Complications and side effects associated with large-bore catheters in the subclavian and internal jugular veins.

Since the introduction of large-bore catheters for acute hemodialysis 30 years ago, many problems with handling, material, and contamination of these catheters exist. Nevertheless, catheterization of the inferior and superior vena cava with a large-bore catheter has proved to be suitable as a rapid connection process for hemodialysis, hemofiltration, hemoperfusion, plasmapheresis, plasmaperfusion, among others. In a retrospective study with 2,741 large-bore catheters in 1,716 patients, the frequency of infections, thrombosis, bleeding, and other side effects was investigated. All complications and side effects are presented dependent on vascular access route. In total, the complication rate was 48.9% higher in subclavian puncture than in internal jugular puncture (24.8%). The highest complication rates for both vascular access routes were infections or septicemia; infections were observed in 19.5% of subclavian catheters versus 10% of internal jugular catheters.

Elimination of an artificial immune complex by plasmapheresis in rabbits.

The behaviour of an artificial immune complex was investigated in 15 rabbits. The immune complex was labelled with iodine-125 (125I). The advantage was that the immune complex could not be metabolized without being eliminated by the kidney. This artificial immune complex was injected in different dosages (0.8 micrograms-40 micrograms/rabbit) on 2 consecutive days. One group (n = 9) was treated with plasmapheresis: the other (n = 6) was a control group that did not undergo plasmapheresis. A miniaturized plasmapheresis system eliminated the immune complex very effectively. The increasing concentration of the immune complex in the blood before the next plasmapheresis treatment was probably an expression of mobilization of the immune complex from different organs. The animals of the plasmapheresis group were in better condition than the control group.

The behavior of an artificial immune complex was investigated in 15 rabbits. The immune complex was labeled with iodine-125 (125I). The advantage was that the immune complex could not be metabolized without being eliminated by the kidney. This artificial immune complex was injected in a dosage of 0.8 microgram, in 2 rabbits, 5.0 micrograms in 4 rabbits, 10 micrograms in 3 rabbits, 20 micrograms in 2 rabbits, and 40 micrograms in 4 rabbits on two consecutive days. One group (n = 9) was treated with plasmapheresis: the other (n = 6) was a control group that did not undergo plasmapheresis. A miniaturized plasmapheresis system eliminated the immune complex very effectively with an average exchange volume of 95.7 ml per treatment. The increasing concentration of the immune complex in the blood before the next plasmapheresis treatment was probably an expression of mobilization of the immune complex from different organs. The animals of the plasmapheresis group were in better condition than the control group.

The 'female prostate': location, morphology, immunohistochemical characteristics and significance.

Using immunostaining for prostate-specific antigen (PSA) and prostatic acid phosphatase (PAP), we can demonstrate prostate-analogous periurethral glands (the 'female prostate') in 66.7% of 33 cases. Histomorphologically, these glands resemble strongly the male prostate glands before puberty. They remain immature throughout life from the fetal period up to advanced age obviously because of a lack of an androgenic stimulus. The glands have long ducts leading into the urethra. A proper stroma component is missing. The immunohistochemical behavior of these glands also corresponds widely to that of the male prostate glands before puberty. No indications can be found for a proper biological function.

MRI and pathological findings in two cases of Askin tumors.

Askin tumors are primitive peripheral neuroectodermal tumors (PPNET) located in the thoracopulmonary region. This entity was first proposed in 1979 by Askin et al. These highly malignant tumors occur primarily in children and young adults. Pre- und postoperative MRI findings are presented for two pathologically proven cases. MRI is the most appropriate imaging modality for the diagnosis and eventual follow-up for these tumors. They appear homogenous, and iso- or discretely hypointense in comparison to the spinal cord on T1-weighted images and hyperintense on proton density and T2-weighted images. They show a very stark contrast enhancement after i.v. injection of paramagnetic contrast agents. Sharp tissue borders, and exact tumor extension were shown in both cases. The high signal intensity on T2-weighted images was not altered by chemotherapy.

Morphological and immunohistochemical investigations of the utriculus prostaticus from the fetal period up to adulthood.

We investigated the utriculus prostaticus from the fetal period up to adulthood in 148 prostates. During the second half of gestation the utriculus had a simple tubular or a cystic form and was lined with metaplastic squamous epithelium which showed immunohistochemical positivity for different keratins, carcinoembryonic antigen, and peanut agglutinin binding sites. After birth, alveolar outgrowths of the utriculus developed. After puberty, the utriculus had become a complicated and variable structure. The epithelium no longer differed from that of the prostate glands either morphologically or immunohistochemically. Within the epithelium numerous endocrine cells were found containing neuron-specific enolase, chromogranin, and serotonin. The utriculus and ejaculatory ducts were embedded in a fibrous stroma with, after birth, numerous plexus-like blood vessels. This fibrous zone was peripherally bordered by a layer of smooth muscle. There was no evidence for a function of the utriculus differing from that of the prostate glands. Since the epithelium of both structures is identical immunohistochemically, the epithelium of the sinus urogenitalis most likely particpates in the lining of the utriculus during embryogenesis.

[Malignant hemangiopericytomas in the head and neck area].

Since 1975 we have treated nine patients with a malignant haemangiopericytoma. Three patients died 5, 9 and 14 years later of metastasis in the lung, liver, skeletal system and mastoid. However, five of the patients are still alive. Regular clinical and radiological follow-up is performed, the average period of follow-up being 8.2 years. Four patients suffered local recurrence within 1-5 years. Surgical resection was the primary therapy in five cases, supplemented in two cases by local radiation. Recurrences were treated by surgery on four occasions and six by radiation, but no complete remission was achieved. Chemotherapy (CyVADic regimen) was attempted in three patients with metastases and one patient with a huge local recurrence, but proved to be ineffective. A re-examination of the histological specimen of all primary tumours and recurrences did not reveal a correlation between histological appearance and clinical behaviour. Our experience in the treatment of malignant haemangiopericytoma and the data in the literature suggest that primary resection with a wide safety margin is the therapy of choice. Radiotherapy is recommended for incomplete resection, local recurrence, and metastases and inoperable tumours. Since recurrence and metastasis can occur after many years (up to 7 years in our patients) a lifelong regular follow-up is necessary.

Hemophagocytic syndrome. Differential diagnostic aspects in a case of well-differentiated malignant histiocytosis.

Diagnosis of malignant histiocytosis (MH) often resembles the solving of an intricate puzzle consisting of clinical symptoms such as lymphadenopathy, splenomegaly, fatigue, fever, and rapid progression, and hematopathological findings such as the presence of atypical histiocytes, especially in blood and bone marrow smears. The lack of one or more of these criteria may greatly impede diagnosis, as in the case of a 45-year-old male with an unusual hematopathological manifestation of MH. The major clinical findings included panhemocytopenia, splenomegaly, and signs of liver dysfunction with severe jaundice. During life, a definite diagnosis could not be established. Histological and cytological evaluation of the spleen following splenectomy revealed a marked increase in histiocytes/macrophages with pronounced hemophagocytosis. These findings were interpreted as a (benign) hemophagocytic syndrome, possibly related to a viral infection. Extensive serological investigations, however, furnished no evidence of a so-called virus- or infection-associated hemophagocytic syndrome. The patient died 5 months after the onset of disease with symptoms of progressive liver failure. Meticulous histological examination of bone marrow revealed a few patchy tumorous infiltrates consisting of dense pleomorphic histiocytes. Thus, a diagnosis of MH was established. This case of MH was unusual with particular regard to its pronounced hemophagocytosis, slight cytological atypia of the histiocytes, and absence of infiltration of lymph nodes.

[Thymoma--a retrospective study of 48 cases].

Of the 48 thymomas operated on between 1968 and 1985 50% were associated with myasthenia gravis, in 25% diagnosis was made due to accidental X-ray findings. In the staging according to Masaoka et al. [10] the following distribution was noted: I:22, II:5, III:18, IV:3. The 5-year survival rate of the 37 curatively resected patients was 78.5% (stage I: 88.6%, stage III: 55.6%). No stage III patient with residual tumor survived more than 2 years. Predominantly epithelial tumors had a significantly worse prognosis as compared to those with lymphocytic predominance. None of the 4 patients with category II thymoma survived more than 15 months. Association with myasthenia proved to have a negative prognostic influence in stage I and II patients. Frequency of local recurrences and metastases is documented. The value of irradiation and chemotherapy is discussed.

Surface treated large bore catheters with silver based coatings versus untreated catheters for extracorporeal detoxification methods.

Infection, thrombosis, and stenosis are among the most frequent complications associated with blood contacting catheters. Because these problems are usually related to surface properties of the base catheter material, surface treatment processes, such as ion implantation and ion beam assisted deposition (silver based coatings), can be used to mitigate such complications. Because these ion beam based processes affect only the near-surface region (approximately the outer 1 microm), there is little effect on bulk material properties. This study evaluated silver coated large bore catheters used for extracorporeal detoxification. In a 122 patient prospective study, 156 large bore catheters were inserted into the internal jugular or subclavian veins. Seventy-eight surface treated catheters (SPI-ARGENT, Spire Corporation, Bedford, MA; n = 32 acute catheters, n = 45 long-term catheters) were Bambauer inserted in 55 patients. Seventy-eight untreated catheters placed in 67 patients served as controls (n = 35 acute catheters, n = 43 long-term catheters). After removal, the catheters were cultured for bacterial colonization using standard microbiologic assays. They were also examined using a scanning electron microscope (SEM). Bacterial colonization was observed in 15.4% of the treated catheters compared with 44.9% of untreated catheters. The SEM investigations showed all treated catheters to possess low thrombogenicity. Results of the study indicate that ion beam based processes can be used to improve thrombus and infection resistance of blood contacting catheters.
