent formation of metastases at all stages. Metastases generally occur as multiple lesions of lymph nodes.

[Relation between the extent of resection of cutaneous melanomas and the incidence of local recurrence].

Under study was the influence of the volume of a dissected primary tumor, the method of plasty of the wound defect and anesthesia upon the incidence of local recidivations of cutaneous melanoma in 1334 patients. Free skin plasty was shown to be necessary in cases of the surgical treatment of cutaneous melanoma excepting localization of the primary tumor on the trunk. Parsimonious excision of the primary tumor and local infiltration anesthesia during operation are thought to be the factors responsible for the development of local recidivation of skin melanomas.

Anesthesia, General↗

[Melanoma of the skin of the concha auriculae].

The results of treatment of 25 cases of malignant cutaneous melanoma of the floor of the auricle are presented. The involvement of the central area of the organ was matched by the least favorable clinical course. All patients of the latter group developed regional metastases. Therefore, a simultaneous removal of primary tumor and fascial-capsular excision of regional lymph nodes should be carried out unless the lymphatics are involved. The involvement of lymphatics makes the case for Crile's operation. In cases of peripheral position of melanoma, the approach to lymph node treatment should consider such factors as tumor invasion degree, growth pattern, histological type, patient's age and condition.

[Evaluation of the treatment tactics in patients with stage II malignant neoplasms].

The paper deals with an analysis of the schemes of treatment of patients suffering from malignant tumors of the intestinal and respiratory tract, breast, cervix and corpus uteri in different Union Republics of the USSR within 1970-1980. This period witnessed noticeable improvements including increased effectiveness of the said schemes, extension of indications for surgery and development and integration of new drugs with practice.

[Growth rate as one of the characteristics of peripheral lung cancer].

A correlation between the rate of growth of peripheral cancer of the lung and patient's age was established by a study involving 262 cases. The duration of tumor presence in the body appeared to be a factor influencing the process of tumor dissemination to regional lymph nodes. An analysis of the long-term results of surgical treatment of 127 patients yielded a significant correlation between the time of doubling of tumor size and survival within the first two years after operation. At later stages, survival rates tended to rise in patients with slowly-growing tumors, with the exception of those with the tumor doubling time under 20 days and a bad prognosis.

[Effect of the morphological structure of lung cancer on the choice of surgery].

The authors compare the character of surgical interventions in 1519 patients with the morphologically verified pulmonary carcinoma with its histological structure. It was found that underdifferentiated forms of cancer were characterized by the decreased operability and resectability, increased volume of operations and number of exploratory interventions.

[Preliminary results of a cooperative study of the efficacy of combined treatment of lung cancer].

The clinical data provided by the members of a CMEA joint study on the problem of Lung Cancer (Work group 3--9.1.1) are analysed. The study undertaken by the researchers of Bulgaria, Hungary, Poland, USSR and Czechoslovakia was concerned with evaluation of the effectiveness of a modality of combined treatment of well--differentiated cancer of the lung. The study group consisted of 286 patients; 231 surgically treated patients were in control. It was found that preoperative intensive concentrated radiation treatment did not interfere with surgical procedures. Nor did it specifically involve any intraoperative complications or change their frequency. However, combined treatment (preoperative irradiation + surgery) was followed by a relatively high mortality and a significant increase in the frequency of postoperative complications, especially purulent ones, such as bronchial fistula and empyema of the pleural cavity. Maximal difference in mortality rates between the groups was observed in cases of stage III cancer. The long-term results were studied in 379 cases (221 patients after combined treatment and 158--after surgery). 76.5 and 77.2, 49.4, and 41.2, and 34.4 and 20.7% of patients survived over one, three and five years, respectively. Better results registered in the study group were largely due to cases of stage III squamous cell carcinoma.

Adult↗

[Lung cancer in patients over 70].

The validity of the surgical treatment has been established by the investigation of immediate and late results of the treatment of 118 patients with lung cancer aged from 70 to 84. The postoperative lethality was 17,8%, the 5-year survival being 25,8% of the patients discharged from the hospital. No biologically specific features of lung carcinoma in patients older than 70 were found which could promise longer life of such patients without surgery.

[Treatment of patients with lung cancer who had suffered acute myocardial infarction].

The study was concerned with a comparison of 64 cases of lung cancer with concomitant postinfarctional cardiosclerosis, treated by surgery, radiation and chemotherapy, with similar patients who had not suffered from myocardial infarction. It was found that infarction patients may be given one component of combined treatment or a combination of two (radical surgery followed by a course of postoperative radiation treatment in a dose of 5,000 rad), if required. The long-term results of treatment of patients of both groups were similar.

Three-year results of surgical and combined treatment of lung cancer with a preoperative radiotherapy.

In this paper the analysis of immediate results of clinical material is given which has been accumulated by participants of the joint investigation in some CMEA member countries (Project 3-9.1.1 of the Lung Cancer Program; Bulgaria, Hungary, Poland, USSR, Czechoslovakia) with the aim to study the effectiveness of one of the variants of combined treatment of patients with differentiated forms of lung cancer. Combined treatment received 286 patients, 231 patients (control group) were treated surgically. It is concluded that the preoperative radiotherapy using the method of the intense concentrated course does not complicate the operation itself and is not reflected in the nature and frequencies of intraoperative complications. However, in combination with the subsequent surgical intervention, it significantly elevates the number of postoperative complications, particularly of those of purulent nature (bronchial fistula, intrapleural empyema) and also the lethality. The difference between the groups in this respect is particularly significant at State III of the disease. Late results were studied in 379 patients (in 221 of them after a combined treatment and in 158 after surgical treatment). For more than 1, 3 and 5 years survived 76.5, 49.4 and 34.48% (combined treatment) and 77.2, 41.2 and 20.7% (surgery), respectively. The differences between the groups in favor of the combined treatment are due to patients with Stage III planocellular carcinoma.

[Correlation of the level of humoral cytotoxins and delayed hypersensitivity skin reactions with survival of patients with melanoma of the skin].

The study was concerned with relationships between blood-humoral cytotoxin level, time-delayed hypersensitivity skin reaction to DNCB and 3- and 5-year survival in skin melanoma patients. Humoral cytotoxin level proved to be irrelevant for prognosis, although this index may be sometimes used as a supplementary criterion for evaluating immunocompetence. Skin reactions to DNCB provide a valuable prognostic means in skin melanoma patients: 3- and 5-year survival rates are significantly higher in patients showing positive reaction than in those with negative reaction.

[Characteristic clinical manifestations of lung cancer in women].

In women lung cancer was found to have a latent development much more often than in men. An analysis of the duration of the period from the appearance of clinical symptoms of lung cancer up to the medical consultation has shown that duration of the medical history in women was longer than that in men. An earlier admittance to the hospital of men with lung cancer can be explained by an earlier appearance of clinical symptoms making the patient seek medical advice.

[Prospects of improving the early diagnosis of lung cancer in women].

Insufficient alertness of general practitioners resulting in diagnostic errors (47%) and latency of the disease (28.8%) are chiefly responsible for advanced cases of lung cancer among females. Improvement in the techniques of early diagnosis of lung cancer promises particularly good results for mass X-ray screenings of females since peripheral cancer is predominant in them. Lung cancer was detected at early stages in 78.7% of females and only in 50.5% of males. Early detection of the disease had a beneficial effect on the course of surgical treatment of lung cancer in female patients. Operable cases among them were 68.9% (v. 38.7% in males) and saving operations were performed in 73.8% of females and 60.2% of males.
